1. I Probiotici e il loro utilizzo nello sport di elite

2. La letteratura internazionale concorda nel ritenere che le infezioni delle
vie aeree superiori e le gastroenteriti infettive, sono, insieme alle
infezioni cutanee la principale problematica degli atleti in trasferta.
(Ahmadinejad Z. et al., 2015).
Atleti di Elite ed infezioni
- Le infezioni delle vie aeree superiori, sono 4 volte più frequenti negli
atleti rispetto ai sedentari e colpiscono di più gli atleti di endurance
con un incidenza che si attesta anche fino al 30%.
(Grosset-Janin A. et al., 2012).
- Le infezioni gastrointestinali, sono piuttosto diffuse e costituiscono
un rischio di contagio degli altri atleti e degli altri membri dello staff,
nelle trasferte, nonché un possibile rischio di insorgenza di epidemie
nell’ambito di una manifestazione sportiva internazionale.

3. Esercizio fisico ed immunità
L'esercizio fisico moderato ha effetti positivi sul sistema
immunitario, riducendo così il rischio di infezioni. Al
contrario, l'esercizio fisico intenso, lo stress emotivo ed il
sovrallenamento, esercitano effetti negativi sul sistema
immunitario aumentando il rischio di infezioni.

4. Effetti dell’attività fisica intensa
- Granulociti neutrofili: riduzione burst ossidativo
- Linfociti: riduzione perforina
- NK: attività ridotta dal 40 al 60%
- Linfociti: riduzione rapporto CD4+/CD8+

10. Lactobacillus fermentum (PCC®) supplementation and
gastrointestinal and respiratory-tract illness symptoms: a randomised
control trial in athletes.

Competitive cyclists (64 M and 35 F)
1)Supplementing with L. fermentum attenuated acute exercise-
induced changes in both anti- and pro-inflammatory cytokines (IL-
1RA, IL-6, IL-8, IL-10, GM-CSF, IFN-γ, TNF-α)
2)There were no substantial differences in the concentration of the
mucosal immune measures lactoferrin, lysozyme or SIgA between the
probiotic and placebo groups
3)Supplementation with L. fermentum was associated with a reduction
of symptoms in clinical indices of lower respiratory illness, GI
symptoms at high training loads, and cold and flu medication use in
well-trained male cyclists. These outcomes were not evident in well-
trained females

saliva IgA; and lysozyme concentrations did not change over the
course of the study and were not different on PRO compared with
PLA.

13. Probiotic supplementation reduces the duration and incidence of
infections but not severity in elite rugby union players.

1) During the probiotic treatment 14/30 participants never experienced
a single upper respiratory tract illness (URTI) or gastrointestinal (GI)
episode, compared to 6/30 on the placebo supplementation.
2) The number of days of illness tended to be higher for the placebo,
(5.8±6.6 days) than probiotic (3.4±4.6 days).
3) There was no significant difference in the severity of the symptoms
between the two treatment groups.

14. Exercise, intestinal barrier dysfunction and probiotic supplementation.

1) Some results demonstrate promising benefits for probiotic use on
the athlete's immune system.
2) There is also evidence that probiotic supplementation can
beneficially influence intestinal barrier integrity.
3) There is still a lack of studies with appropriate data and a gap to
understand the underlying mechanisms to support such health
beneficial statements.

Several studies conducted since 2006 examining probiotic
supplementation in athletes or highly active individuals indicate
modest clinical benefits in terms of reduced frequency, severity and/or
duration of respiratory and gastrointestinal illness.

Immunological aspects of sport nutrition.

1) Vitamin D may be a particular concern as recent studies have
emphasised its importance in limiting infection episode incidence and
duration.
2) There is only limited evidence that individual amino acids, β-
glucans, herbal extracts and zinc are capable of boosting immunity or
reducing infection risk in athletes.
3) The ingestion of carbohydrate during exercise and daily
consumption of probiotics, vitamin D3, currently offer the best chance
of success, particularly for those individuals who are prone to illness.

16. CONCLUSIONI
1) Ceppi utilizzati nei diversi studi:
Lattobacilli: L. acidophilus, L. casei, L. rhamnosus, L. salivarius, L.
plantarum.
Bifidobatteri: B. animalis, B. breve, B, infantis, B. longum, B.
adolescentis, B. lactis, B. bifidum.
2) Studi molto spesso di piccole dimensioni e corti follow up.
3) Discipline diverse.
4) Pochi studi hanno messo in correlazione i risultati clinici con
valutazioni analitiche ed immunologiche.
5) Nessuna indicazione che l’utilizzo di probiotici possa influire sulle
prestazioni.
6) Il probabile beneficio clinico deve essere validato.
