TRANSCRIPT EXPLANATION
ACCREDITATION AND ACADEMIC CALENDARS
Florida State University is a fully accredited member of the Southern
Association of Colleges and Schools, and thereby is accredited for the
Southern region, and reciprocally, for the country.
Florida State University College of Medicine was accredited by the Liaison
Committee on Medical Education of the Association of American Medical
Colleges and the American Medical Association, February 4, 2005.
Florida State College for Women (1909-1947) by act of the 1947 Florida
State Legislature was made a co-educational institution and designated
“Florida State University.” The unit of credit at that time was the quarter hour
and term designation was Quarter.
Effective September, 1951 – The academic calendar for Florida State
University was divided into two equal terms and a summer session, thereby
having semester hours as the unit of credit.
Effective September, 1962 – The academic calendar for Florida State
University was divided into three equal terms, the unit of credit was semester;
and the term designation was Trimester.
Effective September, 1967 – The academic calendar for Florida State
University reverted to four terms and the unit of credit was designated the
quarter hour.
Effective August, 1981 – The academic calendar for Florida State
University reverted to two equal terms and a summer session and the unit of
credit was semester hours.
Effective August, 1984 – Florida State University implemented a grading
system in which pluses and minuses may be added to the traditional letter
grades.
GRADING SYSTEM
Letter grades are assigned for all courses except for those specifically
designated in the Bulletin as S/U graded only, or those for which the student
has utilized the S/U option. The scale of letter grade is as follows:

B - Good S - Satisfactory
C - Average U - Unsatisfactory
D - Passing EC - CLEP Exam Credit/CEEB
F - Failed P - Passed (Graduate Tests)

GE - No Grade Expired WD - Withdrawn with permission of
dean
HM - Honors Medicine *W - Withdrew/Late drop
PM - Pass Medicine NG - No Grade Reported
CR - Credit Received
AE - A.I.C.E. Credit
* A grade of “W” is used only to denote that a student was passing a course
at the time he withdrew from the University.
QUALITY POINTS
Prior to the Fall Semester, 1955, the University operated on a three point
grade system. However, from Fall Semester, 1955 to Fall 1984 quality points
were assigned to passing grades as follows: A: 4 q.p.; B: 3 q.p.; C: 2 q.p.;
D: 1 q.p.; F: 0 q.p.; IE: 0 q.p.; GE: 0 q.p.
Effective Fall Semester, 1984, quality points are assigned to grades
earned as follows: A: 4.00 q.p.; A-: 3.75 q.p.; B+: 3.25 q.p.; B: 3.00 q.p.; B-:
2.75 q.p.; C+: 2.25 q.p.; C: 2.00 q.p.; C-: 1.75 q.p.; D+: 1.25 q.p.; D: 1.00 q.p.;
D-: 0.75 q.p.; F: 0.00 q.p.; IE: 0.00 q.p.; GE: 0.00 q.p.
The grade point average is computed by dividing the number of hours
attempted with letter grades (A, B, C, D, F, IE, and GE) at Florida State
University into the total number of quality points assigned.
Effective Spring Quarter, 1971, the University instituted a “forgiveness”
policy for undergraduate students which permitted said students to repeat
grades of “D” and “F” in order to improve their level of performance (g.p.a.);
the last attempt only is computed in the average.
Effective Spring Quarter, 1974, the University instituted a “forgiveness”
policy for graduate students which permitted said students to repeat grades
of “C” or below in order to improve their level of performance (g.p.a.). This
option could be exercised for no more than two courses and only once for any
particular course.
Effective Fall Quarter, 1980, both undergraduate and graduate students
were also limited to two repeats under the forgiveness policy for the purpose
of improving the g.p.a.
Effective Summer 2003, the College of Medicine instituted grades of
Honors Medicine (HM), 4.00 q.p. and Pass Medicine (PM), 3.00 q.p.
Effective Fall 2004, the forgiveness policy was discontinued.

GRADUATION REQUIREMENTS
Undergraduate: 120 semester hours minimum for all degrees and a minimum
cumulative 2.00 g.p.a. on all college hours attempted.
Graduate: For master’s degree, 30 semester hours, minimum, with additional credit,
thesis, etc. subject to specific departmental requirements and a minimum cumulative 3.00
g.p.a. on all college hours attempted. For doctoral degrees, completion of 30 semester
hours of graduate work or award of the master’s degree, after which the student must be
continuously enrolled on Florida State University campus or in one of its teaching centers,
for a minimum of 24 semester credit hours during one academic year; and a minimum
cumulative 3.00 g.p.a. on all college hours attempted.
COURSE NUMBERING SYSTEM
Effective September, 1978, prefixes and numbers appear according to the Common
Course Number designations implemented by the State Department of Education. The first
digit of the 4-digit number represents the level of the course (0, introductory, college prep:
1, 2 lower level; 3, 4 upper level; 5, 6 graduate level: 7, 8, 9 graduate and professional)
and is assigned by Florida State University. The second digit represents the major
subclassification of the discipline, i.e., 0, introductory, entry level course; 9, miscellaneous
general categories such as thesis and seminars. The third and fourth digits are used for
indicating level of complexity and for sequencing. The outside indicator “L” means that the
course is a laboratory course, the “C” means that the course is a combination of lecture
and laboratory.

TRANSCRIPT EXPLANATION ACCREDITATION AND ACADEMIC CALENDARS Florida State University is a fully accredited member of the Southern Association of Colleges and Schools, and thereby is accredited for the Southern region, and reciprocally, for the country. Florida State University College of Medicine was accredited by the Liaison Committee on Medical Education of the Association of American Medical Colleges and the American Medical Association, February 4, 2005. Florida State College for Women (1909-1947) by act of the 1947 Florida State Legislature was made a co-educational institution and designated “Florida State University.” The unit of credit at that time was the quarter hour and term designation was Quarter. Effective September, 1951 – The academic calendar for Florida State University was divided into two equal terms and a summer session, thereby having semester hours as the unit of credit. Effective September, 1962 – The academic calendar for Florida State University was divided into three equal terms, the unit of credit was semester; and the term designation was Trimester. Effective September, 1967 – The academic calendar for Florida State University reverted to four terms and the unit of credit was designated the quarter hour. Effective August, 1981 – The academic calendar for Florida State University reverted to two equal terms and a summer session and the unit of credit was semester hours. Effective August, 1984 – Florida State University implemented a grading system in which pluses and minuses may be added to the traditional letter grades. GRADING SYSTEM Letter grades are assigned for all courses except for those specifically designated in the Bulletin as S/U graded only, or those for which the student has utilized the S/U option. The scale of letter grade is as follows: Used to Compute G.P.A. Not Used to Compute G.P.A. A - Excellent I - Incomplete B - Good S - Satisfactory C - Average U - Unsatisfactory D - Passing EC - CLEP Exam Credit/CEEB F - Failed P - Passed (Graduate Tests) IE - Incomplete Expired ED - Departmental Exam Credit GE - No Grade Expired WD - Withdrawn with permission of dean HM - Honors Medicine *W - Withdrew/Late drop PM - Pass Medicine NG - No Grade Reported CR - Credit Received AE - A.I.C.E. Credit * A grade of “W” is used only to denote that a student was passing a course at the time he withdrew from the University. QUALITY POINTS Prior to the Fall Semester, 1955, the University operated on a three point grade system. However, from Fall Semester, 1955 to Fall 1984 quality points were assigned to passing grades as follows: A: 4 q.p.; B: 3 q.p.; C: 2 q.p.; D: 1 q.p.; F: 0 q.p.; IE: 0 q.p.; GE: 0 q.p. Effective Fall Semester, 1984, quality points are assigned to grades earned as follows: A: 4.00 q.p.; A-: 3.75 q.p.; B+: 3.25 q.p.; B: 3.00 q.p.; B-: 2.75 q.p.; C+: 2.25 q.p.; C: 2.00 q.p.; C-: 1.75 q.p.; D+: 1.25 q.p.; D: 1.00 q.p.; D-: 0.75 q.p.; F: 0.00 q.p.; IE: 0.00 q.p.; GE: 0.00 q.p. The grade point average is computed by dividing the number of hours attempted with letter grades (A, B, C, D, F, IE, and GE) at Florida State University into the total number of quality points assigned. Effective Spring Quarter, 1971, the University instituted a “forgiveness” policy for undergraduate students which permitted said students to repeat grades of “D” and “F” in order to improve their level of performance (g.p.a.); the last attempt only is computed in the average. Effective Spring Quarter, 1974, the University instituted a “forgiveness” policy for graduate students which permitted said students to repeat grades of “C” or below in order to improve their level of performance (g.p.a.). This option could be exercised for no more than two courses and only once for any particular course. Effective Fall Quarter, 1980, both undergraduate and graduate students were also limited to two repeats under the forgiveness policy for the purpose of improving the g.p.a. Effective Summer 2003, the College of Medicine instituted grades of Honors Medicine (HM), 4.00 q.p. and Pass Medicine (PM), 3.00 q.p. Effective Fall 2004, the forgiveness policy was discontinued. Graduate: For master’s degree, 30 semester hours, minimum, with additional credit, thesis, etc. subject to specific departmental requirements and a minimum cumulative 3.00 g.p.a. on all college hours attempted. For doctoral degrees, completion of 30 semester hours of graduate work or award of the master’s degree, after which the student must be continuously enrolled on Florida State University campus or in one of its teaching centers, for a minimum of 24 semester credit hours during one academic year; and a minimum cumulative 3.00 g.p.a. on all college hours attempted. COURSE NUMBERING SYSTEM Effective September, 1978, prefixes and numbers appear according to the Common Course Number designations implemented by the State Department of Education. The first digit of the 4-digit number represents the level of the course (0, introductory, college prep: 1, 2 lower level; 3, 4 upper level; 5, 6 graduate level: 7, 8, 9 graduate and professional) and is assigned by Florida State University. The second digit represents the major subclassification of the discipline, i.e., 0, introductory, entry level course; 9, miscellaneous general categories such as thesis and seminars. The third and fourth digits are used for indicating level of complexity and for sequencing. The outside indicator “L” means that the course is a laboratory course, the “C” means that the course is a combination of lecture and laboratory.
