ABOUT OUR WELLNESS CARE CLINICS
Spay and Neuter Kansas City holds walk-in Wellness Care Clinics offering low-cost vaccinations, heartworm
tests, FIV tests, feline leukemia tests, heartworm and flea prevention, treatment for worms, microchips, and
other preventative health services for pets. We also issue pet licenses for the city of Kansas City, Missouri. The
services offered during Wellness Clinic promote responsible pet ownership and animal welfare.
Spay and Neuter Kansas City is not a full-service veterinary clinic. In order to keep our costs affordable, our
organization does not provide client/veterinarian consultations, but the pet does receive a brief visual exam.
If a client has concerns about their pet’s health or can afford regular full-service care, Spay and Neuter
Kansas City recommends our clients visit a full-service clinic.
DISCOUNTS & COUPONS
DISCOUNTS FOR ALTERED ANIMALS
As our agency is a Spay and Neuter Clinic, we offer discounted services for our clients with altered (spayed
or neutered) pets. This acts as an added incentive to spay or neuter, along with all of the health benefits to
doing so.
VACCINATION SERVICE COUPONS
Spay and Neuter Kansas City will occasionally send out coupons to our clients for discounted vaccination
services. Restrictions differ for each coupon: some coupons are only applicable to altered pets, some
coupons are applicable to all pets in a single household, etc.
Examples of some coupons include:
FULL-SET COUPON: $5 off vaccination services
PUPPY/KITTEN COUPONS
Another incentive offered by SNKC is our Puppy/Kitten Coupon. This coupon offers spay or neuter services
for puppies or kittens under 6 months of age and under 40 pounds for $25.
RESCUE GROUP DISCOUNTS
We appreciate all of the hard-work that other organizations put forth in the efforts of animal welfare and pet
overpopulation. We offer discounted rates of spay and neuter and vaccination services for rescue
organizations.

PUPPY/KITTEN COUPON POLICIES AND PROCEDURES
Puppies and kittens will be issued a coupon allowing them to receive a $25 spay/neuter surgery if the
following occur:
Puppy or kitten must be under 6 months of age and under 25 pounds, whichever comes
first. (Must weigh at least 3 pounds for surgery.)
Clients must come into one of our wellness clinics to receive a $20 or $30 puppy/kitten
booster vaccination package. (This allows the veterinarian to determine age and size, as
well as, if the pet qualifies. It also gets at least one set of vaccinations in the pet prior to
surgery.)
WHEN SHOULD THEY SET THEIR APPOINTMENT?
Clients with large breed pets should be encouraged to set a surgery appointment quickly
so they don’t reach 25 pounds prior to the date of the appointment. Indicate to client that if
pet is over 25 pounds at time of surgery, they will be paying full price.
If the pet is close to going over 6 months of age, also encourage the Client to set the
surgery appointment quickly, as they will be paying full price once the pet is over the age
of 6 months.
Same protocol and prices should be followed for pets with undescended testicles.
Additional charges apply if testicles are not descended at the time of surgery.
All other services at time of surgery are at the normal low-cost price.
THINGS TO KNOW:
Coupons are non-transferable to another pet, but can be transferred to a new owner of the
same pet the coupon was issued to.
Coupon must be issued by a vet. If no coupon is presented at time of surgery and/or is not
recorded within Avimark as being issued, client will need to come to clinic to get one or
they may pay full price.
If coupon is expired or lost and the pet is still under 6 months and/or 40 pounds, a new
coupon may be re-issued either at the time of another round of vaccines or at the time of
surgery.
Rescue groups are NOT eligible for this coupon. They are able to recoup their funds for
each pet through an adoption fee. This coupon is not intended for them to make a profit
from our services. It is intended to keep already owned pets in their homes and not
creating more litters in our community.

PROCESS ON HOW TO ISSUE COUPON DURING WELLNESS CARE CLINIC
If a puppy or kitten comes for vaccines during wellness care clinic and the pet appears to be under 40 lbs.
and/or 6 months of age, then a puppy coupon can be issued at data entry but will ultimately be determined
by the veterinarian if it is valid. (Should the vet determine that the pet is over 6 months of age or the weight
at clinic is at or over 25 lbs., the coupon will not be valid or given to the client. Therefore, if you are in doubt
that the pet is eligible, please do not discuss the coupon with the client but, obviously, discuss getting the
pet fixed at some point at the normal, low cost price.)
CUSTOMER SERVICE REPRESENTATIVES
Please write “Issue puppy or kitten coupon” on the paperwork to alert data entry. Please also indicate on
the paperwork if the pet is receiving the puppy/kitten booster round or the full booster round, and discuss
with the client all of the previously mentioned “musts” in order for them to receive this great surgery price.
IF CLIENT ASKS WHY WE ARE ABLE TO DO THE SURGERY SO CHEAP, YOU CAN EXPLAIN TO
THEM:
SNKC is targeting puppies and kittens because we are trying to eliminate any “oops” litters before
they are reproductively active.
Getting females spayed prior to their first heat cycle reduces their chances of mammary cancer by
over 98%. And, if a castration or an ovariohysterectomy is performed, the chance of them getting
testicular or ovarian/uterine cancer is reduced by 100% because those parts are completely
removed.
Getting pets fixed at a young age often times means there are less complications and a faster
healing period because the pet is already in a state of high metabolism and growth which leads to
a faster recovery.
We are not trying to make a profit from these surgeries; we are simply trying to fulfil our mission of
decreasing the pet overpopulation through spaying & neutering. There are fewer drugs, materials,
and labor hours utilized with younger, smaller pets; therefore making it cost effective.
DATA ENTRY
If you see that the Customer Service Representative has indicated that a coupon should be issued OR you
see within Avimark that the pet is at an age below 6 months and is below 25 lbs. and no coupon has been
issued yet, you may issue a coupon. (It could be the Customer Service Representative forgot to write it
down. Again, the vet will determine upon exam, if the coupon will be valid.)
In Avimark, in the Medical History area, right click, then find “Choose” and click on “Treatments”.
The “Doctor” should be entered as “SNV: VET Care Center”.
In Category, click on “SNKC - Spay & Neuters”, then click on “Puppy/Kitten Coupon Issued” and
click “Done”.
The treatment will appear in the Medical History.
On the coupon, please fill out the account number & the pet’s first and last name.
Attach the coupon to the Wellness Clinic form with a paper clip so it can easily be removed by the
vet staff and the front desk staff.
You may proceed as normal with your clinic paperwork.

HEALTH CERTIFICATE POLICIES AND PROCEDURES
Health Certificates are required for any pet traveling across states within the United States. Transportation
modes include: car, air, rail, and truck. Spay and Neuter Kansas City does not write Health Certificates for
international transport. If the pet is traveling by air, be sure that the client has contacted their airline for any
specific instructions as they may vary. Spay and Neuter Kansas City will not be responsible for obtaining
this information.
IN ORDER FOR A CLIENT TO RECEIVE A HEALTH CERTIFICATE AT SNKC:
The Client must be a Missouri resident. We cannot write them for residents outside of the state of
Missouri.
The Client must provide a starting or “consignor” and an ending or “consignee” address.
The Client must provide a copy of the pet’s valid Rabies Certificate or SNKC will need to do a
Rabies vaccination.
The Rabies Certificate must have the vaccine-type information, and the Rabies tag
number.
Rabies Certificates without a veterinarian’s signature (i.e. “Angel’s Express) will not be
accepted.
**Cost is non-negotiable.
PROCEDURE
1. Get a blank health certificate. It is a triplicate form – white/yellow/pink pages
2. Fill in the address information. Please do not let the Client do this so it is legible.
a. Address must be complete
b. Consignor: the person currently in possession of the pet
c. Consignee: the person receiving the pet or the pet’s destination
3. Fill in the pet information and Rabies vaccine information using the Rabies Certificate
4. Add Spay and Neuter Kansas City’s address in the box at the bottom
5. Bring the certificate and the SNKC form to the Doctor on Duty for completion
6. At Check Out, give the Client the top white copy. The yellow and pink copies go to the front desk
and they will mail them into the state.

ONE & THREE YEAR RABIES POLICIES AND PROCEDURES
HOW TO ENTER A 3-YEAR RABIES AND THE HEALTH CARE PACKAGE
Spay and Neuter Kansas City will honor a 3-year Rabies Certificate in lieu of the 1 year Rabies within our
dog or cat health care packages. We can omit the Rabies we would be giving in those packages only if a 3
year Rabies Certificate can be presented.
GUIDELINES FOR OMITTING THE RABIES
Client MUST present proof of 1 or 3 year Rabies Certificate.
Please copy and attach to wellness paperwork.
Please enter the 1 or 3 year Rabies into vaccination history (will show how to below)
There will be NO price adjustment. We are simply making it easier for the client to get a
package deal.
** This will not pertain to any other treatment within this package!! If the pet is current on their DA2PP and
Bordetella and want everything else, they will have to purchase individual services, or they can get
everything all over again. It will not hurt the pet and in fact, in the case of Bordetella, it's actually beneficial
to give a booster on every 6 months especially if they are groomed or boarded frequently.
HOW TO ENTER A 3 YR RABIES AND ADJUST A DOG OR CAT HEALTH CARE PACKAGE:
Enter the dog or cat health care package as normal for the pet.
When the green rabies vaccine box opens up, click cancel. Then click NO on the box questioning if
you want to save the vaccine information for this vaccine.
(This will make sure that you are not entering one of our rabies tag numbers or the lot number, etc
for our rabies vaccine.)
At this point, the health care package has populated in the medical history section.
Click to highlight the 1 year rabies vaccine in that package, then right click and choose remove.
Click yes to remove.
Now, click on the patient's name to highlight and then right click to choose Vacc History. Click YES
to proceed.
Find “3 yr Rabies ***Elsewhere" or “1 yr Rabies in the box and enter the date that the vaccine was
given elsewhere. Click done.
A green box will pop up with empty fields. Do not enter any information into the fields. Click done.
Your 3 yr rabies will be black in the medical history and it will appear in the reminders box to the
right with the correct date when the pet will be due in the future.
Highlight the black "3 yr Rabies ***Elsewhere" entry and then click on notes at the right.
In the notes box, please enter which vet clinic administered the rabies, their phone #, the rabies tag
# and your initials. You can now proceed as you normally do.
** ON THE WELLNESS FORM, please cross through the 1 year Rabies to be administered and make a
note that this pet is current on a 3 year Rabies and that we will be omitting the rabies in this package for
today.**

CHECKING IN CLIENTS
As clients arrive to our facilities for our Walk-In Wellness Clinics, they are directed to the first waiting room
to check-in at the podium for vaccination services. Checking in Clients is the very first step of their
vaccination services.
BEFORE CLINIC:
1. Set out the following: Clinic Check-In Sheet, a small stack of Wellness Care Clinic-Dog and
Wellness Care Clinic-Cat sheets, pens, clipboards, Check-In Number Cards, and hand sanitizer.
2. Prep Check-In Number Cards by attaching them to the Clipboards
AT CHECK-IN:
1. Write the Client/Patient information in numerical order of arrival on the Wellness Clinic Check-In
Sheet
Write the time of the Client’s arrival in the “Time-In” box.
Ask the Client for their first and last name, and proper spelling of both. Write this
information in the “Client’s Full Name” box.
Ask the Client for the name(s) of the patient(s) receiving vaccination services that day,
and ensure proper spelling. Write this information in the “Pet’s Name” box.
2. Assign the Client either a Wellness Clinic-Dog or Wellness Clinic-Cat sheet per each dog and/or
cat receiving services.
In the top right hand corner of each Wellness Clinic Sheet, write the Check-In Number
for the Client. If there are multiple pets, please write the Check-In Number and assign
alphabetical values for each pet.
Example: If a client is the 12th client to check-in and they have three pets, their Check-
In Numbers would be 12A, 12B, and 12C, etc.

3. Give the Client a clipboard with their Check-In Number Card and Wellness Clinic Sheets attached,
as well as a pen, and politely instruct them to have a seat in our waiting area, fill out the top
informational portion of each sheet, and that a Customer Service Representative will be with them
shortly to discuss their service options with them.
Clients are to fill out the information in the highlighted area.
4. Accommodate the needs of Clients and Patients.
If a Client has an animal in need of a quieter or more secluded space, direct them to
the second waiting area, the hallway, or the kitchen. Please choose a location based
on the volume of business and what best fits their needs. In the event that a Client is
placed in the hallway or kitchen, make a note on the Check-In Sheet with the location
of the Client in case a Customer Service Representative has a question in regards to
their whereabouts so that they may help the Client when it comes their turn.
DURING CLINIC:
If you have extra down-time, there are always other tasks that can be done while waiting on the arrival of
new Clients/Patients. These tasks include, but are not limited to:
Handing out dog and cat treats to Clients and Patients
Washing and folding laundry
Opening syringes
Assembling microchip packs
Monitoring animal behavior in waiting areas

CUSTOMER SERVICE
Customer Service is a very important part of our Walk-In Wellness Clinics. During Customer Service, our
Customer Service Representatives discuss different vaccination service options with our Clients and assist
our Clients with reaching the needs of their pets.
1. Assist Clients in numerical order.
It is extremely important that Clients are seen in order of arrival. As we offer our clinics
with no need for appointments, Clients who arrive early, deserve to be seen early.
2. Ensure that all personal information (except for the account number) is correctly filled out for the
Client and the Pet(s) being seen. Weight can be left blank if unknown. All patients are weighed
during their exam.

3. Discuss with Clients the vaccination needs for their pet(s) and help them decide what package best
fits their needs and their budget. Discuss any additional boosters that may be needed for young
animals, and check the teeth of puppies of kittens to see if they are eligible for a Rabies Round of
vaccinations.
4. Put an (X) next to each service to be rendered and circle the amount to be paid.
Mark an (X) next to the package selected, and next to each individual service included
in that package
Example: If a Client chooses a Canine Full Set for their dog, mark an (X) next to the
Canine Full Set Package and an (X) next to Rabies, DA2PP, and Bordetella in the
individual services box.

whether or not the animal is altered, and circle prices based on package vs. individual
services. Make a note of any coupons the Client has provided for their services.
Example A: If a Client chooses a Canine Full Set for their altered dog, circle only the
price for the Canine Full Set Package for altered dogs, and do not circle the prices of
the individual vaccines included in the Canine Full Set package.

5. Review all information and services with Client
Review all of the vaccination services to be rendered with the client. (Any special
needs or special notes for the patient may be written in the Vaccination Labels/Notes
box on the right hand sign of the Wellness Sheet.)
Go over all pricing, and total the estimated balance of services on the bottom of the
Wellness Sheet. Have the Client initial the box agreeing to pay for all charges at
check-out.
6. If the Client is a New Client, be sure they fill out the referral box located on the top half of the
Wellness Sheet, and have them sign the New Client disclosure stating that we are not a full-service
clinic.
7. Initial your initials on the top right-hand corner of the Wellness Sheet labeled “Customer Service
Rep Initials” and make any additional notes such as hallway or kitchen location, schedule
spay/neuter surgery, give puppy/kitten coupon, boosters due, etc.,
8. Give the Check-In Number Card to the Client, and let them know that a staff member will be over to
assist their pet(s) when their number is called.
9. Turn the Client’s Wellness Sheet(s) into Data Entry by placing the sheet in the metal file holder in
numerical order, and return to the clipboard to the Check-In podium.

After you have located the Client’s account, please double check that all contact information and referral
information are up-to-date.

POPULATED INFORMATION
Avimark will populate all of the information needed for packages.
RABIES VACCINES
For example, if you select a Dog Health Package, Avimark will populate the individual vaccination services
included. You would enter your Rabies Tag number (if Data Entry is assigning Rabies) in the Vacc ID box
in the Vaccine Information pop-up window and then select DONE or hit enter.
Avimark would then display the Dog Health Package in the Medical History Area
MICROCHIPS
Enter the Microchip number in the Patient Microchip ID Window
Enter the Microchip Number into Medical history, by highlighting the treatment in the Medical History area,
right-clicking with your mouse, and select ‘CHANGE’ OR by highlighting the treatment and hitting F3

RE-ENTER THE INFORMATION
Re-enter the information in “Description Box” and hit ‘ENTER’
The Microchip information should look like this in the Medical History and the Patient information areas

CITY LICENSES
Enter in the City License Number in the ePet Locator Pop-Up Window
Enter in the City License Number into Medical History
City License should show like this in Medical History

TO REMOVE A TREATMENT
Highlight the treatment(s) and right-click. Go to ‘REMOVE’ or hit F4
Confirm removal of the treatment by using the ‘Y’ key or by clicking ‘YES’
FILLING OUT THE WELLNESS CLINIC SHEET
Write the Client’s Account Number, which can be found at the top right hand corner of the Avimark Client
screen, in the Account # Line on the Wellness Sheet.
Write any and all of the following onto the Wellness Sheet, if applicable:
Rabies Tag Number
City License Number
Microchip Number (if previously chipped)
If assigning a Microchip Number, remove one sticker from the Microchip pack and place it in the
dashed rectangle area on the Wellness Clinic Sheet

If only a Rabies tag is assigned, tape the Rabies tags to the bottom right hand corner of the
Wellness Clinic Sheet
If a Rabies tag and a City License are assigned, tape the Rabies tag to the top of the City License,
write the pet’s first and last name on the back of the City License and staple to the back of the top-
left corner of the Wellness Clinic Sheet.
If a Rabies tag, City License, and Microchip are assigned, tape the Rabies tag to the City License
and staple the City License to the Microchip Card, write the pet’s first and last name on the
Microchip Card, and then staple that to the back of the top-left corner of the Wellness Clinic Sheet.
If only a Microchip is assigned, write the pet’s first and last name on the Microchip Card, and staple
it to the top-left corner of the Wellness Clinic Sheet.
All puppy/kitten coupons issued should be paper-clipped to the back of the Wellness Clinic Sheet.
When finished, you may take the completed sheet to the Clinic Dropbox.
