  1. Key message: To deliver on our commitment to Smarter Care, we have a set of focused solutions that begin to target these three layers: foundation, analytics and cognitive, and coordination. Analytics and Cognitive computing – that enable population analytics, diagnostic support, data driven care pathways, operational reporting – and Cognitive computing ushered in by Watson that provide confidence weighted outcomes with data transparency, systematic learning, natural language processing Coordination -- technologies that help determine program eligibility, and enable care planning, collaboration, and outcome evaluation; business solutions that help find programs and services from the community, private sector and government. -- These solutions help determine eligibility and entitlement -- and the constructs and individual care plan -- based on the knowledge of what works and what is available. By enabling the delivery of care services in a coordinated way, this supports the achievement of outcomes for the individual at the most effective cost point for the providers.
