New models of care organisation demonstrate that advanced ICT systems
and services may have the potential to respond to the increasing burden of
chronic disease and the complexity of co-morbidities and in doing so
contribute to the sustainability of health and care systems.
One challenge is to develop integrated care models that are more closely
oriented to the needs of patients and older persons: multidisciplinary, well-
coordinated, anchored in community and home care settings, and shifting
from a reactive approach to proactive and patient-centred care.

1. The future health of millions of children, the sustainability of the NHS, and the economic
prosperity of Britain all now depend on a radical upgrade in prevention and public health.
1. The NHS will now back hard-hitting national action on obesity, smoking, alcohol and
other major health risks. We will help develop and support new workplace incentives to
promote employee health and cut sickness-related unemployment.
3. The NHS will take decisive steps to break down the barriers in how care is provided
between family doctors and hospitals, between physical and mental health, between
health and social care.
4. Different local health communities will instead be supported by the NHS’ national
leadership to choose from amongst a small number of radical new care delivery options

Tunstall Healthcare New models of care organisation demonstrate that advanced ICT systems and services may have the potential to respond to the increasing burden of chronic disease and the complexity of co-morbidities and in doing so contribute to the sustainability of health and care systems. One challenge is to develop integrated care models that are more closely oriented to the needs of patients and older persons: multidisciplinary, well- coordinated, anchored in community and home care settings, and shifting from a reactive approach to proactive and patient-centred care. The future health of millions of children, the sustainability of the NHS, and the economic prosperity of Britain all now depend on a radical upgrade in prevention and public health. 1. The NHS will now back hard-hitting national action on obesity, smoking, alcohol and other major health risks. We will help develop and support new workplace incentives to promote employee health and cut sickness-related unemployment. 3. The NHS will take decisive steps to break down the barriers in how care is provided between family doctors and hospitals, between physical and mental health, between health and social care. 4. Different local health communities will instead be supported by the NHS’ national leadership to choose from amongst a small number of radical new care delivery options NHS UK 5 YEAR VIEW – KEY POINTS
