After-School Free Meal Program
Guardians for ages 12 and under have to sign in on the sheet, which has a section for
allergies and restrictions. Record those from sign-in sheet here.
For ages 13+, before handing them their meal, ask if they have any allergies and record
here. Please record the first time they disclose their allergy/restriction and put ‘N’ for
‘Provided?’ For all times after that, please record and put ‘Y’ under ‘Provided?’
