Critical illness insurance problems of disclosureKelly Shultis
Critical illness insurance often covers claims for cancer, which many people are diagnosed with. Insurers thoroughly check applicants' medical histories and smoking status when claims are filed to check for non-disclosure of health information when the policy was obtained. Non-disclosure, even if accidental, can allow insurers to deny a claim by arguing the policy may not have been provided had full information been disclosed originally. While insurers say this protects the system, intense scrutiny of claims creates bad press. Larger insurers have tried to address problems by allowing customers to provide new information without penalty during a temporary period.
A low cost alternative to meeting rising healthcare cost. Addresses the root causes of accelerating health care cost and solves the issue of rising healthcare cost.
Kentuckiana Medical Center (KMC), managed by the national healthcare development firm Galichia Hospital Group (GHG), is ranked as the #1 Hospital in Louisville Area for Patient Safety by the Consumer Reports organization. The top ranking was first released March 27, 2014, by Louisville’s WAVE-TV.
Get Real Health developed “myhealthlocker” built on the InstantPHRTM platform which works with Microsoft HealhVault and allows SLaM’s service users to gather, store and share their own health information.
Early elective deliveries between 37 and 39 weeks gestation exemplify the five biggest problems in the US healthcare system: (1) too much unnecessary care, (2) avoidable harm to patients, (3) billions wasted on unnecessary care, (4) perverse incentives in how care is paid for, and (5) lack of transparency. Public reporting of early elective delivery rates by hospitals has helped reduce the national rate from 17% to 11.2% since 2010 by galvanizing changes, showing the power of transparency. While progress has been made on this issue, continued pressure is needed to encourage real change and apply this model across other areas to address the systemic problems.
The document discusses how managed care organizations (MCOs) need to partner with millennials as baby boomers age into retirement. It notes that millennials will influence boomers' healthcare choices and that MCOs will need to appeal to both generations through virtual services, a multi-channel marketing approach, and member services tailored for each group. The document also examines how MCOs may need to work with providers to develop new virtualized services for boomers and tighten compliance and utilization management with the influence of millennials.

Final Human Benefits September Pet Ins MonthLoran Hickton
As pet owners consider health insurance for their pets, they might think of it in the context of their own health. Research shows that humans with a puppy, kitten, dog, or cat in their home enjoy a measurable improvement in health, and in fact spend less on their own healthcare.
Documented studies on human health suggest that pet ownership bestows the following benefits:
• Reduced risk of cardiovascular disease
• Higher survival rates from heart attacks
• Significantly lower use of general practitioner services
• Reduced risk of asthma and allergic rhinitis in children exposed to pet allergens during the first year of their life
• Better physical and psychological well-being for seniors

A dead bat found by a school nurse was brought to an elementary school and shown to students, allowing them to touch it. The bat later tested positive for rabies, exposing over 100 people. The school had to pay $70,000 for rabies shots. Bats can carry rabies but are also beneficial for pollinating plants and eating insects. If a bat is found in a home, one should assume a bite occurred and seek medical advice, as bat bites are small and not always noticeable.

The document summarizes issues with the current US healthcare system including high costs, large number of uninsured, restricted access to care, and high administrative costs. It presents single-payer healthcare as an alternative that could provide universal comprehensive coverage for all Americans through tax funding, reduce costs, improve access and choice, while maintaining physician autonomy and quality of care. Medical students would have lower debt under such a system.

Perceptions Impact
Your Market?
By Nicole Olynk Widmar and
Melissa McKendree, Purdue University
I aintaining existing mar-
kets for pork products,
I cultivating new markets
for existing products and
creating new products for new markets
are some avenues that the U.S. pork
industry has sought, and continues to
explore, for growth. When it comes to
maintaining markets, there are several
relationships that must be considered.
End consumers, whether in restaurant
or supermarket settings, are increas-
ingly interested in social issues and the
production processes employed in food
production. Livestock products (meat
and dairy products) certainly seem
to get the majority of the spotlight in
regard to consumers' concern for pro-
duction processes.
Shoppers in supermarkets and din-
ers in restaurants have increased access
to information via the Internet, and are
in constant communication with one
another via social media and alterna-
tive news sources about perceptions
of animal agriculture. Even though
most U.S. consumers are not directly
in contact with livestock, concern for
the treatment of animals, including
those employed in food production,
is evident — and increasing. While
in the past consumers were mainly
concerned with factors like the fat or
nutritional content of pork, for exam-
ple, today's savvy shoppers are con-
sidering other factors, like the welfare
of livestock (pigs), safety of workers
employed on farms and potential envi-
ronmental impacts (externalities) of
livestock operations.

production due to pressures from vari-
ous interested parties. Changes such
as the discontinued use of gestation
stalls, for example, are being sought
via traditional regulatory channels in
some states, but are also being pushed
via non-traditional market channels.
Consider the cumbersome process
of changing regulations, versus the
oftentimes faster (and perhaps easier)
channel of influencing key market
actors. It is no surprise that consum-
ers' concerns are increasingly voiced to
supermarkets and restaurants which,
in turn, take action to satisfy their
customers by placing pressure on sup-
ply-chain players. Changes sought via
"the market," rather than legislation or
regulation, are increasingly common,
and the use of market channels for
communicating throughout the supply
chain is unlikely to stop anytime soon.

The document discusses arguments against mandatory spay/neuter (MSN) laws from several animal welfare organizations. It notes that MSN laws have not reduced shelter intake or euthanasia, negatively impact low-income families, do not address responsible ownership, and increase costs. Alternative approaches are proposed like reducing intake, improving adoptions, expanding low-cost spay/neuter access, and increased community outreach and education. The document advocates that Long Beach can improve animal welfare outcomes through these alternatives instead of pursuing MSN laws.
Concierge medicine is a growing medical model in the United States where physicians charge patients an annual fee in exchange for personalized medical services like same-day appointments and in-home visits. This addresses issues in the traditional healthcare system like increasing costs and physician stress. Some key points about concierge medicine include that over 5,000 physicians have adopted this model since 2005, annual patient fees typically range from $1,500-2,000, and 64% of physicians believe it has the greatest financial success potential.
My team and I worked to provide a lucrative business plan for a network of 100 veterinarians by cutting costs, increasing marketing tactics, and implementing inventory software as well as a pet health insurance plan.
The document proposes establishing a cat rescue foundation and no-kill shelter called Scratching Post Cat Sanctuary. It aims to address the inadequate animal welfare solutions in the city. The shelter would provide a cage-free environment for cats and daily attention from staff. Goals include increasing survival rates for sheltered cats to 90% through medical care partnerships and adopting out older/younger cats. It also plans a Trap-Neuter-Release program to reduce the feral cat population and illness through spaying/neutering and vaccinations. The proposal discusses establishing financial guidelines, understanding root causes of shelter cats, and creating volunteer and employee protocols.
Managed care organizations increasingly require physicians to consider costs of treatment, creating conflicts with their duty to patients. Dr. X faces financial hardship from a new HMO reimbursement model. Dr. Solomon's practice group formed "pods" where costs dominated patient care discussions, distributing "report cards" on physicians' costs. While knowing quality and costs is important, tying pay to decisions may compromise care. Most US physicians face some financial risk for patient care costs, risking resentment of high-need patients and "bedside rationing" of care.

Breast cancer: Post menopausal endocrine therapyDr. Sumit KUMAR
Breast cancer in postmenopausal women with hormone receptor-positive (HR+) status is a common and complex condition that necessitates a multifaceted approach to management. HR+ breast cancer means that the cancer cells grow in response to hormones such as estrogen and progesterone. This subtype is prevalent among postmenopausal women and typically exhibits a more indolent course compared to other forms of breast cancer, which allows for a variety of treatment options.
Diagnosis and Staging
The diagnosis of HR+ breast cancer begins with clinical evaluation, imaging, and biopsy. Imaging modalities such as mammography, ultrasound, and MRI help in assessing the extent of the disease. Histopathological examination and immunohistochemical staining of the biopsy sample confirm the diagnosis and hormone receptor status by identifying the presence of estrogen receptors (ER) and progesterone receptors (PR) on the tumor cells.
Staging involves determining the size of the tumor (T), the involvement of regional lymph nodes (N), and the presence of distant metastasis (M). The American Joint Committee on Cancer (AJCC) staging system is commonly used. Accurate staging is critical as it guides treatment decisions.
Treatment Options
Endocrine Therapy
Endocrine therapy is the cornerstone of treatment for HR+ breast cancer in postmenopausal women. The primary goal is to reduce the levels of estrogen or block its effects on cancer cells. Commonly used agents include:
Selective Estrogen Receptor Modulators (SERMs): Tamoxifen is a SERM that binds to estrogen receptors, blocking estrogen from stimulating breast cancer cells. It is effective but may have side effects such as increased risk of endometrial cancer and thromboembolic events.
Aromatase Inhibitors (AIs): These drugs, including anastrozole, letrozole, and exemestane, lower estrogen levels by inhibiting the aromatase enzyme, which converts androgens to estrogen in peripheral tissues. AIs are generally preferred in postmenopausal women due to their efficacy and safety profile compared to tamoxifen.
Selective Estrogen Receptor Downregulators (SERDs): Fulvestrant is a SERD that degrades estrogen receptors and is used in cases where resistance to other endocrine therapies develops.
Combination Therapies
Combining endocrine therapy with other treatments enhances efficacy. Examples include:
Endocrine Therapy with CDK4/6 Inhibitors: Palbociclib, ribociclib, and abemaciclib are CDK4/6 inhibitors that, when combined with endocrine therapy, significantly improve progression-free survival in advanced HR+ breast cancer.
Endocrine Therapy with mTOR Inhibitors: Everolimus, an mTOR inhibitor, can be added to endocrine therapy for patients who have developed resistance to aromatase inhibitors.
Chemotherapy
Chemotherapy is generally reserved for patients with high-risk features, such as large tumor size, high-grade histology, or extensive lymph node involvement. Regimens often include anthracyclines and taxanes.

The biomechanics of running involves the study of the mechanical principles underlying running movements. It includes the analysis of the running gait cycle, which consists of the stance phase (foot contact to push-off) and the swing phase (foot lift-off to next contact). Key aspects include kinematics (joint angles and movements, stride length and frequency) and kinetics (forces involved in running, including ground reaction and muscle forces). Understanding these factors helps in improving running performance, optimizing technique, and preventing injuries.
Summer is a time for fun in the sun, but the heat and humidity can also wreak havoc on your skin. From itchy rashes to unwanted pigmentation, several skin conditions become more prevalent during these warmer months.
2. Historically, most pet owners
viewed the veterinarian’s
office as a one-stop shop.
Unlike human patients, who
might see one specialist for
their irregular heartbeat and
another for their itching eyes,
veterinary general
practitioners were expected
to handle the majority of pet
health concerns in-house.
Referrals were rare; visits to
specialists were even fewer.
3. Today, our engagement with animal-centered
health mirrors the more specialized model
that we use for human healthcare. Owners
now have more options to provide additional
care to extend life expectancies of their pets.
The primary care veterinarians that pet
owners used to rely on for all-purpose care
have become the central care directors in
increasingly wide-reaching networks of
specialty veterinarians. This newer model of a
triad of care between pet owner, general
practice veterinarian and veterinary specialist
has provided pet owners today with animal
care solutions that they might not have
thought possible a decade ago.
4. Consider the capabilities of SAGE Veterinary Centers — a
California-based provider network offering specialty and
emergency veterinary services in Dublin (CA), Concord,
Campbell, and Redwood City. Each of the network’s
locations provides access to specialty services including
cardiology, internal medicine, oncology, neurology,
surgery, and physical rehabilitation offerings. The
procedures these veterinarians perform are often
remarkably similar to those that humans receive. In 2018
alone, SAGE internal medicine doctors administered more
than 1,300 ultrasounds and 550 endoscopies. Their
veterinary surgeons performed more than 1,100 ACL tear
repairs on canine patients with over 150 arthroscopies
(minimally invasive surgery). SAGE’s oncologists, in
collaboration with the surgeons, had over 550 masses
removed and provided over 430 veterinary patients with
radiation therapy.
5. Pro-pet trends certainly indicate a
receptive market. Rates of pet
ownership in the United States are
high, especially among Millennials.
According to a survey published by the
research firm Mintel, 75% of Americans
in their 30s have dogs, and 51% have
cats. These pet owners aren’t skimping
on their pet-centered financial
commitments, either. The same study
also reported that pet spending
topped $11 billion in 2017. More than
half of those surveyed agreed that it
was important for their pet to be
comfortable as they age, and a full
quarter stated that they would pay
medical procedures to extend their
pet’s life “no matter what the cost.”
6. Consider the case of Ana
Homayoun, a New York
Times reporter who recently
chronicled her decision to support
her dog through hospice rather than
put him down. She was able to fund
his protracted end-of-life care
because she had invested in pet
health insurance. The policy
Homayoun chose took most of the
cost burden for the treatment off of
her shoulders, thereby empowering
her to pursue a specialized care
option that she might not have felt
comfortable investing in otherwise.
7. Insurance plans vary in design; some are accident-specific, but
the vast majority (98%) encompass both accident-caused claims
and animal wellness. They can either be bought by an individual
or obtained as an employer-sponsored benefit. Trupanion, one of
the leading pet insurers in the U.S., advertises that with some
exceptions, their members will only have to pay 10% of
unexpected veterinary bills. In doing so, they help pet owners
provide their animals with the specialized support they need to
live long and healthy lives.
However, Homayoun is still very much in the minority of pet
owners. The vast majority of Americans do not have pet insurance;
according to a report prepared by the North American Pet Health
Insurance Association (NAPHIA), only 3% of pet owners in the U.S.
have taken out policies for their pets. This number is strikingly low
compared to the 25% the UK maintains and the impressive 50%
that Sweden, the originator of pet health insurance policies,
currently has. If the U.S. increases its insurance adoption rate,
American owners could have greater financial freedom to engage
with specialist services and thus bolster the pet care sector
overall.
8. Expectations in pet care are
changing. Americans —
particularly millennial Americ
ans — want more care
solutions for their pets. The
highly specialized animal care
sector can provide the
support pet owners ask for
now — there is room in the
market to grow even further.
