Mental health has been in the news quite a bit lately. Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).

Organizational culture includes values, norms, systems, symbols, language, assumptions, beliefs, and habits that influence employee behaviors and how people interpret those behaviors. It is important because culture can help or hinder a company's success. Some key aspects of Netflix's culture that help it achieve results include hiring smartly so every position has stars, focusing on attitude over just aptitude, and having a strict policy against peacocks, whiners, and jerks.

Un home de 26 anys era addicte a la inhalació de dissolvents des que en tenia 14. Va in-
toxicar-se inhalant-ne un que contenia un 70 % de toluè i en va ingerir uns 100 mL en confon-
dre'l amb aigua. El matí següent el seu germà el castigà lligant-li els canells i els turmells junts
per darrere de l'esquena i enganxats a un piló del jardí; el va tenir així nou hores al sol, sense
donar-li beguda. El pacient va estar intentant amb totes les seves forces de deslliurar-se, però no
va aconseguir-ho. A la tarda el van deslligar i, en veure que no podia moure els braços, el van
portar a l'hospital.
Estava conscient però lleugerament excitat. La seva temperatura era de 37,2 °C i la
pressió arterial, de 154/32 mm Hg (!) amb 98 batecs/min. Les quatre extremitats estaven
inflades i sensibles i tenia abrasions lineals voltant-li els canells i els turmells.
Quins poden ser els motius d'aquesta paràlisi?.
Quin tractament necessita?. Cal ingressar-lo?.
Com que tenia les dues extremitats superiors paralitzades, van sospitar una parèsia dels
plexes braquials. Patia també oligúria (232 mL d'orina el primer dia, i 364 mL el segon, a pesar
de què van donar-li grans dosis de furosemida).
Els resultats de les anàlisis de laboratori van ser: hemoglobina, 15,9 g/dL; hematies, 4,9
milions/µL; leucòcits, 18 500/µL; plaquetes, 146 000/µL; creatinina = 9,7; àcid úric = 16,0,
bilirubina total = 0,4, tot en mg/dL; SGOT = 664, SGPT = 235, CPK = 45 450 i LDH = 3250,
tot en UI/L; Na+
= 115, K+
= 5,8, Cl-
= 86 i Ca2+
= 2,3, tot en mEq/L; l'osmolaritat sèrica era de
288 mOsm/kg i la urinària, de 285. La gasometria arterial va donar la següent informació: pH =
7,18, PO2 = 127 mm Hg, PCO2 = 13 mm Hg, excés de bases = -24 mEq/L. La radiografia
toràcica va ser normal.
Quins són els motius de l'oligúria?.
Cal algun tractament abans d'arribar al diagnòstic causal?.
La concentració de mioglobina sèrica era > 500 mg/L (N < 60). Van afegir al diagnòstic
una insuficiència renal aguda secundària a rabdomiòlisi massiva.
Van hemodialitzar-lo 9 cops els primers 15 dies d'estada, mentre la funció renal tornava
a poc a poc a la normalitat: la creatinina es va normalitzar a les cinc setmanes. També van fer-
ho, en 10 dies, els enzims sèrics AST, ALT i LDH.
El 43è dia d'estada hospitalària van fer-li una biòpsia renal i van diagnosticar una
degeneració dels túbuls distals.
Van confirmar la parèsia dels plexes braquials per electromiografia. En ser donat d'alta
hospitalària, el 83è dia, encara no s'havia recuperat del tot.
2. LETÀRGIA I DESORIENTACIÓ

Van portar a Urgències un home de 67 anys, amb letàrgia, desorientació i incontinència.
Era obès, però sense antecedents de convulsions, feridures o altres alteracions neurològiques o
cardiovasculars. Patia hipertensió des de feia anys, un trastorn bipolar de cinquanta anys d'e-
volució, que tractava amb liti els darrers anys i a penes complia les prescripcions terapèutiques.
Estava prenent carbonat de liti (2,1 g/d), perfenacina (16 mg/12 h), carbamacepina (200 mg/
12 h), hidralacina, verapamil i hidroclorur de quinapril.
Enllà el van trobar amb constants vitals estables, sense febre i amb una pressió arterial
de 160/100 mm Hg. L'exploració física era normal, llevat de l'adormiment i la desorientació.
Quines poden ser les causes del seu estat?.
La concentració sèrica de creatinina era de 255 µmol/L; la de liti, de 4,04 mmol/L i la
de carbamacepina, de 2,8 mg/L (ventall terapèutic = 8-12). En el líquid cefalo-raquidi van
trobar una concentració de proteïnes de 65 mg/dL.
Aquesta concentració de liti, és tòxica?.
Li van perfondre líquids i la litièmia del pacient va anar baixant a 3,24 mmol/L, però la
seva consciència va anar deteriorant-se fins el coma i la creatinina, pujant a 409 µmol/L. El
quart dia d'estada li van fer una hemodiàlisi i el dia següent la consciència i la funció renal
havien millorat.
En vàries setmanes d'ingrés, el pacient va anar recuperant l'alerta i l'orientació, però van
aparèixer disàrtria i tremolors grollers a les extremitats. La concentració sèrica de liti era de
0,15 mmol/L i la de creatinina, de 164 µmol/L. Tenia una hipereflèxia lleu en el costat
esquerre, amb signe de Babinski positiu, absència de reflexes aquilis i dificultat en fer les
proves "dit al nas" i "taló a canyella".
Què li està passant ara al pacient?.
Van donar-li àcid valproic per tractar el trastorn bipolar i antihipertensors, però, sense
recuperar-se, va començar a patir mania i elevacions intermitents de la pressió arterial fins a
220/120 mm Hg. El pacient va morir inesperadament a les onze setmanes d'estada.
Quins factors poden haver col×laborat en la intoxicació per liti d'aquest pacient?.
Quines conclusions treiem d'aquest cas en relació amb l'educació sanitària?.
3. VÒMITS I DIARREA

Vora les dotze del migdia, una parella van trobar sa filla de setze mesos en el pati de
casa, que havia ingerit part del contingut d'un paquet de formicida que contenia triòxid
d'arsènic; li van prendre i van rentar-li de la cara les restes del producte. Abans de mitja hora, la
nena va vomitar espontàniament i va patir una diarrea aquosa i els pares la van portar a un
hospital local.
Quines manifestacions podem esperar de la intoxicació oral per triòxid d'arsènic?.
Quin tractament adoptaríeu?.
Enllà li van administrar 15 mL de xarop d'ipecacuana, que la va fer vomitar abun-
dantment, i 20 g de carbó activat amb 2,5 g de sulfat sòdic. La nena va quedar letàrgica després
de vomitar i va patir tres episodis més de diarrea "en aigua d'arròs", sense que aparegués carbó
en la femta. Estimant que havia ingerit 2-3 g d'insecticida (9-14 mg d'As2O3) i que pesava 10
kg, a les cinc de la tarda van injectar-li 25 mg de BAL intramuscular i decidiren traslladar-la
per helicòpter a un hospital de referència.
Enllà van trobar-la aparentment bé: 130-150 batecs/min, pressió arterial de 110/55 mm
Hg, 36 respiracions/min i 36,7 °C rectals. Les anàlisis de laboratori (glucosa, creatinina, ions,
funció hepàtica i paràmetres hematològics i urinaris) van ser totes normals. Els resultats de les
determinacions d'arsènic en sang i en orina no estaven encara disponibles.
Han passat unes sis hores des de la intoxicació. Podem esperar noves manifestacions o
alguna complicació?.
Van ingressar-la a la UCI pediàtrica i van continuar la teràpia quelant: tres dosis de 45
mg de BAL i.m., a les 22:30, a les 02:30 i a les 06:30 i una dosi final de 25 mg a les 10:30.
Simultàniament van començar a donar-li D-penicil×lamina oral: 250 mg/6 hores, començant a
les 02:30 i durant cinc dies.
La freqüència cardíaca de la pacient va normalitzar-se i a les 10 hores va defecar bona
part del carbó activat. Els altres dos dies d'estada no van tenir incidències.
Per què van canviar de quelant?.
Trobeu justificada la intensa teràpia practicada?.
4. DISTENSIÓ ABDOMINAL I FEBRE

Una nena de 16 mesos va vomitar i començà a patir febre i taquipnea. Es va posar cada
cop més irritable i se li va distendir l'abdomen, motiu pel què van decidir portar-la a Urgències.
En arribar tenia 39 °C, 172 batecs i 52 respiracions/min i una pressió arterial de 118/80
mm Hg; li van estimar una pèrdua d'aigua d'un 10 %.
Les anàlisis de laboratori donaven resultats normals, tenint en compte la deshidratació.
Per què pot haver-li passat això?.
En una radiografia se li veia un cos estrany radioopac rodó allotjat a la part superior del
tòrax. Els pares recordaven trobar en falta una pila alcalina del flash de la seva càmera des de
feia tres dies.
Les piles contenen una solució d'hidròxid potàssic al 45 % (» 8 N). Quina mena de lesió
causa això?.
Amb l'esofagoscopi van recuperar la pila, de 22 mm f i 5 mm de gruix. La porció d'e-
sòfag que l'envoltava estava necròtica, tacada de negre i probablement perforada. Mitjançant
una toracotomia li van treure uns 100 mL d'un líquid de color de palla barrejat amb partícules
negres.
La pila és del tamany de una moneda de 1 €. Com és que va quedar trabada i no va
arribar a l'estómac?.
Van donar a la nena clindamicina intravenosa i líquids i al principi va romandre estable,
però després va haver de tornar a quiròfan per a que li drenessin el mediastí i per a instaurar-li
una gastrostomia d'alimentació.
Unes dues hores després va patir una aturada càrdio-respiratòria. Durant els intents de
reanimació li van treure una gran quantitat de sang de l'estómac, però la nena va morir.
En l'autòpsia van diagnosticar com a causa de mort l'hemorràgia secundària a la per-
foració. L'examen microscòpic va mostrar una necrosi liqüefaent que afectava la mucosa i la
submucosa esofàgiques.
5. Ama /09

Baró de 44 anys d'edat, trobat al seu domicili per familiars, amb un sever quadre de
desorientació i visió borrosa després del consum abusiu d'alcohol els dies previs. Quan és
ingressat està conscient, orientat i refereix visió borrosa. Els familiars refereixen consum
excessiu de benzodiazepines i un increment del consum d'etanol els dies previs. A més, no
descarten la possibilitat del consum d'altres tòxics (colònia).
Quin tòxic, relativament característic en individus amb etilisme crònic, podem sospitar que
hi està implicat?
En l'exploració destaca la midriasi bilateral arreactiva i l'auscultació cardiopulmonar normal,
com a la resta de l'exploració física. En les exploracions complementàries destaca a la
gasometria: pH: 7.19, HCO3: 6,8 mm/l, Anió Gap: 30.
Sospitant inicialment una intoxicació benzodiazepínica, el pacient és traslladat a l'UCI i
s'instaura el tractament.
Quin tractament instauraries?
La família no descarta el consum de colònia, dada que s'afegeix a la dificultat de revertir
l'acidosi i la nul·la activitat del flumazenil, la qual cosa reorienta la sospita clínica inicial cap
a la intoxicació per metanol, i s'instaura el tractament.
Quina és la dosi tòxica d'aquest tòxic?
Quin és el tractament?
Un cop estabilitzat el pacient, s'efectua una valoració oftalmològica: no percep la llum i té les
pupil·les midriàtiques dèbilment reactives. Al fons d'ull, les papil·les són lleument pàl·lides,
sense hemorràgia ni edema. El mateix pacient reconeix el consum d'altres formes d'alcohol
més econòmiques.
El pacient és donat d'alta vuit dies després de l'ingrés en situació de ceguesa bilateral.
Quin és el mecanisme de la toxicitat ocular?
6. COMA
Van portar al Servei d’Urgències del Hospital a una noia de 14 anys
procedent d’un disco-bar. La pacient estava inconscient (Glasgow 5),
presentaba signes clínics de hipoventilació i midriasis bilateral. Es va protegir la
via aéria i es va col.locar mascareta amb O2 al 50%. Les seves constants van
ser normals.
No va respondre a varies dosis de Naloxona i Flumacenil i va ser
necessari ventilar a la pacient amb Ambu.
No es va observar signes de venopunció i el TAC cranial va ser normal.
Per què buscar signes de venopunció i fer un TAC?
La analitica no presentaba alteracions i la gasometria mostraba: pH 7.32;
pCO2 52 mmHg; pO2 81 mmHg.
Als 40 minuts va despertar sobtadament. No es va detectar drogues
d’abús a orina ni psicofàrmacs i altres depresors a plasma, amb excepció de 50
mg/dL d’etanol a sang.
Aquesta alcoholemia justifica el coma?
La noia va negar sempre el consum de droga i va ser donada d’alta el día
seguent a l’ingrès. Una setmana desprès la noia va acudir a revisió al Servei de
Toxicología Clínica i va referir que el noi de 17 anys que la va portar la nit de
l’ingrès va posar-li sense que ella es donés compte un líquid a la beguda que
estava prenent. Interrogat el noi va confessar que buscava un efecte afrodisiac
amb aquesta combinació.
7. Cas Clínic 75/09
Un home de 66 anys d'edat, sense antecedents patològics d'interès, va beure un glop d'una
ampolla que contenia una solució al 20 % de paraquat. En beure'l, va notar doloriment faringi i
retroesternal. Mitja hora després va començar a vomitar i a patir una diarrea verdosa.
Després de passar per un hospital menys equipat, va arribar a l'hospital cinc hores i
mitja després de la intoxicació. Se li va rentar l'estómac i se li va administrar solució de manitol
al 10 % en aigua amb terra de Fuller. La concentració urinària de paraquat era > 100 mg/mL.
Minuts després, l'ingressaven a la UCI.
Aquestes concentracions de paraquat, són tòxiques?
És important aquest retard en l’atenció?
Caldria, doncs que tots els hospitals estessin equipats per atendre les intoxicacions per
paraquat?
En el moment de l'ingrés, l'examen físic, les concentracions de glucosa, urea, creatinina,
GOT, GPT, LDH, Na i K, l'ECG i la radiografia de tòrax eren normals. Es van afegir al
tractament famotidina, sulfat de magnesi i 3 mg/kg/min de dopamina.
A les 24 hores tenia una pO2 de 53 mm Hg respirant aire ambiental i a les 36 hores patia
taquipnea i depressió de consciència progressiva fins el coma, motiu pel qual se'l va intubar.
En iniciar l'hemoperfusió contínua amb cartutxos de carbó activat (Hemosorba CH-
350R
), el pacient va patir una hipotensió no controlable.
El seu estat hemodinàmic va anar empitjorant a pesar del tractament, així com també la
funció renal. Va pressentar distress respiratori i va morir al 8é dia.
Qual penses que ha estat la causa de la mort?
Aquesta evolució, era esperable?
8. Cas 78/09
Una dona de 78 anys és portada a urgències després que la trobessin al seu
domicili inconscient. La família va manifestar que es trobava en tractament amb
neurolèptics i antidepressius tricíclics, i es va deduir que n'havia ingerit al
voltant de 2,5 gr d'amitriptilina.
En ingressar, la pacient estava comatosa, amb una pressió arterial de 93/68 mm Hg,
135 pulsacions/min i una temperatura rectal de 36,8 ºC. A l'exploració de la pell i
mucoses, es mostraren seques, les pupil·les dilatades amb mínima reactivitat a la
llum, i una disminució del peristaltisme intestinal.
Quin és el diagnòstic sindròmic?
Quins factors poden haver influït en la farmacocinètica d'aquesta intoxicació?
A l'ECG presentava taquicàrdia sinusal amb un complex QRS de 150 ms i un
interval QT de 250 ms. La radiografia de tòrax era normal. L'analítica mostrava una
leucocitosi, Na+
136mEq/L; K+
3,9 mEq/L; Ca2+
9 mg/dL, i glucèmia de 156
mg/dL. La gasometria arterial mostrava un pH de 7,41; pO2 de 58 mm Hg, i pCO2 de
34 mm Hg.
Comenta el quadre clínic que presenta.
Quin és l'indicador més sensible de cardiotoxicitat?
Van intubar la pacient, se li va realitzar un rentat gàstric i se li va administrar carbó
activat i citrat magnèsic, i se li va corregir el desequilibri hidroelectrolític.
És correcte el tractament? Pel que fa al rentat gàstric, s'ha de fer encara que hagin
passat hores des de la ingestió d’ADT?
La pacient va ser extubada, va passar de l'UVI a la planta i l'evolució clínica va ser
satisfactòria.
9. CASO 7/03
Niño de 7 años, procedente del medio rural, que es traido por sus padres
al hospital por presentar un cuadro de desorientación e intranquilidad, de
aparición repentina, sin otra sintomatología acompañante. Los padres negaron la
posibilidad de intoxicación medicamentosa (ADT, anfetaminas) o ingesta
alcohólica. A su llegada a urgencias, destacaba un cuadro confusional con
desorientación y agitación psicomotriz. Temperatura axilar 37 ºC; FC:130
lat/min; FR; 26 resp/min; presión arterial 90/50 mmHg: En la exploración física
presentaba estado general conservado, buena perfusión periférica, rubicundez
facial y sequedad de mucosas. El examen neurológico: midriasis bilateral
arreactiva, no focalidad neurológica ni signos meningeos.
Que sindrome presenta?
A que puede ser debido?
La ausculatación cardiopulmonar era normal y en la exploración
abdominal se apreciaba disminución de los ruidos intestinales. Los parámetros
analíticos fueron normales. El ECG reveló taquicardia sinusal y la TC craneal
fue normal.
Que tratamiento se realizaría?
Con el diagnostico clínico de sindrome anticolinérgico se procedió a la
monitorización y observación del paciente. Se realizó lavado gástrico seguido
de administración de carbón activado, purgante salino y diuresis forzada. Fue
necesaria laadministración de diacepan intravenoso.
Después de interrogar a los amigos del niño, los padres traen las semillas
de una planta, identificada como Datura stramonium, que el niño había ingerido
3-4 horas antes del inicio del cuadro, tras recogerla en el campo.
El tratamiento instaurado es correcto?
Dado el tiempo transcurrido, es eficaz el lavado gástrico?
10. CREMADES

Un home de 23 anys estava treballant en una fàbrica amb un recipient que contenia pols
de nitrat sòdic i nitrat potàssic. Quan l'havien calentat a 246 °C, el recipient va explotar i l'home
va quedar recobert d'aquests productes. Li van treure la roba immediatament i se'l van endur en
ambulància a l'hospital.
A Urgències tenia cremades de segon i tercer grau, que afectaven el 70 % de la seva
superfície corporal, incloent el cap, el coll, l'esquena, dues terceres parts del pit i les dues
extremitats superiors. Les àrees cremades estaven recobertes d'una crosta blanquinosa,
semblant a sal. El pacient estava pàl×lid, amb una certa morenor dels llavis i de la mucosa oral.
Estava despert i contestava les preguntes de forma coherent i amb veu normal. Encara que va
dir que li costava una mica respirar, no estava taquipneic i l'auscultació pulmonar era normal.
La seva pressió arterial era de 90/60 mm Hg, amb 110 batecs/min. Les venes superficials
estaven col×lapsades i les parts acres, fresques i humides.
Quin és el diagnòstic sindròmic?.
Quines complicacions poden esperar-se?.
Quin tractament cal instaurar?.
Li van rentar les crostes amb sèrum fisiològic, sense desbridar les cremades, i li van
dissecar la safena, per inserir-li una via venosa. Encara que el pacient es queixava de dolor, no
li van donar narcòtics perquè patia hipotensió.
És una decisió correcta?.
Deu o quinze minuts després de l'ingrés va anar quedant-se cianòtic i inconscient i va
patir una convulsió generalitzada. El van intubar immediatament i van administrar-li oxigen al
100 %. Uns minuts després, va patir una aturada cardíaca, de la que no el van poder reanimar.
En l'autòpsia van trobar una traqueobronquitis lleu i l'inici d'un edema pulmonar.
Quina va ser, doncs, la causa de la seva mort?.
Amb quina prova confirmareu el diagnòstic?.
En una mostra de sang recollida en l'autòpsia es va trobar un percentatge de
metahemoglobina del 65 %.
En conjunt, podem dir que l'actuació mèdica ha estat correcta?. Si no és el cas, quins
han estat els errors?.
11. DEPRESSIÓ DE CONSCIÈNCIA

Un home de 65 anys va ingressar a la UCI per angina inestable. Era hipertens i fumador,
però no tenia antecedents d'hepatopatia o nefropatia. El van tractar amb 25 mg/d de diacepam
oral, 125 mg/d de salicilat oral, 10 mg/h d'heparina sòdica (1 mg = 100 U) i 45 µg/min de
nitroglicerina i.v.
El tractament, és adequat?.
Com que va patir nous episodis d'angina amb hipertensió arterial i no es va poder fer
una coronariografia urgent, van afegir al tractament 240 mg de propranolol i 30 mg de nifedipí
orals/d; també van anar augmentant les dosis de nitroglicerina fins als 242,7 µg/min el quart
dia.
A les poques hores, el pacient va patir disàrtria i estupor i va entrar en coma.
Quin és el diagnòstic diferencial?.
Van suspendre les perfusions d'heparina i nitroglicerina i van demanar una alcoholèmia
i una TAC cerebral. Com que l'alcoholèmia era de 69 mmol/L, van deixar anar la TAC. Van
reinstaurar la perfusió d'heparina i van administrar els nitrats per via transdèrmica. Vuit hores
després, el pacient va tornar al seu estat mental normal.
Augmentant les dosis de fàrmacs orals van desaparèixer les crisis anginoses i el pacient
fou donat d'alta de la UCI tres dies després.
L'alcoholèmia, és important?. Com pot haver-se intoxicat?.
Hi ha intervingut algun dels fàrmacs que rebia?.

Un home de 37 anys i les seves dues filles, de 12 i 14 anys, van anar a Urgències de
medicina general perquè patien basques, vòmits i diarrea abundant.
L'examen físic dels tres era normal i van enviar-los a casa després de prescriure'ls carbó
activat.
A pesar del tractament, les manifestacions persistien i els afectats van anar a Urgències
de l'hospital trenta-sis hores després de la primera visita. Enllà els van trobar cansats, però
l'examen físic continuava sent normal, llevat d'una sensibilitat abdominal lleu i difusa. En les
anàlisis bioquímiques sanguínies van trobar-los aquestes concentracions d'AST i ALT: el pare,
de 3402 i 6720, la filla petita, de 990 i 1097 i la gran, de 638 i 847 UI/L. El pare tenia una
hemoglobina de 19,4 g/dL i una creatinina de 127 mmol/L (N = 50-110).
Quines poden ser les causes del seu trastorn?.
Quines proves poden fer-se per diagnosticar-lo?.
Sospitant una intoxicació per bolets, van tornar a preguntar-los i van saber que unes
quinze hores abans de la primera visita n'havien menjat. Després els van trobar restes d'a-
amanitina en l'orina.
Els van donar líquids intravenosos, carbó activat oral i una perfusió de dosis altes de
penicil×lina G i sil×libinina.
Per què els donen carbÇó activat?.
Quines complicacions podeu esperar?.
L'evolució clínica de tots tres va ser favorable, i els paràmetres hepàtics van
normalitzar-se en pocs dies.
13. DOLOR ABDOMINAL I CONFUSIÓ

Van portar a Urgències un home de 24 anys que patia des de feia sis hores basques,
vòmits, dolor abdominal greu i confusió.
Estava letàrgic i desorientat, amb mal aspecte. Tenia miosi intensa, ptosi bilateral i visió
borrosa, fasciculacions de la llengua i disàrtria. Estava flàcid i hiporeflèxic. La seva pressió
arterial era de 100/80 mm Hg, amb 40 batecs/min, disrítmics; sense febre però suant molt.
Després de preguntar-li molts cops, va admetre que, deu hores abans, havia begut per
curiositat suc de taronja on havia posat, picades, sis fulles que havia recollit del terra. No tenia
antecedents mèdics d'interés.
Quina síndrome pateix, que ens permeti suposar la planta tòxica?.
Mentre arribaven al diagnóstic, van fer-li un rentat gàstric, van donar-li 50 g de carbó
activat en sorbitol al 70 %/4 hores i van infondre-li glucosa al 5 % en sèrum salí al 0,45 %.
En les anàlisis de laboratori destacaven 19 800 leucòcits/mm3
, amb 93 % de granulòcits,
fosfat = 0,3 mmol/L i potassi = 5,2 mEq/L; el calci i el magnesi eren normals. En l'ECG es
veien un blocatge aurículo-ventricular, amb ritme nodal (40/min) i una depressió ST difusa.
Aquest electrocardiograma, ens orienta sobre la causa?.
Sospitaven una intoxicació per glicòsids digitàlics i una mica després ho van confirmar
en identificar les fulles com de baladre (Nerium oleander) i en trobar 1,0 nmol/L de digoxina en
el radioimmunoassaig.
Com es tracta la intoxicació per baladre?.
Tot i administrar-li una resina d'intercanvi iònic, la potassèmia va pujar fins a 6,5
mmol/L, coincidint amb una hipofosfatèmia. Van infondre-li 50 mL de glucosa al 5 % amb 10
U d'insulina i 44 mmol/L de bicarbonat, a més de solucions amb fòsfor i magnesi, tot sense
èxit.
Per quin mecanisme rebaixa la potassèmia aquesta infusió?.
A pesar del tractament de suport apropiat, l'estat del pacient va anar empitjorant: coma,
hipotensió, oligúria i creatininèmia de 155 µmol/L.
Van decidir provar amb anticossos anti-digoxina: 480 mg i.v. en mitja hora. Va millorar
radicalment en pocs minuts: es va despertar, la seva parla es va fer coherent i van desaparèixer
les basques i la diplòpia. El ritme cardíac va tornar a ser sinusal, amb un interval PR de 0,26
mil×lisegons i depressió del segment ST; la pressió arterial va pujar a 100/60 mm Hg. La
potassèmia i la fosfatèmia van tornar a la normalitat.
La recuperació va ser continuada i li van donar l'alta.

Un home de 61 anys va venir a la consulta perquè des de feia un any patia astènia,
anorèxia, fatigabilitat, insomni, nerviosisme, irritabilitat, dolors abdominals i restrenyiment. La
seva exploració física era normal, sense ribet de Burton. El pacient treballava des de feia trenta
anys en una foneria de bronze i antimoni i darrerament s'ocupava també de recuperar bateries.
Quins són els diagnòstics més probables?.
Les anàlisis bioquímiques generals, la coagulació i les funcions hepàtica i renal van ser
normals. No patia anèmia ni li van trobar puntejat basòfil en els eritròcits. La plumbèmia era de
78 µg/dL, la concentració de la protoporfirina lligada al zinc (ZPP), de 382 µg/dL (N < 200), la
plumbúria, de 115 µg/L, la concentració d'àcid delta aminolevulínic en orina era de 3,5 mg/dL
(N < 0,5) i l'excreció de coproporfirines en orina era de 770 µg/24 hores (N < 283).
Per què van pensar en una intoxicació per plom?.
És aquesta clínica d'intoxicació crònica per aquest metall?.
Necessita algun tractament?. Quines alternatives hi ha?.
Com que el pacient rebutjava les injeccions d'EDTA i era al.lèrgic a la penicil.lina, cosa
que contraindicava l'ús de penicil.lamina, el van tractar amb DMSA (25 mg/kg/dia, 5 dies a la
setmana durant dues setmanes), a més de separar-lo de l'exposició laboral al plom.
El tractament va augmentar l'aclariment del metall, de manera que la quantitat total de
plom evacuada va ser de 9480 µg i l'últim dia de tractament la plumbèmia era de 10 µg/dL.
L'excreció de coproporfirines i d'àcid delta aminolevulínic van anar normalitzant-se, tot i què
durant uns dies la plumbèmia va tornar a 50 µg/ dL. L'estat clínic del pacient va millorar i els
únics efectes secundaris apareguts van ser un episodi de diarrea a les 24 hores de l'inici del
tractament i boca seca i amargant durant la primera setmana d'aquest.
Perquè augmenta la plumbèmia després del tractament quelant?.
Que tractament quelant en cas d’existéncia d’encefalopatía saturnina utilitzaries?

Niño de 16 años que ingresa en el hospital por estado mental y emocional alterado. Unos días
antes el paciente presentaba diarrea. Una semana antes del ingreso desarrolla dolor de
garganta, tos progresiva y febrícula. Un día antes del ingreso comenzó a “hablar
incoherencias”, fue visitado en otro hospital donde mencionó algo que sugirió inyección de
drogas ilícitas, se le administró Haloperidol por episodio de agitación. Posteriormente
comenzó a hiperventilar y se tornó “rígido”, remitiendose de urgencia al segundo hospital.
Causas de un estado confusional agudo en un adolescente?
El paciente había nacido en Camboya pero vivía en España desde la infancia. La madre le
había dado un “te herbal” por la diarrea..
Al examen físico estaba somnoliento, realizando muecas, desorientado en el tiempo,
logorreico, con sacudidas de los músculos de brazos y piernas, y posteriormente se tornó
rígido con contracción sostenida de los músculos de la mandíbula, todo ello con estado de
conciencia conservado. La radiología de columna cervical, torax, abdomen, TAC cerebral,
analítica general y punción lumbar fueron normales.
Cuales serían los diagnósticos diferenciales?
Se realizó una impresión diagnóstica de intoxicación por estricnina dadas las intensas
convulsiones que involucraban todo el cuerpo con el estado de conciencia prolongado.
Como se pudo intoxicar con estricnina?
La orina del paciente se analizó detectándose estricnina. La fuente de estricnina no pudo ser
identificada. Fueron descartados drogas de abuso, rodenticidas o plaguicidas. La única fuente
sospechosa de estricnina era la preparación herbaria que el paciente tomó. Durante la tarde
del segundo día en el hospital el paciente tenía otro episodios de rigidez sostenida que mejoró
después de otra dosis de lorazepam. La hiperreflexia y fasciculaciones disminuyeron y
finalmente cesaron al día siguiente. Fue dado de alta 11 días después.
