Practice Guideline of Zero Tolerance to Incivility at the Workplace: Fostering Workplace
Psychological Safety and Improving Patient SafetyThe problem of incivility at the healthcare
workplace has been there for a long time. It stems from many factors that include strained
interpersonal relationships among healthcare employees such as nurses and technicians,
verbal abuse from physicians directed at nurses, and rudeness of healthcare workers
towards patients and their relatives amongst others. When it is the healthcare workers such
as nurses that suffer this kind of incivility, they become affected emotionally and
psychologically. An Overview of a Healthcare System Practice.This makes them be prone to
making grave mistakes that endanger the lives of patients putting patient safety at risk. This
kind of situation becomes possible because the psychological safety of such a healthcare
facility as a workplace has become compromised with regard to the employees that work in
it. Nurses and other allied healthcare workers who are not treated with respect and dignity
but suffer verbal and physical abuse as a result of incivility at the workplace pose a danger
to patients. Eliminating workplace incivility will thus make them feel wanted and that they
are working in a conducive work environment. As a result, they will make fewer mistakes
and uphold patient safety and high quality nursing care in the process (Clark, 2018; Clark,
2015). This paper analyzes the practice guideline of zero tolerance to workplace incivility as
a policy in one of the healthcare facilities that the author has worked in previously.An
Overview of a Healthcare System Practice.The Practice GuidelineThis practice guideline had
as its aim to ensure that patient safety is maintained throughout the organization by
guaranteeing that nurses and all the other healthcare workers operated in an environment
of psychological safety. It expressly stated that the organization did not tolerate any kind of
incivility from one employee to another, from the management to the subordinates, from
those in higher professional positions (like physicians) to those lower than them (like
nurses), from the healthcare employees to patents and their relatives/ friends, and from
patients/ their relatives/ friends to the healthcare workers. This practice guideline was
displayed in large print at strategic locations all over the facility. Furthermore, it was
highlighted prominently in the code of conduct that each and every employee (from the top
management to the cleaner at the bottom) signed before taking up a job at the healthcare
organization.How Different Healthcare Professionals in the Organization are Held to this
GuidelineAs stated above, this practice guideline applied equally to all the employees of the
healthcare organization. This is because the formulators of the policy realized that patient
safety can be compromised by an act of incivility from any employee within the healthcare
organization. It does not necessarily have to come from employees that deal directly with
the patients such as physicians and nurses. A nurse who has been rudely addressed by a
pharmacist will be emotionally and psychologically hurt and will be a danger to the
patients. Likewise, a nurse aid who has been insulted by a registered nurse may in turn
show rudeness to a patient and hence spoil the quality of care that the patient is receiving at
the healthcare facility. As such, management in this organization are expected to address
their subordinates in a civil manner, even when the matter involves disciplinary action.
Physicians are expected to consider nurses and other members of the interprofessional
team as colleagues who are important in contributing to the overall care of the patient. They
should appreciate that everyone has an important role to play in making the patient
comfortable and treating them. In this same way, pharmacists, technicians, nurse aids,
nurse educators, nurse managers, nurse informaticists, and all other healthcare workers
within the organization are expected by this practice policy guideline to realize that any
incivility on their part will impact patient safety and will therefore not be tolerated.The
Reference/ Research Used by the Organization to Adopt the GuidelineThe main research’
reference used by this healthcare organization to adopt this practice policy guideline is the
work by Clark et al. (2011), published in the Journal of Nursing Administration (JONA). This
was a groundbreaking piece of work that was followed by several other scholarly works by
Griffin and Clark (2014) and Clark (2015; 2018); all focussing on workplace civility and its
importance in ensuring that patient safety is maintained and upheld. Clark et al. (2011) was
a cross-sectional survey of 174 nurse leaders, 68 nurse executives, and 106 nurse managers
whose views were surveyed concerning the matter of incivility at the healthcare workplace.
The findings included strategies that healthcare organizations can use to foster workplace
civility in the healthcare practice setting. Some of these were (Clark et al., 2011):An

policies with expected behaviors well and clearly definedMaintaining a healthy practice
environment for all that puts emphasis on workplace civilityProviding education to staff on
conflict resolution, respectful communication, and problem solving amongst other related
issues, andReinforcement of positive behaviorsThe Evidence Used to Define the Guideline
and its LevelThe evidence in this case showed amongst other things that about 27% of all
workers in the United States experience some form of workplace incivility that affects their
productivity. This includes verbal abuse and intimidation (Griffin & Clark, 2014). The same
evidence also points out that nursing as a profession is four times more dangerous than any
other occupation, and that nurses are twice as likely to experience violence and incivility at
their workplace compared to other occupations and professions (Clark et al., 2011). There
was therefore enough evidence to convince the policy makers within this particular
healthcare organization that incivility should be forbidden at the healthcare workplace. This
is because the evidence showed that incivility at the healthcare workplace adversely
affected the quality of patient care delivered as well as the level of patient safety. The level
of evidence (LOE) of the evidence in this case is level VI (Winoma State University, 2020).
An Overview of a Healthcare System Practice.Opinion on How Well this Guideline is
Followed by Professionals in the OrganizationBecause this policy guideline has been given
such great prominence by the top management of the healthcare organization, every
employee who comes to work there is inducted into an organization culture of civility. Also,
there are specific sanctions for incivility and rewards that act as positive reinforcement for
civil behavior. Being uncivil in the organization has therefore come to be frowned upon such
that no one wants to be seen as breaking this organizational taboo. The straightforward
opinion is therefore that all the professionals in this organization are strictly following this
practice guideline on a day-to-day basis.An Overview of a Healthcare System
Practice.ConclusionWorkplace incivility is a problem in many organizations. It is also
rampant within healthcare organizations. In the latter, however, its impact on patient safety
is something that has to be avoided by preventing it. Part of the strategy of preventing it is
coming up with policy guidelines that encourage civility and therefore foster patient safety.
There is enough evidence in support of this approach to reducing incivility at the healthcare

It stems from many factors that include strained interpersonal relationships among healthcare employees such as nurses and technicians, verbal abuse from physicians directed at nurses, and rudeness of healthcare workers towards patients and their relatives amongst others. When it is the healthcare workers such as nurses that suffer this kind of incivility, they become affected emotionally and psychologically. An Overview of a Healthcare System Practice.This makes them be prone to making grave mistakes that endanger the lives of patients putting patient safety at risk. This kind of situation becomes possible because the psychological safety of such a healthcare facility as a workplace has become compromised with regard to the employees that work in it. Nurses and other allied healthcare workers who are not treated with respect and dignity but suffer verbal and physical abuse as a result of incivility at the workplace pose a danger to patients. Eliminating workplace incivility will thus make them feel wanted and that they are working in a conducive work environment. As a result, they will make fewer mistakes and uphold patient safety and high quality nursing care in the process (Clark, 2018; Clark, 2015). This paper analyzes the practice guideline of zero tolerance to workplace incivility as a policy in one of the healthcare facilities that the author has worked in previously.An Overview of a Healthcare System Practice.The Practice GuidelineThis practice guideline had as its aim to ensure that patient safety is maintained throughout the organization by
  • 2. guaranteeing that nurses and all the other healthcare workers operated in an environment of psychological safety. It expressly stated that the organization did not tolerate any kind of incivility from one employee to another, from the management to the subordinates, from those in higher professional positions (like physicians) to those lower than them (like nurses), from the healthcare employees to patents and their relatives/ friends, and from patients/ their relatives/ friends to the healthcare workers. This practice guideline was displayed in large print at strategic locations all over the facility. Furthermore, it was highlighted prominently in the code of conduct that each and every employee (from the top management to the cleaner at the bottom) signed before taking up a job at the healthcare organization.How Different Healthcare Professionals in the Organization are Held to this GuidelineAs stated above, this practice guideline applied equally to all the employees of the healthcare organization. This is because the formulators of the policy realized that patient safety can be compromised by an act of incivility from any employee within the healthcare organization. It does not necessarily have to come from employees that deal directly with the patients such as physicians and nurses. A nurse who has been rudely addressed by a pharmacist will be emotionally and psychologically hurt and will be a danger to the patients. Likewise, a nurse aid who has been insulted by a registered nurse may in turn show rudeness to a patient and hence spoil the quality of care that the patient is receiving at the healthcare facility. As such, management in this organization are expected to address their subordinates in a civil manner, even when the matter involves disciplinary action. Physicians are expected to consider nurses and other members of the interprofessional team as colleagues who are important in contributing to the overall care of the patient. They should appreciate that everyone has an important role to play in making the patient comfortable and treating them. In this same way, pharmacists, technicians, nurse aids, nurse educators, nurse managers, nurse informaticists, and all other healthcare workers within the organization are expected by this practice policy guideline to realize that any incivility on their part will impact patient safety and will therefore not be tolerated.The Reference/ Research Used by the Organization to Adopt the GuidelineThe main research’ reference used by this healthcare organization to adopt this practice policy guideline is the work by Clark et al. (2011), published in the Journal of Nursing Administration (JONA). This was a groundbreaking piece of work that was followed by several other scholarly works by Griffin and Clark (2014) and Clark (2015; 2018); all focussing on workplace civility and its importance in ensuring that patient safety is maintained and upheld. Clark et al. (2011) was a cross-sectional survey of 174 nurse leaders, 68 nurse executives, and 106 nurse managers whose views were surveyed concerning the matter of incivility at the healthcare workplace. The findings included strategies that healthcare organizations can use to foster workplace civility in the healthcare practice setting. Some of these were (Clark et al., 2011):An Overview of a Healthcare System Practice.The creation of clear codes of conduct and policies with expected behaviors well and clearly definedMaintaining a healthy practice environment for all that puts emphasis on workplace civilityProviding education to staff on conflict resolution, respectful communication, and problem solving amongst other related issues, andReinforcement of positive behaviorsThe Evidence Used to Define the Guideline and its LevelThe evidence in this case showed amongst other things that about 27% of all
  • 3. workers in the United States experience some form of workplace incivility that affects their productivity. This includes verbal abuse and intimidation (Griffin & Clark, 2014). The same evidence also points out that nursing as a profession is four times more dangerous than any other occupation, and that nurses are twice as likely to experience violence and incivility at their workplace compared to other occupations and professions (Clark et al., 2011). There was therefore enough evidence to convince the policy makers within this particular healthcare organization that incivility should be forbidden at the healthcare workplace. This is because the evidence showed that incivility at the healthcare workplace adversely affected the quality of patient care delivered as well as the level of patient safety. The level of evidence (LOE) of the evidence in this case is level VI (Winoma State University, 2020). An Overview of a Healthcare System Practice.Opinion on How Well this Guideline is Followed by Professionals in the OrganizationBecause this policy guideline has been given such great prominence by the top management of the healthcare organization, every employee who comes to work there is inducted into an organization culture of civility. Also, there are specific sanctions for incivility and rewards that act as positive reinforcement for civil behavior. Being uncivil in the organization has therefore come to be frowned upon such that no one wants to be seen as breaking this organizational taboo. The straightforward opinion is therefore that all the professionals in this organization are strictly following this practice guideline on a day-to-day basis.An Overview of a Healthcare System Practice.ConclusionWorkplace incivility is a problem in many organizations. It is also rampant within healthcare organizations. In the latter, however, its impact on patient safety is something that has to be avoided by preventing it. Part of the strategy of preventing it is coming up with policy guidelines that encourage civility and therefore foster patient safety.
