 The first stage of the Model MHA program is to complete
the Application. MHAs completed the 40 question
Application and submitted the following supporting
documents:
 Most recently updated MHA Constitution / Bylaws
 Board of Directors and committee member listings
 Minutes of last Annual General Meeting
 Latest Financial Statement

5. Self-Assessment
 Following the submission of the Application and supporting
documents, MHAs completed the 150 question Self-
Assessment focusing on:
 Hockey Governance
 Hockey Respect and Player Safety
 Hockey Operations
 Program Standards

6. QualityAssessment
 The Quality Assessment combines 30 of the most
important questions from the Application and Self-
Assessment, including:
 Hockey Governance
 Hockey Operations
 Respect and Safety
 MHAs who receive a score of at least 25/30
are designated Model MHAs
