2. Overview
Digoxin is considered as a prototype cardiac glycoside, combine with
a steroid nucleus with an unsaturated five membered lactone ring at
the 17th position and a series of sugar linked to carbon 3 of
nucleus, containg three molecule of digitoxose linked to digitoxigenin.
Digoxin is obtained from the leaves of Digitalis lanata.
3. Characteristics
Digoxin also known as Digitek. It is of natural origin and belongs to
glycosides.
Digoxin is Anti arrhythmic agent.
The molecular weight of Digoxin is 780.90
Its pKa is unionized.
4. Mechanism of Action
Digitalis compounds are potent inhibitors of cellular Na/K-ATPase.
This ion transport system moves sodium ions out of the cell and
brings potassium ions to the cell.
5. Indication
Digoxin is primarily indicated in condition like acute left ventricular
failure, Atrial fibrillation, Atrial flutter, chronic left ventricular failure,
congestive heart failure, Supraventricular arrhythmias,
Supraventricular tachycardia.
6. Contraindication
Digoxin is contraindicated in condition like heart block, obstructive
cardiomyopathy, wolf-Parkinson- white syndrome, congestive
cardiomyopathies, constrictive pericarditis, Aortic valve disease.
7. Side Effect
Digoxin give rise to further complication include Ventricular
Tachycardia, Ventricular fibrillation, Supraventricular extra beats,
heart block, Rhodopsia,Cyanopsia,Increased estrogen, Decrease LH,
Decrease Testosterone and color vision disturbance.
