Haemostasis is very important in laparoscopic surgery. Vessel sealing with energy devises play a major role in keeping the surgical field clear. Energy devices are also used for tissue sealing and transection. Despite never types of energy devises electro-surgery is still very popular in gynaecological laparoscopy. Desiccation, dissection, and coagulation are the main effects of electro-surgery that are used for various purposes. Higher thermal injury with monopolar devices lead to the invention of bipolar devices with less tissue damage. Ligasure, pk gyrus, ENSEAL are some of the more advanced bipolar devices. Ultrasonic devices have the capability of coagulation and cutting tissues. During the process it can produce significant thermal injury. Thunderbeat combines bipolar and ultrasonic energy for coagulation and cutting respectively for more precise effects. Laser devices emit a beam of photons with a high degree of spatial and temporal coherence with tissue effects depending on the time of exposure and power density. CO2, Argon, Nd: YAG, KTP-532 are different laser types with different properties. Plasma is the fourth state of matter following solid, liquid and gas. Argon neutral plasma (System 7550TM ABC, Cardioblate) can produce energy in 3 forms including light, heat and kinetic energy. Laser and plasma energy are gaining more popularity for endometriosis surgery due to its localised effects and better preservation of ovarian follicles.
Instruments that apply energy to cut, coagulate and dissect tissue with minimal bleeding facilitate surgery. The improper use of energy devices may increase patient morbidity and mortality.
With growing industrialization and mechanization of every household electrical injuries are becoming quite common. Electrical injuries are quite intricate with the damage caused. They cause not only external burns injury but a wide spectrum of visceral injuries which in many cases is difficult to diagnose and manage. Therefore, a sound understanding of the engineering aspects is pivotal in diagnosing and managing these cases. A brief review of the pathophysiology and management of electrical injuries is presented.

Electricity is the backbone for modern industrialization, development and day to day needs. Electrical Injuries are among the most devastating of burn injuries. High voltage electrical injuries result in extensive deep tissue damage and are associated with multiple complications, long term morbidity and a high mortality rate. The passage of electric current when passes through the body, it produces wide range effects, varying from insignificant localised spasm, little or no contact burns, fatality with little or no burns or extreme severe burning. We describe the case of a 35-year-old male driver who suffered high voltage electric injury of anterior abdominal wall and left upper limb due to accidental contact with high tension wire. This paper also emphasizes the safety measures to be taken at work place pertaining to high voltage cable.
Dissection is defined as the separation of tissues with hemostasis. It consists of a sensory visual and tactile component, an access component involving tissue manipulation, and instrument maneuverability.
We all have good and bad thoughts from time to time and situation to situation. We are bombarded daily with spiraling thoughts(both negative and positive) creating all-consuming feel , making us difficult to manage with associated suffering. Good thoughts are like our Mob Signal (Positive thought) amidst noise(negative thought) in the atmosphere. Negative thoughts like noise outweigh positive thoughts. These thoughts often create unwanted confusion, trouble, stress and frustration in our mind as well as chaos in our physical world. Negative thoughts are also known as “distorted thinking”.

2024.06.01 Introducing a competency framework for languag learning materials ...Sandy Millin
http://sandymillin.wordpress.com/iateflwebinar2024
Published classroom materials form the basis of syllabuses, drive teacher professional development, and have a potentially huge influence on learners, teachers and education systems. All teachers also create their own materials, whether a few sentences on a blackboard, a highly-structured fully-realised online course, or anything in between. Despite this, the knowledge and skills needed to create effective language learning materials are rarely part of teacher training, and are mostly learnt by trial and error.
Knowledge and skills frameworks, generally called competency frameworks, for ELT teachers, trainers and managers have existed for a few years now. However, until I created one for my MA dissertation, there wasn’t one drawing together what we need to know and do to be able to effectively produce language learning materials.
This webinar will introduce you to my framework, highlighting the key competencies I identified from my research. It will also show how anybody involved in language teaching (any language, not just English!), teacher training, managing schools or developing language learning materials can benefit from using the framework.

2. INTRODUCTION
Cauterization is a medical practice or technique of burning a part of
body to remove or close of a part of it.
Cauterization is when electric current is passed through a metal
instrument called an electrocautery device. The device can be in the
form of a hand held pencil-like instrument or a larger instrument with
interchangeable tips. The tip of the device is heated by the electric
current, which causes the tissue to coagulate, sealing blood vessels
and preventing bleeding.
3. DEFINITION
“Electrocautery, also known as thermal cautery, refers to a
process in which a direct or alternating current is passed
through a metal wire electrode, generating heat to cut or
coagulate tissue”
4. type of thermal cautery
• In this technique, a single active electrode is
used to deliver the electric current to the
tissue. The current flows from the active
electrode through the tissue to a large
neutral electrode placed elsewhere on the
patient's body.
monopoler
cauterization
• : In bipolar cautery, two electrodes are used,
both of which are placed in close proximity to
each other. The electric current flows
between these two electrodes, heating and
cauterizing the tissue in between.
Bipoler
cautrization
5.
6. Indication
1. they use electricity or chemicals to burn tissue in order to close a
wound.
2. Cervical erosion.
3. Some malignant condition .
4. Chronic cervicitis.
5. endometrial ablation
6. in many surgical procedure.
It may also be done to remove harmful tissue
7. Contraindications
1. Patient with electrical pacemaker or other implant electrical
devices.
2. Pregnant women.
3. Area near vital structure.
4. patient with compromise wound healing.
5. hypersensitivity
8. PROCEDURE
1. Prepare the patient
2. Local or general anesthesia
should be provided
3. Placement of grounding pad
on the patient body
4. Selection of Cautery machine
1. To reduce risk of infection
2. To ensure pt comfortable
during procedure
3. To complete the electric
current
4. For specific surgical
procedure or desired
outcome
Procedure rational
9. 5. Application for electric current
6. hemostasis and tissue
manipulation
5. The electric current generate
heat that coagulate blood vessels
and , cut tissue
6.helps in achieving hemostasis
by sealing blood vessels and
preventing bleeding. It also
allows the surgeon to manipulate
tissue and achieve the desired
surgical outcome, such as
removing a tumor or cauterizing
a bleeding vessel.
10. 7. Monitoring and control: 7. Throughout the procedure, the
surgeon carefully monitors the
tissue response to the electric
cautery. They adjust the settings
and technique as needed to
ensure precise and controlled
tissue manipulation.
12. NURSING MANAGEMENT
1. Pre-operative care:
i. Nurses should ensure that the patient is adequately
prepared for the procedure.
ii. This includes obtaining informed consent, verifying the
patient's identity and surgical site, and providing pre-
operative instructions such as fasting guidelines.
13. 2. Intra operative care:
i. Nurses play a crucial role in assisting the surgeon during the
procedure.
ii. They should ensure that the necessary equipment and supplies are
available, including the electric cautery device.
iii. Nurses should also monitor the patient's vital signs, administer
anesthesia if required, and assist with positioning the patient for
optimal access to the surgical site.
14. 3. Post-operative care:
i. After the procedure, nurses should closely monitor the patient for
any signs of complications such as bleeding, infection, or pain.
ii. They should assess the surgical site for any signs of redness,
swelling, or drainage.
iii. Nurses should provide appropriate wound care and administer
medications as prescribed, such as antibiotics or pain relievers.
iv. They should also educate the patient on post-operative care
instructions, including wound care, activity restrictions, and signs of
potential complications that require medical attention
