  • 3. We support people through individual packages of care and support. For example: • Expert and accredited services for older people with a dementia; as well as high quality nursing care in communities designed to promote independence, dignity and choice • Services for people with an acquired brain injury; people with a physical disability and for people with a neurological disability across a care pathway • Registered independent hospitals which support people with long-term mental health needs and early onset /organic mental health needs in people under 65 • Care villages – purpose-built retirement villages for independent/assisted living • Services for people with a learning disability (and accompanying complex needs) and services for people with autism through accommodation and support services and supported living pathways.

  • 8. Different Regional and Local Approaches  Scotland, Wales, Ireland and England all have different approaches linked to choice if people need public funding  Across England , different pace of change in each local authority/primary care trust
  • 9. Reform Agenda 3 key drivers  rising public expectation  public service reform (drive innovation and reduce poor performance)  intractable inequality : service response does not work Plus the economic reality of next 30 years
  • 10. Personal Budgets and Personal Health Budgets  Personal budgets : LA linked to the ‘universal offer’ everyone has access to lots of information provided by the LA including self funders  Personal Health budgets : piloting for people with long term conditions
