This document contains a 22 question survey for a student to complete about their preferences for different classroom activities. The student is asked to answer "Yes" or "No" to questions about whether they like making things, watching films, listening to music, and other activities related to arts, sports, lessons outdoors, school trips, dancing, writing, talking, show and tell, debating, drawing diagrams, and different classroom tools. They are told to only answer questions that apply to them.
