Who we are and what we do with regards to our laboratory services for Lingual Orthodontics. From simple 2D bonding, to 3D B.E.S.T. system using standard charts for Tip and Torque, or go for the fully customized IN-Tendo techniques which needs us to make diagnostic study models, then using the TTS and BPI to bond the brackets to the mal-occlusion models.

Scan to Model Project
Elly Ball conducted a project to scan the interior of two buildings - a library and a small theater - using a Trimble TX5 3D laser scanner. She then used Trimble Realworks software to register the scan data and extract a 3D model. Ball worked to import the model into SketchUp using a plugin in order to create an accurate virtual representation of the spaces. While the process worked well for the simple theater space, modeling the more complex library proved more difficult due to limitations in transferring precise data between the software programs. Overall, Ball believes 3D scanning has great potential for interior design documentation and visualization but the workflow needs further refinement.

The future goal is to link this instrument to a Digital software to align both the manual precision with 3D user technology.

Our first course in Thailand with Dr. Henrique Valdetaro. Our aim to remove the mysteries and uncertainty about Lingual Orthodontics, by showing both a structured technique for both clinical and laboratory work. With 'The Torque and Angulation lab' we demonstrated how Tip and Torque are controlled through lingual indirect bonding and what the clinical results can be if proper clinical procedures are followed. From impression to finish Dr. Henrique and Peter Sheffield showed the step by step techniques used.

A summary / description about what the "IN-tendo" system of indirect bonding is and what clients are likely to expect. It is related to both Lingual and Labial Indirect bonding at "The Torque and Angulation lab", Chiang Mai, Thailand.
How we use the Tip and Torque surveyor to derive values for Tip ( Angulation) and Torque ( Inclination ) in Orthodontic Indirect Bonding to the master models.
Actually 'Torque' is a torsional force (as described in most engineering and physics books) between the wire and the bracket slot. When the slot has been programmed with a certain value for a prescribed inclination required in relation to the flat wire plane, then the flat wire will be distorted in the slot causing a torsional force to move the tooth to the desired position. However this does depend on many factors including slot V's wire size causing 'slop' or the 'Torque Trap' and of cause the bio-mechanics.
Short lecture 20 mins "Lets talk about torque in Lingual Orthodontics"
A short lecture I gave at the ESLO conference in Athens last year. This study was also used for the poster that got 3rd place and described how just changing torque in the Upper Central Incisor will change the position on the lingual surface of the tooth, both in regards to gingival proximity and the fit of the base with an average lingual bracket slot of 55 degrees to base. I have converted it to an online PDF format here so it is easier to see.

12th ESLO POSTER Presentation about Height V's Torque in Lingual orthodontics
This study was done at 'The Torque and Angulation Lab' by Peter Sheffield actually a couple of years before the ESLO 2016 but never used or published.
It highlights one of the common problems in Lingual Orthodontics, which is bracket placement with regards to 'Torque'. As positive crown Torque increases, even with the same vertical height setting to the wire plane, then the brackets will be bonded more and more gingival. This has other effects as the distance from bracket slot to the vestibular face of the tooth in question increases on anterior teeth. Thus understanding this phenomenon and just how much 'torque loss' we can expect due to wire sizes, then we should be able to adjust the height setting to match our results if we really desire a 'flat wire'. Also by understanding the "Thickness'" measurements, we can compensate with composite pads to reduce 1st Order bends in the anterior arch wire.

An evolution of the Lab techniques we use in our 'Indirect Bonding for Orthodontics' via accurate bracket placement using standard Orthodontic brackets for Labial and the Lingual technique. How we customize cases by measuring malocclusion models, doing diagnostic set-ups and Kesling models, then measuring the difference in Tip and Torque to get individualized prescriptions for bonding. A technique started in 2010 by Peter Sheffield in Chiang Mai, Thailand.

Lingual Orthodontics lab techniques and new instruments used for Indirect Bonding of Lingual Brackets, following the same theory that the TARG from Ormco was based on. However these instruments were revolutionary in that they were the first 'Digital' ones and so easy to use! It is through their built in precision and the "Ease of Use" factor that enables us to not only work easier as Dental Technicians, Orthodontists, Assistants and whoever else wants to be involved, but also do more to help the clinic with diagnostics. The T.A.D. or "Torque Angulation Device" evolved to not only follow the TARG and B.E.S.T techniques of Lingual, but was also designed to measure Orthodontic models for Tip and Torque values.
Labial Indirect Bonding...an accurate method of controling Torque.
Article on Labial Indirect Bonding techniques for Orthodontics using the instruments TAD and BPD. This technique was used to accurately program torque and doesn't go into the realms of what torque should be used...this would be an entirely new article!
It highlights how modern Indirect Bonding for Orthodontics can be done via precise instruments that control the TORQUE. This is something that even today CAD CAM techniques have problems with. They can position the brackets on the teeth, but as we know in Orthodontics, torque is a factor that can change many other things, one of the obvious ones being the leveling of the teeth, When Torque is leveled out between slightly differing morphologies of teeth, then height will be more predictable.
Crown Angulation and Inclination with good occlusion
This was my first contribution to an Orthodontic article. With Dr. Dhirawat of the Chiang Mai Orthodontic faculty, we measured many models of Northern Thai's with good occlusion and facial profile with our NEW instrument the T.A.D. or Torque Angulation Device. This instrument was the first digital instrument able to measure Tip and Torque accurately and easily from Orthodontic study models. As all previous studies were probably done on a particular population and geographical location, then we wanted to compare the values derived with previous studies such as Andrews, Hilgers, Bennet, Roth, MBT, etc.. having a new instrument also meant that the readings were more in tune with the related tooth morphology Conclusions, should bracket prescriptions differ for different races, facial types, etc?.
Linguals Orthodontics, Lab technique with Instruments Pt 2
This document provides a step-by-step guide for performing lingual indirect bonding using the Torque Angulation Device (TAD) and Bracket Placement Device (BPD). The summary describes:
1) Key steps in the process include surveying the model with the BPD to record height and thickness, setting the torque and angulation of each bracket with the TAD, and placing brackets on the model with the BPD according to the survey.
2) After bonding the brackets, a resin separator is applied and silicone is used to make a transfer tray containing the placed brackets.
3) The tray is then used to indirectly bond the brackets to the patient's teeth.

Overall life span (LS) was 1671.7±1721.6 days and cumulative 5YS reached 62.4%, 10 years – 50.4%, 20 years – 44.6%. 94 LCP lived more than 5 years without cancer (LS=2958.6±1723.6 days), 22 – more than 10 years (LS=5571±1841.8 days). 67 LCP died because of LC (LS=471.9±344 days). AT significantly improved 5YS (68% vs. 53.7%) (P=0.028 by log-rank test). Cox modeling displayed that 5YS of LCP significantly depended on: N0-N12, T3-4, blood cell circuit, cell ratio factors (ratio between cancer cells-CC and blood cells subpopulations), LC cell dynamics, recalcification time, heparin tolerance, prothrombin index, protein, AT, procedure type (P=0.000-0.031). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and N0-12 (rank=1), thrombocytes/CC (rank=2), segmented neutrophils/CC (3), eosinophils/CC (4), erythrocytes/CC (5), healthy cells/CC (6), lymphocytes/CC (7), stick neutrophils/CC (8), leucocytes/CC (9), monocytes/CC (10). Correct prediction of 5YS was 100% by neural networks computing (error=0.000; area under ROC curve=1.0).
