Effective communication is vital to constructing an accurate and detailed patient history. A patient’s health or illness is influenced by many factors, including age, gender, ethnicity, and environmental setting. As an advanced practice nurse, you must be aware of these factors and tailor your communication techniques accordingly. Doing so will not only help you establish rapport with your patients, but it will also enable you to more effectively gather the information needed to assess your patients’ health risks.
For this Discussion, you will take on the role of a clinician who is building a health history for a particular new patient assigned by your Instructor.
To prepare:
With the information presented in Chapter 1 of Ball et al. in mind, consider the following:
· By Day 1 of this week, you will be assigned a new patient profile by your Instructor for this Discussion. Note: Please see the “Course Announcements” section of the classroom for your new patient profile assignment.
· How would your communication and interview techniques for building a health history differ with each patient?
· How might you target your questions for building a health history based on the patient’s social determinants of health?
· What risk assessment instruments would be appropriate to use with each patient, or what questions would you ask each patient to assess his or her health risks?
· Identify any potential health-related risks based upon the patient’s age, gender, ethnicity, or environmental setting that should be taken into consideration.
· Select one of the risk assessment instruments presented in Chapter 1 or Chapter 5 of the Seidel's Guide to Physical Examination text, or another tool with which you are familiar, related to your selected patient.
· Develop at least five targeted questions you would ask your selected patient to assess his or her health risks and begin building a health history.
By Day 3 of Week 1
Post a summary of the interview and a description of the communication techniques you would use with your assigned patient. Explain why you would use these techniques. Identify the risk assessment instrument you selected, and justify why it would be applicable to the selected patient. Provide at least five targeted questions you would ask the patient.

As an advanced practice nurse, it is important to tailor communication techniques and target questions when building a health history for patients. For a 14-year-old biracial male living in public housing, the nurse would use open-ended questions, active listening and age-appropriate language to establish rapport. Targeted questions would address environmental exposures, social support systems, and substance abuse risk factors. The CRAFFT screening tool would help identify substance abuse risks. Potential health risks include infectious diseases and pollution exposure due to his living situation.

The patient profile is a 76-year-old black male with disabilities living in an urban setting. The discussion summarizes how the clinician would conduct an interview and assessment for this patient. Effective communication techniques are emphasized, such as speaking slowly and clearly given potential hearing loss. A full assessment of functional status, activities of daily living, fall risk, medications, mental health, and social support systems would be conducted using tools like HEEADSSS. Targeted questions focus on living situation, assistance needs, falls history, medications, expenses, and substance use. Risk factors addressed include falls, functional impairment, polypharmacy, and potential for depression or substance abuse issues.

Final Exam
Spend up to the next 2 hours to complete the following task, to the best of your ability. This exam is worth 100 points. There is no specific word count requirement.
Topic: Vitamin C - https://vitamincfoundation.org/squares/
Your submission should be in the form of written paragraphs, but you will not be evaluated on the quality of writing beyond the minimum necessary to understand what you are communicating. Your submission does not need to be as structured as the analysis assignments submitted during the semester. You should answer parts 1-5 of the task individually, each in paragraph form. Number each answer to correspond to parts 1-5, and then references at the end.
1. In one paragraph, summarize the product or service and describe what it is supposed to do.
2. In one paragraph, identify and clearly state at least one scientific claim being made about the product or service.
3. Identify and clearly describe at least two sources that are purported to support the claim. Each source should be described in a single paragraph (i.e., two paragraphs total). You must provide references for these sources (as well as any other that you use to complete the other tasks) at the end of your document, in the same format that you have been using throughout the semester.
4. Briefly evaluate the quality of the sources and whether or not the sources appear to support the claim. This section may be anywhere from one to several paragraphs long, as needed.
◦ State whether or not you believe the claim is justified, correct, and/or relevant based on your overall evaluation. This evaluation must take into account the sources you identified and evaluated (in steps 3 and 4).
◦ You may utilize sources that do not support the claim in your overall evaluation (i.e., sources in addition to the two from step 3).
◦ Your evaluation should explicitly consider at least two logical fallacies. These may be fallacies that you believe are present, or ones that could have been an issue but that you determined were not.
5. Based on your review of the product or service, summarize in one paragraph when use of the product or service might be beneficial: who might benefit and under what circumstances.
You are free to draw on prior knowledge, your textbook, and electronic resources, but should indicate when you have done so by using the same referencing methods utilized during the semester.
You may not use other humans. Communication in any form (verbal, physical, or electronic) with another human being during this exam will be considered academic misconduct and result in an automatic grade of zero. The sole exception is if you have an urgent issue that you communicate to an instructor. Your instructors will not provide assistance with this exam or the resources you utilize to complete it.
Your final document should be uploaded to the Assignments link provided through Brightspace. After verifying that the document has successfully uploaded, you are free to lea.

The document provides background information for a capstone course project involving a situation at a home health care agency. A home health aide, Sara, contracted tuberculosis (TB) and faced isolation from coworkers upon returning from medical leave. Another aide is now displaying similar symptoms. The student has been tasked with investigating how the agency handled the situation.
The first steps outlined are: 1) research TB and ensure education materials are available for staff, 2) research disclosure requirements to ensure protocol was followed, and 3) interview Sara, coworkers, supervisors, and HR to determine if a privacy violation or disparate treatment occurred. Researching TB will help address staff fears and interviews will provide facts from all perspectives to compile an accurate
This document discusses the challenges of assessing the thorax, lungs, cardiovascular, and peripheral vascular systems due to their complex interrelated structures and functions. It notes that less experienced nurses like the author may provide inaccurate information when evaluating these systems. Age differences can also impact findings interpretation. Additionally, pediatric patients cannot always supply clarifying information during exams. The author advocates collaborating with experienced nurses to accurately interpret exam findings like breath sounds. Patient factors like occupation may further aid interpretation.
This document discusses building an effective therapeutic relationship with an adolescent Hispanic/Latino patient living in a middle-class suburb. Communication challenges may arise due to potential language barriers and low health literacy. The patient's age also presents an obstacle, as teenagers are often reluctant to speak and exploring risky behaviors. It is important to maintain confidentiality and clarify boundaries. Assessment tools like HEEADSS can help discover social determinants of health risks and promote prevention.
1) The document discusses analyzing health-related risks by considering social determinants of health like age, gender, ethnicity, and environment when building a patient's health history.
2) It focuses on the case of an adolescent Hispanic/Latino boy living in a middle-class suburb and how communication techniques should be tailored to effectively interview this patient.
3) Key factors like the patient's age, ethnicity, and environment are examined to understand potential health risks and inform targeted questions using the HEEADSS risk assessment tool.
This document provides guidance on effective patient interviewing skills for physicians. It discusses the importance of professionalism, ethics, using a biopsychosocial model, and patient-centered care. The four core ethical principles are autonomy, beneficence, non-maleficence, and justice. Effective communication involves actively listening, establishing rapport, asking open-ended questions to understand the patient's perspective, and using closed-ended questions to obtain specific details. The goal is to collaborate with patients to understand their health issues and concerns in a holistic manner.

ewly appointed Police Chief Alexandra Delatorre of the Anytown Police Department (APD) has been attending several town and city council meetings whereby she has heard numerous complaints about the increase in armed robberies being committed by the local transients (homeless people) in and near the Anytown Gallery Shopping Mall. Chief Delatorre checked with the crime analysis unit of the department to obtain information on the crime statistics for robberies in the area of the shopping mall. The crime analysis unit determined that there was 75% increase in the number of robberies between 2009 and 2011. The chief attended a meeting with the shopping mall executives and learned that there had been a steady increase in the number of businesses leaving the mall, which served as major sources for employment and revenue for the city.
One week ago, Chief Delatorre drove to the shopping mall one evening at 9:30 p.m., before the mall was about to close, and discovered that several sections of the subterranean multilevel parking lot had poor lighting and no security gate that secured the parking area, and the guard shack was in an obscure portion of the first level and appeared to not have been used for quite some time. She also found what appeared to be small “camps” of blankets, trash, old and dirty clothes, and metal shopping carts that belonged to different local grocery stores in the far corners of the parking lot on the lower two levels. There was also a very strong odor of urine in the area of the “camps.”
Chief Delatorre has convened you and your team to make recommendations on addressing the problem of the robberies at the shopping mall. She wants to implement a program using the community-oriented policing (COP) philosophy. She has several specific areas that need to be considered for the COP program to reduce or even eliminate the robberies at the shopping mall. She would like to give the program a name. You are to compose a written memo that addresses the following::

Performing Effective Project Monitoring and Risk Management
Imagine that you are employed as an IT project manager by a prestigious coffeemaker organization. This organization operates many coffee shops within the region and would like to promote its brand by creating a mobile application that will provide its customers with the ability to view the nearest coffee shop location within their geographical area.
As a member of the software development team, you estimate a total project cost of $150,000. You have designated control points to measure project progress. At control point 2, the following data is available:
Budget Cost of Work Performed
$ 24,000
Actual Cost of Work Performed
$ 27,500
There are various stakeholders that are interested in the progress of the project. These stakeholders include the marketing management team (internal customers), software designers, programmers, testers, and upper management. The software development team has attempted to release a mobile application of this magnitude in the past; however, lack of sponsorship, mobile development expertise, and technical infrastructure has limited the team’s success.
Write a four to six (4-6) page paper in which you:
Identify at least four (4) attributes of the mobile application development project that can be measured and controlled and evaluate how each is a critical factor for the success of the project.
Generate a project plan summary of the various project milestones. Develop a WBS that details work packages required to complete project scope.
Develop a workflow model that can be used to inspect and detect defects during the acceptance of this mobile product through the use of graphical tools in Microsoft Word or Visio, or an open source alternative such as Dia. Note: The graphically depicted solution is not included in the required page length.
Describe how the defects detected during the acceptance of the mobile application should be reported and explain the circumstances in which a defect may not require reporting.
Analyze the communication needs of the different project stakeholders. Explain the types of project status reports that would be useful to each.
Compute the cost variance, schedule variance, cost performance index, schedule performance index, and estimated actual cost using the information presented at control point 2. Interpret the project schedule and budget status from the calculations.
Explain how work package, binary tracking, and earned valued reporting can be used effectively during the maintenance phase of the software life cycle if various change requests may be assigned to individuals and processed on an individual basis.
Develop a risk register that will document all of the estimated risks. Assign one (1) risk management technique for each risk and explain the basis for your selection.
Use at least three (3) quality resources in this assignment. Note: Wikipedia and similar Websites do not qualify as quality resources.
Your ass.

This week's audio is very informative but one point stuck out to me the most. I found it interesting that a big issue is the struggle to have communication between agencies, municipalities, and federally. It brought up that an example of this is communication through radio. Each individual has completely different equipment which makes communication with anyone outside of the agency via radio impossible. You would think with today's technology they would find away to maintain easy communication between all emergency related organizations. This would ensure that they all are on the same page and can have effective cooperation during big emergencies such as 9/11. What are the methods of communication that currently exist and how do they help different emergency related agencies stay connected?
The first Amendment is a so important to our way of life so when it is abused and people die should the media be held responsible? There are attempts to hold gunmakers liable for gun deaths so is it the same with media when they provide inaccurate information that may then cause violent protest?
Given the perpetual imbalance between funding and needs in emergency services and criminal justice, what are possible sources of funding for a emergency management training, equipment, and sustainability?
The Department of Homeland Security and FEMA as well as various state and federal grants can be requested to assist in funding the various state and local agencies in their Emergency Management related maintenance and training. The equipment and personnel are expensive to have and to keep current in training so any help with grants and donations is always appreciated. Could local fund drives help?
Do you support a national standard for communication equipment for emergency management? If not, why not? How would agencies pay for interoperability communications?

This means that in any conflicts posed by applying different values to cases, there are no general rules for deciding which values are more important. We have to take each case on its merits.
They might be right about this. It's a commonly held perspective on moral values.
In the Lecture I discussed the relation between Justice and Compassion.
We sometimes encounter conflicts between these two values, and I think the story of Robin Hood is the classic tale of such conflict. Robin Hood acted compassionately in robbing from the rich to give to the poor.
Now I'm assuming that the rich deserved their wealth, and did not acquire it unjustly. That is, I am supposing that Robin Hood was not moved by his sense of justice, on this count (but see below). This is a controversial assumption, for there are those who would argue that the rich (or at least, the inordinately rich) could not have become that rich without transgressing some moral principle. But I will make this assumption in order to simplify the discussion. So let us assume that the rich people Robin Hood took from had not made their fortune unjustly. But even this does not mean that Robin Hood was wrong to take from them. It could be, from the perspective of some sort of duty theory, that the rich had a duty to take care of the poor, and they had failed in this moral duty. So Robin Hood was correcting their immoral behavior (not in obtaining the wealth, but in not dispensing it as they ought to). He was moved by the suffering of the poor, and the immorality of the rich in not responding to this suffering, which makes him a compassionate and moral hero. (By the way, I am not committed to this view--I am mentioning it to show you that there is much more to be discussed here than might meet the eye of our 21st. century, prior to philosophical reflection.
The questions, therefore, are:
Is Robin Hood acting justly, or unjustly but because he is moved by compassion?
Could a compassionate Robin Hood ever be
right
to violate the rules of justice in order to alleviate suffering?
With regard to the second of these, I assume that the standard answer in our 21st. Century culture would be: No; justice should always win in conflicts with compassion. So there is at least one absolute priority relation between two of the basic moral values in the set.

Scenario
You are a probation officer and have a client, Paul Rosen, who was recently released from prison and is a registered sex offender. One of the obligations of a probationer is to keep in close contact with his probation officer. This particular probationer failed to maintain contact with you for a period of 4 months. During your effort to find Paul Rosen, you find out that Paul was involved with a woman who has three children, ages 6, 9, and 14. You feel compelled to contact this woman and find out whether she knew that Paul was a registered sex offender who is out of prison on probation. During the conversation, she was appreciative that you had concerns but explained that Paul told her that he was involved with a 16-year-old, but that the girl had lied to him about her age. She said his conviction was the fault of the girl's parents, who convinced the daughter to press charges as a way to keep him away from her. She said it was not a big deal because the girl lied to Paul about her age. During your conversation with the woman, you feel compelled to tell her about John's past of molesting two girls, ages 4 and 7.
Analyze this scenario, and discuss the following:
Describe how you would handle this situation using normative ethics?
In applied ethics, you do not deal with the facts of individual cases. You focus on applying ethical rules to a class of cases. Describe the class or type of case represented here.
After determining the type of case, describe the professional code of ethics that may apply to this type of case?
Based on the rule you would apply to this type of case, what should you do as a probation officer in this type of case?

$ 24,000
Actual Cost of Work Performed
$ 27,500

5-6 paragraphs
Interagency
is relatively recent as a term, yet as a concept, it has been in practice to varying degrees for as long as nations have had governments, fought wars, engaged in international commerce, or extended diplomacy. What is arguably a recent expansion of the interagency concept is the level of inclusiveness—that is, the broader scope of who is invited to participate in interagency activities. Also recent is the compulsion to use it, be it by leaders, for policies, or just from a plain "need" to employ an interagency approach to decision making and problem solving.
Assignment Guidelines
In 5–6 paragraphs, address the following:
What is your current understanding of the fundamentals of interagency relationships? Explain.
Consider horizontal relationships between federal agencies as well as vertical relationships between federal, state, and local agencies in your response.
What benefits do you think can be realized through effective interagency relationships? Explain.
What consequences might occur because of ineffective interagency relationships? Explain.
What 1 organization do you think should have the most responsibility regarding the formation and evaluation of interagency relations? Why?

LAND USE LAND COVER AND NDVI OF MIRZAPUR DISTRICT, UPRAHUL
This Dissertation explores the particular circumstances of Mirzapur, a region located in the
core of India. Mirzapur, with its varied terrains and abundant biodiversity, offers an optimal
environment for investigating the changes in vegetation cover dynamics. Our study utilizes
advanced technologies such as GIS (Geographic Information Systems) and Remote sensing to
analyze the transformations that have taken place over the course of a decade.
The complex relationship between human activities and the environment has been the focus
of extensive research and worry. As the global community grapples with swift urbanization,
population expansion, and economic progress, the effects on natural ecosystems are becoming
more evident. A crucial element of this impact is the alteration of vegetation cover, which plays a
significant role in maintaining the ecological equilibrium of our planet.Land serves as the foundation for all human activities and provides the necessary materials for
these activities. As the most crucial natural resource, its utilization by humans results in different
'Land uses,' which are determined by both human activities and the physical characteristics of the
land.
The utilization of land is impacted by human needs and environmental factors. In countries
like India, rapid population growth and the emphasis on extensive resource exploitation can lead
to significant land degradation, adversely affecting the region's land cover.
Therefore, human intervention has significantly influenced land use patterns over many
centuries, evolving its structure over time and space. In the present era, these changes have
accelerated due to factors such as agriculture and urbanization. Information regarding land use and
cover is essential for various planning and management tasks related to the Earth's surface,
providing crucial environmental data for scientific, resource management, policy purposes, and
diverse human activities.
Accurate understanding of land use and cover is imperative for the development planning
of any area. Consequently, a wide range of professionals, including earth system scientists, land
and water managers, and urban planners, are interested in obtaining data on land use and cover
changes, conversion trends, and other related patterns. The spatial dimensions of land use and
cover support policymakers and scientists in making well-informed decisions, as alterations in
these patterns indicate shifts in economic and social conditions. Monitoring such changes with the
help of Advanced technologies like Remote Sensing and Geographic Information Systems is
crucial for coordinated efforts across different administrative levels. Advanced technologies like
Remote Sensing and Geographic Information Systems

Initial post, 76 year old African American patient
Top of Form
11. Interview and Communication Techniques
Obtaining a medical history from a patient involves gathering
subjective and objective data to provide proper medical care and
address potential health issues. The patient assigned for this
discussion is a 76-year-old black African American male with
disabilities living in urban area. The first step for this patient is
to identify the type of disability and how it affects the
interviewing process. If it is physical disability, it is crucial to
utilize accessible room with proper lighting that is free of
obstacles such as furniture to ensure safety and efficiency.
Some of the interview techniques that can be utilized for this
patient includes sitting in front of the patient with proper
lighting while conducting the interview and minimizing
interferences by avoiding interruptions while the patient is
talking. According to Falkenberg et al. (2019), proper lighting
is a significant environmental attribute that promotes physical
and mental health of the aging society. The other techniques
that can be applied for this patient includes using short
sentences and open-ended questions, making eye contact, and
avoiding medical terms to prevent confusion. Additionally,
allowing more time for the patient to answer questions and
providing assurance with empathy will promote good rapport.
If the patient exhibits confusion or becomes forgetful, gathering
more data from family members will be beneficial to fill the
gaps and prevent discrepancies. According to Walji et al.
(2021), clear and respectful communication among healthcare
provider will promote trust and medication compliance by
patients. Giving more time for the patient to respond is essential
due to a decline in sensory and cognitive functions.
Risk Assessment
The risk assessment that would benefit this patient includes
functional assessment such as mobility, activities of daily
living, access to transportations and economic status. In
addition, it is crucial to assess for thoughts of suicide, alco hol
dependence and medication compliance. According to
Seematter-Bagnoud and Bulla (2018), factors associated with
12. the occurrence of disability include functional, cognitive,
affective, and social problems along with lifestyle habit and
sensory impairments. The risk assessment will help identify
potential issues that prevent the overall wellbeing of the patient.
In addition, it can be used as a tool to detect warning signs of
decline in patient’s health or behaviors that can harm the
patient.
Five questions asked for the patient includes:
1. How do you get around in your apartment?
Rationale: To identify physical functionality and to assess for
potential risk of fall and injury. This will also help identify
activities the patient can do without assistance and the one’s
that needs assistance.
2. Does your disability prevent you from receiving
healthcare?
Rationale: This question will assess if patient has adequate
access to transportation to receive healthcare on a timely
manner if not arrangement can be made to facilitate telehealth
or to provide resources available for disabled adults. According
to Ahlqvist et al. (2015), healthcare professionals need to
evaluate older patient’s situations to support functional
capacity, resources, personal attributes and living circumstances
and environment
3. Do you consume alcohol?
Rationale: This question is used to assess for alcohol
dependence and the reasoning since aging increases the effect of
alcohol used alone or combined with medications due to change
in metabolism among the aging society.
4. Have you thought about harming yourself?
Rationale: elderly patients with disability are highly susceptible
to suicide therefore it is very important to assess and identify
behaviors that can lead to suicidal actions. Physical disability
preventing a disabled person from performing activities of daily
leaving with medical care promotes risk factors for suicidal
attempts among elderly patients (Zhang et al., 2016).
5. Can you financially afford to refill your medications
13. monthly?
Rationale: This question will assess medication compliance as
well as address any financial constraints that prevent patient
from refilling his prescriptions because Medication
noncompliance increases among geriatric patient due to
financial strain (Osborn et al., 2017).

Initial post, 76 year old African American patient
Interview and Communication Techniques
Obtaining a medical history from a patient involves gathering
subjective and objective data to provide proper medical care
and address potential health issues. The patient assigned for this
discussion is a 76-year-old black African American male with
disabilities living in urban area. The first step for this patient is
to identify the type of disability and how it affects the
interviewing
process. If it is physical disability, it is crucial to utilize
accessible room with proper lighting that is free of obstacles
such as furniture
to ensure safety and efficiency.
Some of the interview techniques that can be utilized for this
patient includes sitting in front of the patient with proper
lighting while conducting the interview and minimizing
interferences by avoiding interruptions while the patient is
talking. According
to Falkenberg et al. (2019), proper lighting is a significant
environmental attribute that promotes physical and mental
health of the
aging society. The other techniques that can be applied for this
patient includes using short sentences and open-ended
questions,
making eye contact, and avoiding medical terms to prevent
confusion. Additionally, allowing more time for the patient to
answer
questions and providing assurance with empathy will promote
39. good rapport.
If the patient exhibits confusion or becomes forgetful, gathering
more data from family members will be beneficial to fill the
gaps and prevent discrepancies. According to Walji et al.
(2021), clear and respectful communication among healthcare
provider will
promote trust and medication compliance by patients. Giving
more time for the patient to respond is essential due to a decline
in
sensory and cognitive functions.
Risk Assessment
The risk assessment that would benefit this patient includes
functional assessment such as mobility, activities of daily
living,
access to transportations and economic status. In addition, it is
crucial to assess for thoughts of suicide, alcohol dependence
and
medication compliance. According to Seematter-Bagnoud and
Bulla (2018), factors associated with the occurrence of
disability
include functional, cognitive, affective, and social problems
along with lifestyle habit and sensory impairments. The risk
assessment
will help identify potential issues that prevent the overall
wellbeing of the patient. In addition, it can be used as a tool to
detect
warning signs of decline in patient’s health or behaviors that
can harm the patient.

Week 1 Discussion Post: Building a Health History
Summary of interview & description of communication
techniques
My patient is a 16-year-old white pregnant female living in an
inner- city neighborhood. In my interview with this patient, I
gathered information about this patient’s medical history, living
conditions, social history, and information about the pregnancy.
Furthermore, in the interview I sought to discover potential
healthcare barriers this patient may face. In addition, I asked
the patient questions about intimate partner violence. The
communication techniques that I would use for this patient
include asking open ended questions and active listening. Open
ended questions versus yes or no questions allow patients to
express their thoughts and feelings (Slade & Sergent, 2021). An
example of an opened question would be, “What are your
concerns regarding your pregnancy?”. Furthermore, active
listening involves paying close attention to what the other
person is saying, asking clarifying questions and rephrasing
what the person says to ensure understanding (Doyle, 2017). An
example of rephrasing a possible statement of concern of my
patient could be “So you feel as though you will not have
enough resources to take care of yourself and your baby, is that
right?
Explanation of communication technique use
I would use open ended questions during my interview with this
41. patient because it would allow this patient to fully express her
concerns to me. Yes or no questions do not allow the patients to
go in depth with their concerns. Furthermore, open ended
questions will give this patient the impression that I care about
what she has to say. In addition, I would practice active
listening with my assigned patient because active listening will
allow me to be nonjudgmental and to understand what my
patients needs are. I can effectively assist my patient when I
understand what my patient’s needs are.
Risk Assessment Instrument Selected
The risk assessment instrument I selected is the HITS (Hurt-
Insult-Threaten-Scream) screening tool for domestic violence.
The HITS screening tool assesses for physical abuse, verbal
abuse, and threats with intimate partners (Ball et al.,2019,
p.10). The HITS screening tool would be justifiable to use in
my assigned patient because adolescent women are at a higher
risk for intimate partner violence. Nearly 70% of adolescent
female victims experience their first intimate partner violence
before age 25 (Thomas et al., 2019). Pregnant adolescents have
significantly higher rates of intimate partner violence than older
mothers and their non pregnant peers (Thomas et al., 2019).
Five Targeted Questions
The five target questions I would ask this patient comes from
the HITS screening tool.
For example,
1.) In the past year how often did your partner physically hurt
you?
2.) In the past year how often did your partner insult or talk
down to you?
3.) In the past year how often did your partner threaten you
with harm?
4.) In the past year how often did your partner scream or
curse at you?
5.) In the past year how often did your partner force you to do
sexual acts that you are not comfortable with?
