Healthcare regulations, funds, workload, and technology continue to complicate and inconvenient the U.S healthcare system. However, the quality and value of care tops. In the United States of America, despite significant healthcare transformation efforts, poor care lingers a considerable concern.
America is second to none in terms of healthcare expenditure across the globe. Ironically, evidence shows that its citizens do not receive the most appropriate care, or at least, which they need. For instance, Graban (2018) documents that preventive care is underutilized in the country, which is escalating the budget of managing advanced diseases. On the other hand, patients of chronic ailments such as diabetes, hypertension, and cardiac complications, do not also usually get treatments that are proven and effective (Wiler, Pines, & Ward, 2019). According to Strome (2019), this case is particularly true and event rampant to the persons that insured, uninsured, or under-insured. The lack of proper coordination of chronic diseases patients' care would only source more or exuberate poor healthcare. The unsurprising healthcare system's underlying fragmentation only fuels the issue given that many health care providers hardly have the payment support such related gears, necessary for effective communication and coordination to improve patient care.
While a significant number of patients miss medically necessary care, other clients get unnecessary or even unsafe attention. Some experts say changes in delivery will erase need for more physicians
You've heard the grim prognosis many times before. Unless immediate action is taken, the U.S. supply of doctors will be 91,500 short of the number needed by 2020 and 130,600 physicians short by 2025, according to an Association of American Medical Colleges estimate.
Dr. Atul Grover, chief public policy officer at the AAMC, projects that the U.S. needs to train an additional 4,000 doctors a year to avoid a shortage, given the Obamacare insurance expansion, an aging physician workforce, shorter hours worked by younger physicians, and an aging population.
But while some physician and hospital organizations wait for the federal government to pump more money into graduate medical education programs to train additional doctors, other health systems and businesses are using the physician shortage as an opportunity to roll out more cost-effective delivery systems and deploy different types of professionals to provide healthcare.
These innovative organizations are figuring what work needs to be done by a doctor and what work can better be done by different types of providers. They are optimizing the use of different professionals in patient-centered medical homes, accountable care organizations and retail clinics, which may well reduce the number of physicians needed. Some of these changes have been tenaciously opposed by organized medicine, but many physician leaders are embracing the new models.
The physician-shortage crisis is based on assumptions that "could be far from the mark � if the production function for primary care can, indeed, be changed," wrote Dr. Thomas Bodenheimer, adjunct professor at the University of California at San Francisco, and Dr. Mark Smith, president and CEO of the California HealthCare Foundation, in the November issue of Health Affairs, which focused on physician workforce issues. They said the shortage issue could be solved with technology and reallocation of responsibilities.
Dr. Scott Shipman, AAMC director of primary-care affairs, wrote in the same issue of Health Affairs that if physicians reassigned 30 minutes of their daily clerical tasks to a nonphysician in their office and spent that time with one patient, it would generate between 30 million and 40 million more physician visits a year. That's exactly what patient-centered medical-home practices are trying to do.
The new delivery models offer hope in the face of the AAMC's bleak outlook. According to the association, the biggest obstacle to increasing the physician workforce is that Medicare funding of physician training has been essentially frozen since 1997.

The foreign nation that I have chosen to work for is the Republic of Kenya; one of the East African countries. I will be working as a healthcare finance manager. The job description of a health finance manager in Kenya and health finance manager in the United States of America are slightly different and so are the education and experience requirements for the job. In addition, the salaries are also different as Kenyan healthcare finance managers are paid significantly less than their counterparts in the US.
The first requirement on becoming a healthcare finance manager in Kenya is a bachelor’s degree in finance, accounting, or any business related degree. It is also necessary that one has a master’s degree that is related to the above-mentioned fields of study. Earning a master of Health administration places one at a higher chance of securing a good job. On the other hand, In the US require candidates seeking the position of health finance managers to have at the least four years bachelor degrees from accredited business schools in the majors of economics, statistics, accounting or finance. Major hospitals or healthcare organizations prefer candidates who have earned their master of Health administration (Singh, 2015). The necessary and the additional skills needed by healthcare finance managers in Kenya are quite similar to what is needed for the same position in the US. For example, it is mandatory that health care finance managers have an administration in the health care sector.
Kenyan healthcare care facilities do not hire finance managers who have less than five years’ experience in health care finance administration. As a matter of fact, most healthcare organizations prefer candidates who have close to ten years’ experience in the finance sector of a health organization (Barasa et al., 2017). On the other hand, American health institutions do not put too much emphasis on the working experience needed for one to become a health care finance manager as Kenyan healthcare organizations do. In the US having the right education qualification and at the minimum four years of working experience in the same role or a similar role would secure a person the healthcare finance manager position. This can be attributed to the high number of health care organizations in the US compared to those in Kenya.

I do believe there is a healthcare crisis in the United States. I feel that it does not matter if you have insurance or not the times are changing and this is causing a lot more out of pocket expenses before insurance will make the first payment. I witness this first hand. I used to be able to go to a provider for medical issues and would not have any out of pocket expenses (deductibles) to meet before the insurance will actually pay, beside the copay. Now most insurance companies require you to meet a deductible before they pay anything. This puts more financial strain on people, not just the uninsured. It seems it is better to be uninsured and just pay out of pocket for services. People who are uninsured are more likely not to get medical services and are more likely to need hospitalization due to life threatening issues such as pneumonia or diabetes that are not controlled by medication (Krager, 2018). They are also more likely to use the emergency room for services than a provider (Krager, 2018). Children that are not insured have higher chances of developmental delays due to not seeing a doctor (Krager, 2018). Many people are uninsured because the cost of insurance premiums are very high and they still have to try and make ends meet (Krager, 2018). Although Medicare is the second largest social insurance in the United States there are still things that it does not cover for those that receive Medicare. For example someone on Medicare can only spend 20 days in a skilled nursing facility hen they will need a copay after 20 day and will have to pay out of their own pocket after 100 days (Krager, 2018). This is an issue that causes most people to get services at home and have the family pay out of pocket for a nurse or sitter or CNA. People have become anxious and fearful about getting and maintaining their health insurance due to payments (Krager, 2018). The rise of hospital care is a major health care expenditure (Krager, 2018). Premiums for employer-sponsored health insurance continues to rise as well, which is reducing paycheck that employees are bringing home, with no raise to compensate the cost of health insurance (Krager, 2018). On top of the increase for premiums the cost for provider visits can be very costly (Krager, 2018).

Economic Challenges of Healthcare Policy
The economics of the healthcare system in the United States is complex and fragmented. Costs associated with care and the prices healthcare consumers pay are determined by a wide variety of factors making it extremely difficult to find a one size fits all solution. Knickman and Kovner (2015) argue that healthcare is neither a commodity or a service, because of the high variabilities in need, cost, and consumption. The United States spends 80% of all healthcare revenue on just 20% of the population. Health insurance can be provided by the government, the private sector, or an employer. Some citizens do not have any form of healthcare coverage. Regardless of type, insurance coverage generally only pays a portion of the total healthcare cost leaving the consumer to pay the remaining balance. Moreover, reimbursement standards differ for facilities and providers. The government reimburses healthcare facilities, such as hospitals a fixed amount per patient, which creates a higher incentive to work efficiently. Healthcare providers, on the other hand, are reimbursed based on a fee for service model, meaning the more services they provide, the more money they are reimbursed (Knickman & Kovner, 2015).
Reindart (2010) maintains the passage of the Affordable Care Act (ACA) created more strain on the system by adding approximately 30 million uninsured Americans to the market. The projected cost to provide such coverage is around 8 billion to 1 trillion dollars over the next decade. However, the estimated expense of healthcare with no legislative intervention is 35 trillion dollars over the next decade (Reindart, 2010). Laureate Education (2012) contends that the human resources required to provide healthcare to an additional 30 million people is another consideration the ACA does not address. The United States is already experiencing staff shortages for key healthcare jobs like nurses and physicians. Additionally, with baby boomers retiring from the workforce and simultaneously needing more healthcare resources as they age will inevitably exacerbate the shortage of providers (Laureate Education, 2012).
Ethical Considerations
It is important to recognize that the private sector often follows the public sector when deciding what services will be covered (Knickman & Kovner, 2015). Therefore, the implications of Medicare deciding not to pay for a drug or service will likely affect the entire population. Stein (2010) asserts that cost should not be a consideration in determining if medications or services will be paid for by Medicare. Nonetheless, that is what happened with Provenge, a vaccine indicated for late stage prostate cancer patients. The drug prolonged the lifespan of patients by about four months. Provenge costs around 93,000 dollars per p.
