Albert is a single African American male. He is 5’9 and weighs 110bs and
occasionally relies on oxygen to breath. He was coherent at the time of the interview.

Family Characteristics
Albert and his brothers were born to Albert and Lossie Lee Lumford in the southern
state of Durham, North Carolina. His father Albert Lumford Sr was a sharecropper and
his mother Lossie Lee Lumford was a homemaker Albert is the eldest of the four
children, his brothers are Charles, Douglas and Pete. . Charles and his wife Doris have 5
children 9 grandchildren and 7 great-grandchildren. Albert has 4 children, 8
grandchildren, and 5 great grandchildren and was married to Gloria M Taliaferro. His
oldest daughter Marie seemed to struggle during her adolescent years into adulthood. In
1975 at age of 17 years old Marie met a man 10 years her senior and ran away from home
to be with him. When she returned she told her parents that she wanted to get married,
but would need their consent. Reluctantly Albert and Gloria signed the consent papers,
and the couple got married at Newark, NJ City Hall. She moved in with her husband and
shortly thereafter became pregnant. Marie stayed in high school and graduated in June of
1975. Four months later she gave birth to a baby girl named Ashiah, and within the next
two years she gave birth to a baby boy named Rashad. The remained together for 8 years
before the couple divorced in 1983.
Sabrina was an over achiever and always did well in school. She graduated with a
Bachelor’s degree in Political Science from Shaw University in Raleigh North Carolina
and landed a job with the US Attorney’s office in Newark, New Jersey. Sabrina met and
married her husband in 1982. There were no children from this union but the marriage
began to deteriorate when Sabrina had a nervous breakdown and was diagnosed with
Paranoid Schizophrenia. She had been placed in several institutions and hospitals in New
Jersey. Today Sabrina is living independently in a senior housing complex.
Denise was affected by the physical and mental abuse towards her mother at the hands of
her father. She began smoking marijuana and drinking wine at 13 years old. By the time
she was 17 her alcohol use increase to hard liquor and sniffing cocaine. By age 30 her
substance uses escalated to smoking cocaine, speedballs and sniffing heroin. She entered
treatment in September 21, 2000 and has been clean ever since. She got married in 2004,
and went back to school to earn an A.S and Bachelors in Human and Social Services and
adopted a sibling group a boy and a girl in 2010. She is currently working towards her
MSW.

Client presented in the emergency room (ER) having been brought in the previous night by her parents. Following an argument with her parents, L cut her right wrist. L's mother reported that L started screaming rapidly and became physically violent toward her prior to cutting her own wrist.
Psychological Data
L is a 17-year-old Hispanic female who resides in Pennsylvania with her mother, father, and older sister. She is in 11th grade at the local public school. L appeared to be of average to above-average intelligence, as she was able to respond to numerous questions in an articulate and intelligent manner. She was well versed about world history and current affairs. Her mother confirmed that she has done well in school, maintaining a B+ average and participating in various school activities (e.g., chorus, school paper) until last year. L slowly dropped out of many activities she liked in the past. Her mother noticed about 8 months ago that L had also begun having difficulty doing schoolwork. Erratic behavior arose during episodes when L also became irritable and explosive. During these repeated episodes, she became quite defiant, cut classes, had to be placed in school detention, and had even assaulted the principal. L has numerous friends and believed she can relate to all types of people. She has a boyfriend who adores her, but she said she doesn't feel the same about him. The school counselor confirmed that L is outgoing, popular, and smart; but during these episodes she became another person, one who is very violent and difficult.
Medical History
A physical examination by a staff doctor revealed superficial cuts on L’s left and right wrist. The cuts appeared to be a few weeks old. There were cigarette burns on her right wrist that looked to be approximately one week old. In questioning L about the cigarette burns, L responded, "I just wanted to see how it felt—now I know." When questioned about old cuts on her left wrist, she responded, "I don't want to talk about it." L weighs 103 pounds and is 5’ 6” tall. L denied any dieting or fasting, but her mother noticed over this past year that her weight has dropped. Substance Abuse History L denied any drug or alcohol use. When she was questioned regarding such, her response was "I could do drugs if I wanted to. I don't want to, because it’s dumb." Family History L’s mother is 42 years old and works as a secretary for a large telephone company. Her father is 49 years old and operates a small landscaping business. Both are U.S. citizens, with a cultural background from Guatemala of which they are proud. Both have 2 a high school education. L’s sister is considerably younger, aged 8.
Their relationship is described as unremarkable, although L’s mother noted that the younger sister stays away when L is upset. Marital circumstances are uncertain, although the parents admitted that they are trying to keep the family together for their children, and they are of .

His mother was single and only 19 years old. Karl had been exposed to violence and lack of
adequate care due to her alcoholism and drug abuse and ended up in hospital on two occasions.
Karl has displayed anti-social and violent behaviour since he was 5. He frequently raided his
foster families’ stores of alcohol and money and fought with neighbours and foster families. Karl
often feels angry doesn’t really trust anyone. His behaviour made it difficult to find a permanent
home, and he has been placed in 10 different homes during his 15 years in care. Karl never
completed Year 10.
Karl is so relieved he has finally turned 18 and can finally refuse to accept what his care worker
tells him he should do and where he should live. He can make his own choices and is choosing to
be angry at the world. Karl has been told about some of his options, like one-off grants but has
found it too hard to figure out what to do and assumes he won’t get the grants anyway.
Karl’s care workers sat down with him to make a “leaving care plan” a month ago which felt like
“planning to nowhere”. Karl found talking about bank accounts, finances and paying rent and bills
boring. He does remember they mentioned something about a game that he could play, but at
that point he was more interested in just saying no to everything.
Karl become homeless after leaving state care. He spent a few ok weeks living with his mum but
was kicked out after an argument with her boyfriend.
Karl is really reluctant to listen to ‘authority’ and is doesn’t want to take the advice of his care
workers about planning his future. He struggles to really trust anyone to help him and doesn’t
think he’ll ever be able to get a job. He has a couple of friends from previous care homes he
stayed in and he tries to keep in touch with them using his phone. He doesn’t like being on the

Karl’s Story “I want my life to be better, but don’t tell me what to do”
Karl’s story
When Karl was 3 years old, he was removed from his home for his own protection.
His mother was single and only 19 years old. Karl had been exposed to violence and lack of
adequate care due to her alcoholism and drug abuse and ended up in hospital on two occasions.
Karl has displayed anti-social and violent behaviour since he was 5. He frequently raided his
foster families’ stores of alcohol and money and fought with neighbours and foster families. Karl
often feels angry doesn’t really trust anyone. His behaviour made it difficult to find a permanent
home, and he has been placed in 10 different homes during his 15 years in care. Karl never
completed Year 10.
Karl is so relieved he has finally turned 18 and can finally refuse to accept what his care worker
tells him he should do and where he should live. He can make his own choices and is choosing to
be angry at the world. Karl has been told about some of his options, like one-off grants but has
found it too hard to figure out what to do and assumes he won’t get the grants anyway.
Karl’s care workers sat down with him to make a “leaving care plan” a month ago which felt like
“planning to nowhere”. Karl found talking about bank accounts, finances and paying rent and bills
boring. He does remember they mentioned something about a game that he could play, but at
