ELECTRONIC IDENTITY PROCESS FLOW Online Registration •Citizen fills out online forms for registration •May be asked to upload proof of identity Verification/Proofing •Officer evaluates online application •Conduct verification process – face to face or online Approval •Citizen receives email for approval of ID •ID can be sent over email (digital) or they can claim a physical card in the LGU Data Uploading •LGU upload existing citizen list and information Generation/ID-Proofing •System generates eID based on inputted information •Officer-in-charge validates and approves generated ID Notification/Releasing •Citizen receives email/text that ID is ready •Citizen can receive the ID via email (for digital copy) LGU Initiated Registration Citizen Initiated Registration

COVID-19 VACCINE PROCESS FLOW Online Registration/Scheduling • Citizen registers for vaccination using eID provided by LGU • Citizen also chooses the available schedule of vaccination Evaluation and Approval • Officer evaluates citizen registration • May require additional verification that can be done online • Officer approves or disapproves vaccine registration Notification and Vaccination Proper (1st dose) • Citizen receives a notification if registration for vaccine is approved • If approved, citizen will receive the schedule details • Citizen is instructed to bring ID and other necessary documents Notification and Vaccination Proper (2nd dose) • Citizen who were previously vaccinated (1st dose) will receive notification for 2nd dose • Citizen should bring their electronic ID for verification Online Registration/Scheduling • LGU uploads existing list of people to be vaccinated • LGU identifies capacity per day and date range Evaluation and Approval • System generates schedule based on list uploaded • Officer-in-charge evaluates and approves the schedule • Officer-in-charge may manually add schedule if needed Notification and Vaccination Proper (1st dose) • Citizen receives a notification for vaccination schedule details • Citizen is instructed to bring ID and other necessary documents Notification and Vaccination Proper (2nd dose) • Citizen who were previously vaccinated (1st dose) will receive notification for 2nd dose • Citizen should bring their electronic ID for verification LGU Initiated Scheduling Citizen Initiated Scheduling

COVID-19 TESTING PROCESS FLOW Online Scheduling • Citizen fills out online forms for testing requirements • Citizen will be asked to input electronic ID number • Citizen will be given a reference number Testing Proper • Officer asks for reference number • Conduct Results send out • Citizen receives results via email • Results are also verifiable LGU Initiated Testing
