• How quickly did it happen?

Instead of focusing on the
person, ask yourself:
• What was the environment like when the
accident took place?
• What time of day did the accident occur?
• Were all safety procedures being followed
prior to the accident?
• Was the person engaged in a behavior or
action that they do everyday or is common
place to them?

of the complexity of the hand.

function and a worker could suffer loss of:

• Grip
• Ability to perform everyday tasks

consider a ‘risk?
Q. If we had a blind safety audit,
would they be able to identify
our safety culture?
