However, the original WLAN had issues with dropped connections and inadequate access points. St. Luke's upgraded to a Cisco wireless infrastructure and deployed NetMotion Mobility software to maintain connections as users moved around. Background
The quality of healthcare is enhanced when patient health information is readily available to
healthcare providers. Even in an increasing electronic environment, use of facsimile (fax)
machines and fax software remains commonplace. Fax machines scan a document and
convert the text to electronic impulses that are then transmitted over telephone lines. The
receiving fax converts the electronic impulses back to text.
Providers may choose to fax health information about a patient, for example, when
intercampus or regular mail delivery proves too slow. Similarly, hospital transcription and
ancillary departments may use fax software to deliver a copy of a dictated report or
laboratory result to the physician's office as soon as the report is transcribed or otherwise
made available. Although fax equipment and software can enhance the quality of healthcare
by facilitating rapid transmission of health information, this same mode of transmission
opens up the possibility that information will be misdirected or intercepted by individuals to
whom access is not intended or authorized. In recent years, numerous reports have described
events wherein patient health records were inadvertently faxed to a wrong location (e.g.,
bank or retail store) rather than the intended recipient.

Background
The quality of healthcare is enhanced when patient health information is readily available to
healthcare providers. Even in an increasing electronic environment, use of facsimile (fax)
machines and fax software remains commonplace. Fax machines scan a document and
convert the text to electronic impulses that are then transmitted over telephone lines. The
receiving fax converts the electronic impulses back to text.
Providers may choose to fax health information about a patient, for example, when
intercampus or regular mail delivery proves too slow. Similarly, hospital transcription and
ancillary departments may use fax software to deliver a copy of a dictated report or
laboratory result to the physician's office as soon as the report is transcribed or otherwise
made available. Although fax equipment and software can enhance the quality of healthcare
by facilitating rapid transmission of health information, this same mode of transmission
opens up the possibility that information will be misdirected or intercepted by individuals to
whom access is not intended or authorized. In recent years, numerous reports have described
events wherein patient health records were inadvertently faxed to a wrong location (e.g.,
bank or retail store) rather than the intended recipient.
Legal and Regulatory Requirements
Most federal regulatory requirements such as HIPAA, the Medicare Conditions of
Participation, and the Confidentiality of Substance Abuse Patient Records do not
specifically address the use of fax equipment or copies. However, the Department of Health
and Human Services addresses fax use in a limited manner through the following HIPAA
guidance:
The HIPAA Privacy Rule permits providers to disclose protected health information to
another health care provider for treatment purposes. This can be done by fax or by other
means. For example:
• A laboratory may fax, or communicate over the phone, a patient's medical test
results to a physician.
• A physician may mail or fax a copy of a patient's medical record to a specialist who
intends to treat the patient.
• A hospital may fax a patient's health care instructions to a nursing home to which the

They had moved out of town but returned to our institution, where the first surgery had been performed successfully. Her family was very anxious because of Mrs. Walsh’s unstable and deteriorating condition, and a family member was always with her 24 hours a day for the first few weeks.
The nurse became involved with this family while Mrs. Walsh was still in surgery, because family members were very anxious that the procedure was taking longer than it had the first time and made repeated calls to the critical care unit to ask about the patient. The nurse met with the family and offered to go into the operating room to talk with the cardiac surgeon to better inform the family of their mother’s status.
One of the helpful things the nurse did to assist this family was to establish a consistent group of nurses to work with Mrs. Walsh, so that family members could establish trust and feel more confident about the care their mother was receiving. This eventually enabled family members to leave the hospital for intervals to get some rest. The nurse related that this was a family whose members were affluent, educated, and well informed, and that they came in prepared with lists of questions. A consistent group of nurses who were familiar with Mrs. Walsh’s particular situation helped both family members and nurses to be more satisfied and less anxious. The family developed a close relationship with the three nurses who consistently cared for Mrs. Walsh and shared with them details about Mrs. Walsh and her life.
The nurse related that there was a tradition in this particular critical care unit not to involve family members in care. She broke that tradition when she responded to the son’s and the daughter’s helpless feelings by teaching them some simple things that they could do for their mother. They learned to give some basic care, such as bathing her. The nurse acknowledged that involving family members in direct patient care with a critically ill patient is complex and requires knowledge and sensitivity. She believes that a developmental process is involved when nurses learn to work with families.
She noted that after a nurse has lots of experience and feels very comfortable with highly technical skills, it becomes okay for family members to be in the room when care is provided. She pointed out that direct observation by anxious family members can be disconcerting to those who are insecure with their skills when family members ask things like, “Why are you doing this? Nurse ‘So and So’ does it differently.” She commented that nurses learn to be flexible and to reset priorities. They should be able to let some things wait that do not need to be done right away to give the family some.

Geely is aiming to be the first China carmaker to design and build satellites to support its autonomous driving programme, the latest step by founder Li Shufu in his bid to build an industry leader.
Geely, which owns Swedish brand Volvo Cars, Malaysia’s Proton, and a stake in Mercedes-Benz owner Daimler, will invest Rmb2.27bn ($325m) in a new development centre and factory to manufacture satellites this year, the company said yesterday.
The announcement makes Geely the first known Chinese carmaker with plans to build its own satellites. Mr Li’s move sparked comparisons in China media with Elon Musk, founder of electric carmaker Tesla and private space exploration company SpaceX.
Last month Geely drew comparisons with Volkswagen when Mr Li’s holding group announced plans to merge Geely Automobile and Volvo Cars, moving the company towards becoming the first global Chinese carmaker.
Che Jun, Communist party boss of China’s eastern Zhejiang province, where Geely is based, said that the complex would be built in Taizhou city and that construction had begun.
The centre will design, test and manufacture low-orbit communication satellites, purpose-built to improve geolocation of vehicles and to support their connected functions, Geely said.
Geely has been pouring money into new technologies from self-driving cars to flying taxis, spending Rmb20bn on research and development in the past year.
The investments are part of the group’s spend on global expansion, such as buying a $9bn stake in Daimler.

Nonprofit organizations are increasingly dependent on the involvement of Millennial
constituencies. Three studies investigated their motivations to support charity-linked
events: emotional identification with a cause, self-determination theory (SDT) regula-
tions, and context-related Facebook promotions. This article addresses the recent call to
expand SDT research from a simple analysis of autonomous versus controlled moti-
vation, to studying the effects of all the regulations in the SDT continuum, in particular,
the inclusion of the tripartite dimensions of intrinsic motivation and integrated moti-
vation. Results demonstrated that the greater the emotional identification with the
cause, the stronger was the tendency to support the charity-linked event. Also, the
results in these social media contexts revealed that specific intrinsic dimensions (e.g.,
experience stimulation) are motivators of online and offline support, as is the personal
value nature of integrated regulation. Whereas only autonomous motivational regula-
tions predicted support for the two events organized specifically a for charitable causes,
both autonomous and controlled regulations predicted support of a for-profit event
organized with a charitable cause as an adjunct. These findings can assist practitioners
in designing more effective social media communications in support of charity-linked
events.

Social media is a new domain offering excit-
ing opportunities to investigate research ques-
tions in social psychology (Greitemeyer, 2011;
Kende, Ujhelyi, Joinson, & Greitemeyer, 2015).
Our research examined motivation to support
charity-linked events of nonprofit organizations
that are currently faced with increased compe-
tition for resources and declining government
support (Paulin, Ferguson, Jost, & Fallu, 2014;
Reed, Aquino, & Levy, 2007; White & Peloza,
2009). Presently, they depend on an ageing set
of traditional supporters (Urbain, Gonzalez, &
Le Gall-Ely, 2013). However, their future suc-
cess lies in ensuring the sustainable involve-
ment of the Millennial generation (Fine, 2009),
distinguished from other generations by their
intense exposure at an early age to interactive
technology and social media (Bolton et al.,
2013).

“Mrs. Walsh, a woman in her 70s, was in critical condition after repeat coronary artery bypass graft (CABG) surgery. Her family lived nearby when Mrs. Walsh had her first CABG surgery. They had moved out of town but returned to our institution, where the first surgery had been performed successfully. Mrs. Walsh remained critically ill and unstable for several weeks before her death. Her family was very anxious because of Mrs. Walsh’s unstable and deteriorating condition, and a family member was always with her 24 hours a day for the first few weeks.
The nurse became involved with this family while Mrs. Walsh was still in surgery, because family members were very anxious that the procedure was taking longer than it had the first time and made repeated calls to the critical care unit to ask about the patient. The nurse met with the family and offered to go into the operating room to talk with the cardiac surgeon to better inform the family of their mother’s status.
· One of the helpful things the nurse did to assist this family was to establish a consistent group of nurses to work with Mrs. Walsh, so that family members could establish trust and feel more confident about the care their mother was receiving. This eventually enabled family members to leave the hospital for intervals to get some rest. The nurse related that this was a family whose members were affluent, educated, and well informed, and that they came in prepared with lists of questions. A consistent group of nurses who were familiar with Mrs. Walsh’s particular situation helped both family members and nurses to be more satisfied and less anxious. The family developed a close relationship with the three nurses who consistently cared for Mrs. Walsh and shared with them details about Mrs. Walsh and her life.
· The nurse related that there was a tradition in this particular critical care unit not to involve family members in care. She broke that tradition when she responded to the son’s and the daughter’s helpless feelings by teaching them some simple things that they could do for their mother. They learned to give some basic care, such as bathing her. The nurse acknowledged that involving family members in direct patient care with a critically ill patient is complex and requires knowledge and sensitivity. She believes that a developmental process is involved when nurses learn to work with families.
· She noted that after a nurse has lots of experience and feels very comfortable with highly technical skills, it becomes okay for family members to be in the room when care is provided. She pointed out that direct observation by anxious family members can be disconcerting to those who are insecure with their skills when family members ask things like, “Why are you doing this? Nurse ‘So and So’ does it differently.” She commented that nurses learn to be flexible and to reset priorities. They should be able to let some things wait that do not need to be done right away to give the famil.

C9-1 CASE STUDY 9 ST. LUKE'S HEALTH CARE SYSTEM Hospitals have been some of the earliest adopters of wireless local area networks (WLANs). The clinician user population is typically mobile and spread out across a number of buildings, with a need to enter and access data in real time. St. Luke's Episcopal Health System in Houston, Texas (www.stlukestexas.com) is a good example of a hospital that
- 4. has made effective use wireless technologies to streamline clinical work processes. Their wireless network is distributed throughout several hospital buildings and is used in many different applications. The majority of the St. Luke’s staff uses wireless devices to access data in real-time, 24 hours a day. Examples include the following: • Diagnosing patients and charting their progress: Doctors and nurses use wireless laptops and tablet PCs to track and chart patient care data. • Prescriptions: Medications are dispensed from a cart that is wheeled from room to room. Clinician uses a wireless scanner to scan the patient's ID bracelet. If a prescription order has been changed or cancelled, the clinician will know immediately because the mobile device displays current patient data.
- 5. http://www.stlukestexas.com/ C9-2 • Critical care units: These areas use the WLAN because running hard wires would mean moving ceiling panels. The dust and microbes that such work stirs up would pose a threat to patients. • Case management: The case managers in the Utilization Management Department use the WLAN to document patient reviews, insurance calls/authorization information, and denial information. The wireless session enables real time access to information that ensures the correct level of care for a patient and/or timely discharge. • Blood management: Blood management is a complex process that involves monitoring both patients and blood products during all stages of a treatment process. To ensure that blood products and patients are
- 6. matched correctly, St. Luke’s uses a wireless bar code scanning process that involves scanning both patient and blood product bar codes during the infusion process. This enables clinicians to confirm patient and blood product identification before proceeding with treatment. • Nutrition and diet: Dietary service representatives collect patient menus at each nursing unit and enter them as they go. This allows more menus to be submitted before the cutoff time, giving more patients more choice. The dietitian can also see current patient information, such as supplement or tube feeding data, and view what the patient actually received for a certain meal. • Mobile x-ray and neurologic units: St. Luke’s has implemented the wireless network infrastructure necessary to enable doctors and clinicians to use mobile x-ray and neurologic scanning units. This makes
- 7. it possible to take x-rays or to perform neurological studies in patient rooms. This minimizes the need to schedule patients for neurology or radiology lab visits. The mobile units also enable equipment to be brought to the bedside of patients that cannot be easily moved. The wireless neurology and x-ray units have also helped to reduce the time between diagnosis and the beginning patient care. C9-3 Original WLAN St. Luke's first WLAN was deployed in January 1998 and made the hospital an early pioneer in wireless health care applications. St. Luke’s first wireless LAN was implemented in a single building using access points (APs) made by Proxim (www.proxim.com). A principal goal of this initial installation was to improve
- 8. efficiency. However, sometimes the WLAN had the opposite effect. The main problem was dropped connections. As a user moved about the building, there was a tendency for the WLAN to drop the connection rather than performing the desired handoff to another access point. As a result, a user had to reestablish the connection, log into the application again, and reenter whatever data might have been lost. There were physical problems as well. The walls in part of the building were constructed around chicken wire, which interfered with radio waves. Some patients' rooms were located in pockets with weak radio signals. For these rooms, a nurse or doctor would sometimes lose a connection and have to step out into the hallway to reconnect. Microwave ovens in the kitchenettes on each floor were also a source of interference.
- 9. Finally, as more users were added to the system, the Proxim APs, with a capacity of 1.2 Mbps, became increasingly inadequate, causing ongoing performance issues. Enhanced LAN To overcome the problems with their original WLAN and reap the potential benefits listed earlier in this case study, St. Luke's made two changes [CONR03, NETM03]. First, the hospital phased out the Proxim APs and replaced them with Cisco Aironet (www.cisco.com) APs. The Cisco APs, using IEEE 802.11b, operated at 11 Mbps. Also, the Cisco APs used direct C9-4 sequence spread spectrum (DSSS), which is more reliable than the frequency-hopping technique used in the Proxim APs.
- 10. The second measure taken by St Luke's was to acquire a software solution from NetMotion Wireless (netmotionwireless.com) called Mobility. The basic layout of the Mobility solution is shown in Figure C9.1. Mobility software is installed in each wireless client device (typically a laptop, handheld, or tablet PC) and in two NetMotion servers whose task is to maintain connections. The two servers provide a backup capability in case C9-5 one server fails. The Mobility software maintains the state of an application even if a wireless device moves out of range, experiences interference, or switches to standby mode. When a user comes back into range or switches into active mode, the user's application resumes where it left off. In essence, Mobility works as follows: Upon connecting, each
- 11. Mobility client is assigned a virtual IP address by the Mobility server on the wired network. The Mobility server manages network traffic on behalf of the client, intercepting packets destined for the client's virtual address and forwarding them to the client's current POP (point of presence) address. While the POP address may change when the device moves to a different subnet, from one coverage area to another, or even from one network to another, the virtual address remains constant while any connections are active. Thus, the Mobility server is a proxy device inserted between a client device and an application server. Enhancing WLAN Security In 2007, St. Luke’s upgraded to Mobility XE mobile VPN solution [NETM07]. This migration was undertaken to enhance security and compliance with
- 12. HIPPA data transmission and privacy requirements. Mobility XE server software was deployed in the IT department’s data center and client software was installed on laptops, handheld devices, and tablet PCs. With Mobility XE running on both clients and servers, all transmitted data passed between them is encrypted using AES (Advanced Encryption Standard) 128-bit encryption. Mobility XE also serves as an additional firewall; devices that are not recognized by the Mobility XE server are not allowed to access the network. This arrangement helped St. Luke’s achieve its IT goal of having encryption for all wireless data communications. Mobility XE also enables the IT department to centrally manage all wireless devices used by clinicians. This allows them to monitor the C9-6
- 13. applications currently being used by any device or user, the amount of data being transmitted, and even the remaining battery life of the wireless device. If a Mobility XE device is stolen or lost, it can be immediately quarantined by network managers. IT executives at St. Luke’s view wireless networking as key lever in their quest to increase clinician productivity and improved patient care. Mobile EKG units have been deployed bringing the total of wireless devices in use to nearly a 1,000. Discussion Questions 1.
