The Five stages of Change By Thursday, June 25, 2015, respond to the discussion. Discussion Question Anthony is a 27 year old heterosexual Caucasian male. He was arrested 2 weeks ago for his second DWI and is facing a license suspension. He works as a delivery driver for a local store and after disclosing the arrest to his employer, as well as the consequences including loss of his license, he was terminated. Anthony lives with his girlfriend of 3 years and their 2 year old son. Anthony’s drinking behavior has increased to consumption of a case of beer on Saturday and Sunday evenings each week. He consumes several beers after work during the week “to maintain.” He has also been using methamphetamines, specifically “crystal meth” several times weekly. Anthony’s girlfriend ended their relationship as a result of his increasing substance use and ongoing difficulties. Anthony feels depressed and anxious about his current life situation, especially now that he realizes that he has no job and may be homeless because of his substance use. He is also feeling down about the loss of his relationship. He researched a few outpatient treatment programs to help him stop using both alcohol and methamphetamines, but is ambivalent about entering treatment. Anthony has considered the need to stop using substances to improve his life and relationships with significant others, though fears that he will lose his friends and miss partying with them if he stops. He also fears what life will be like without the comfort of getting high. Consider and discuss the 5 stages of change. Based upon the information provided discuss what stage Anthony is in, and provide a rationale for your decision. Young-Hwa, a 40-year-old Korean male, had immigrated to the United States 15 years ago without proper documentation. He had a hard life because, despite his training as a chef in Korea, he had difficulty finding a well-paying job without proper documentation. He also had a hard time getting along with others for long periods of time in some of the kitchens that he worked for.
He would do really well for a while talking about how much he enjoyed the job thinking that this was going to be his lucky break, but after several months he would either quit or get fired.

After many years of various positions including kitchen assistant and assistant cook, he finally landed his dream job of a top chef in a Korean restaurant.

John is a 38-year-old male of Euro-American heritage. He has never been married, and
he has one brother whom he rarely sees. John is a long-distance truck driver, and he
spends the vast majority of his time living out of his vehicle driving across the country.
He has a place he calls “home,” but he spends only about a week there every few
months. As a result, John has few close friends and no significant romantic relationship
in his life. John has also alienated several family members and close friends from whom
he has borrowed substantial amounts of money that he has not paid back.
John was referred by his employer’s EAP following an issue while at work. John had
lied about his time log and location, as he had stopped to gamble at a casino in
Mississippi during a cross-country run. He was put on probation for this infraction, and
ordered to seek counseling services. Though John denies having a “gambling problem,”
he admits that he does enjoy gambling and engages in both in-person and online
gaming whenever he has an opportunity. John also admits that he has lost a lot of
money to gambling, though rationalizes this with “at least I was having fun.” He also
notes that his gambling has cost him past relationships and prevented him from
reaching financial goals such as home ownership and retirement planning. John shares
that “it would be different if I was married or something. It’s just me, so who cares if this
is how I spend my money?”
Donna
Donna is a 40-year-old transgender female of Korean American heritage. She is the
manager of a popular local restaurant and club, and is well respected at her job. She
has come to counseling at the insistence of her boyfriend, who has expressed worry
over her increasing drug use. Donna also admits that she and her boyfriend have been
fighting a lot lately, and she is worried he might be cheating on her. In fact, she admits
that a recent altercation became physical, although she blames it on her “temporary
meth rage.”
Donna shares very openly about her use of methamphetamine, although she insists that
she has it “under control.” She states that in her 20s, she used a lot of recreational
drugs, but for a number of years after remained completely abstinent. She claims she
currently uses meth “just every now and then” because she likes the rush and energy it
gives her. She minimizes its role in the altercation with her boyfriend, stating that she
had used a little too much that day but “that never happens.” While Donna tends to
minimize the role and significance of her drug use, she states, “Maybe I could use a
little help getting my temper under control.”

Olivia Pepper is a 30-year-old African American woman who is married with three children (ages 2, 6, and 8). Olivia is a Baptist and is receiving several financial need scholarships which allows her to attend school without working. She has two residential treatments, once as an adolescent for alcohol use and the second when she was 24 for depression and alcohol use. She has had two DUI's, one at 16 and the second at 22. Olivia has previously taken Zoloft and Wellbutrin, and has gone to family counseling as a teenager. Olivia is currently pursuing her Master’s degree and has a 3.8 overall GPA. She has been referred to the University Counseling Center from her professor for erratic behaviors, which include failing two classes and having a sporadic attendance, being late, and smelling of alcohol on more than one occasion. Olivia's peers have reported that she wants to meet at the local bar and is never engaged in learning.
Olivia grew up with loving parents who worked hard. Her mother left when she was 14 and that is when she started to drink. She has been in a “funk” on and off her whole life and reports that most days she just doesn’t want to get out of bed and face the day. She is overwhelmed and sleeps 10-12 hours a day, isolates herself from her family, and avoids going to school. She has attempted suicide once with a drug overdose on Wellbutrin with alcohol. Olivia is currently attending AA meetings regularly and reports that church and AA help her get out of bed in the morning. She has started to drink again and sneaks drinks into her lemonade so that her family does not know. Her husband is angry and wants her to quit school, and Olivia is concerned for her 8-year-old daughter who is exhibiting similar behaviors as her.

Mr. A. Soprano is a 55-year-old white male who presented as an “emergency” to a psychologist at a local ER. After establishing rules of confidentiality he revealed he was a high level crime boss; essentially a mobster. Throughout the interview he spoke in a rather pronounced Brooklyn accent. He preferred to call himself “a highly creative business man.” The client’s chief complaint was a concern over a vague desire to “hurt” himself, sleep problems and was experiencing disturbing “thoughts that don’t make any sense.” He stated his thinking had become very negative, “and weighs heavy on me”, where he doesn’t see anything good or positive in his life, “and I’m normally a positive kinda guy.” Dr. Psychologist proceeded to request the client cooperate with psychological testing to help determine potential causes for the client’s reported problems. This included a clinical interview.
Mr. Soprano began his interview by telling a story about ordering a fellow mobster killed, “a couple months ago.” He said, “I liked the guy. He was my right hand man, but he screwed up Doc and coulda’ got me whacked so ahh, ya know, he had to go but I felt really bad about it. Hey ‘fergit’ about it, it was a business decision. Sometimes ya make those tough one’s but this one really bothered me ya know.” He stated he noticed in the days following, his thoughts would race and be dominated by feelings of guilt. In the last seven weeks, he went through periods of sleeping much more than normal or suffering from insomnia when he felt “stressed out and I don’t get stressed out, Doc. DAVID COOK: I'm David Cook, and I've been in the mental health field for over
25 years. And many of these years have been spent as a substance abuse
counselor and working with people with behavioral and chemical addictions.
People can become addicted to chemicals as well as behaviors. Any chemical
that has the ability to alter the mood or any behavior that has the ability to alter
the mood or is sensationalistic is potentially addictive.
Although chemical and behavioral addiction share many of the same signs and
symptoms, there are also distinct differences between the two. Two similarities
between chemical and behavioral addictions are that they both involve the
compulsive repetition, in spite of negative consequences. And they both
progressively worsen unless treated.
An important difference between the two, and a distinction that should always be
realized, is that behavioral addictions are often hidden addictions. There are no
urinalysises or drug screenings for compulsive gambling. Or you can't smell
pornography on someone's breath. So consequently, because behavioral
addictions are often hidden, the signs and symptoms continue along their
destructive pathways undetected much longer than chemical addictions.
When working with people with chemical and behavioral addictions, the
addictions counselor will find that addicts have many barriers along their road to
recovery.
NICOLE: My name is Nicole. I'm 38 years old, and May 30 I will have five years
in recovery.

GRETCHEN: My name is Gretchen. I'm 44, and I've been in recovery for about

Occupational fraud has become--at least so far--the crime of the 21st century. It is a widespread phenomenon that affects practically every organization. The frauds in the 2004 Report to the Nation on Occupational Fraud and Abuse, from the Association of Certified Fraud Examiners, caused over $761 million in total losses, with a disproportionate percentage committed against small businesses--almost half of the frauds in the study took place in businesses with fewer than 100 employees. Not surprisingly such businesses are less likely to be audited or employ antifraud measures than the larger ones.
Several broad conclusions can be drawn from the 2004 report. First, though the losses have been stable over the years, the fact that in one year alone they are approaching $660 billion is cause for concern. Dishonest executives and employees are plying essentially the same schemes with the same results. Second, although large financial statement frauds receive the most attention, they are relatively uncommon compared to asset misappropriations and corruption. Third, small businesses remain the most vulnerable to occupational fraud because of three factors: They are the least likely to have an audit, a hotline or adequate internal controls. Fourth, audits--both internal and external--although excellent prevention devices are not the most effective means of detecting frauds. Fifth, hotlines and other reporting mechanisms are a vital part of any organization's prevention efforts but should go beyond employees to vendors and customers, too. Finally, occupational fraud cannot be eliminated but organizations that use both hotlines and auditors can greatly reduce these costly crimes.
Occupational fraud schemes can be as simple as pilferage of company supplies or as complex as sophisticated financial statement frauds. This article summarizes some of the key findings of certified fraud examiners (CFEs) in cases they investigated. Internal and external auditors and CPAs advising small business clients will learn of the most effective antifraud measures.

He spent most of his time alone in his room and sometimes complained to his parents that he heard voices in the curtains and in the closet. In his sophomore year, he dropped out of school entirely. With increasing anxiety and agitation, he began to worry that the Nazis were plotting to kill his family and kidnap him.
Case Study 4:
Larry, a 37-year-old gay man, has lived for three years with his partner, whom he met in graduate school. Larry works as a psychologist in a large hospital. Although competent in his work, he often feels strained by the pressures of his demanding position. An added source of tension on the job is his not being out with his co-workers, and, thus, he is not able to confide in anyone or talk about his private life. Most of his leisure activities are with good friends who are also part of the local gay community.

After establishing rules of confidentiality he revealed he was a high level crime boss; essentially a mobster. Throughout the interview he spoke in a rather pronounced Brooklyn accent. He preferred to call himself “a highly creative business man.” The client’s chief complaint was a concern over a vague desire to “hurt” himself, sleep problems and was experiencing disturbing “thoughts that don’t make any sense.” He stated his thinking had become very negative, “and weighs heavy on me”, where he doesn’t see anything good or positive in his life, “and I’m normally a positive kinda guy.” Dr. Psychologist proceeded to request the client cooperate with psychological testing to help determine potential causes for the client’s reported problems. This included a clinical interview.
Mr. Soprano began his interview by telling a story about ordering a fellow mobster killed, “a couple months ago.” He said, “I liked the guy. He was my right hand man, but he screwed up Doc and coulda’ got me whacked so ahh, ya know, he had to go but I felt really bad about it. Hey ‘fergit’ about it, it was a business decision. Sometimes ya make those tough one’s but this one really bothered me ya know.” He stated he noticed in the days following, his thoughts would race and be dominated by feelings of guilt. In the last seven weeks, he went through periods of sleeping much more than normal or suffering from insomnia when he felt “stressed out and I don’t get stressed out, Doc. Normally I sleep like a baby.” He also related in the last month, he had been battling with the strong desire to stay home and “hibernate” and would struggle to fight the fatigue and loss of interest through these times “but I have a business to run. DAVID COOK: I'm David Cook, and I've been in the mental health field for over
25 years. And many of these years have been spent as a substance abuse
counselor and working with people with behavioral and chemical addictions.
People can become addicted to chemicals as well as behaviors. Any chemical
that has the ability to alter the mood or any behavior that has the ability to alter
the mood or is sensationalistic is potentially addictive.
Although chemical and behavioral addiction share many of the same signs and
symptoms, there are also distinct differences between the two. Two similarities
between chemical and behavioral addictions are that they both involve the
compulsive repetition, in spite of negative consequences. And they both
progressively worsen unless treated.
An important difference between the two, and a distinction that should always be
realized, is that behavioral addictions are often hidden addictions. There are no
urinalysises or drug screenings for compulsive gambling. Or you can't smell
pornography on someone's breath. So consequently, because behavioral
addictions are often hidden, the signs and symptoms continue along their
destructive pathways undetected much longer than chemical addictions.
When working with people with chemical and behavioral addictions, the
addictions counselor will find that addicts have many barriers along their road to
recovery.
NICOLE: My name is Nicole. I'm 38 years old, and May 30 I will have five years
in recovery.
RICKY: My name is Ricky. I'm 52 years old. I've been in recovery for 10 years.
GRETCHEN: My name is Gretchen. I'm 44, and I've been in recovery for about
three years.
JASON: My name is Jason. I'm 32, and I've been in recovery for five months.
ODESSA: My name is Odessa. I am 47 years old, and I have been in recovery
for six years.
SHANE: My name is Shane. I am 39 years old, and I have 11 years clean.
NICOLE: My addiction began when I was a child, always feeling different from
everybody else. I was ADHD, always bouncing off the walls. And that's when I
first got my first experience with pills. And I remember that chemical feeling of
feeling relaxed, and I liked it. And throughout my adolescence, I experimented

with marijuana, pills, alcohol, whatever-- just whatever I could get my hands on,
really.
I had a schedule where I would drink at noon, and then I would drink at 2:00. I

thinking and addiction is absolutely crazy.
And how you just convince yourself and can hide it from your family. And you go
to different liquor stores so they don't know, and your friends don't know and
don't tell your fami ...

Audits and hotlines stack up as the bestcrime busters in a new ACFE study.
Occupational fraud has become--at least so far--the crime of the 21st century. It is a widespread phenomenon that affects practically every organization. The frauds in the 2004 Report to the Nation on Occupational Fraud and Abuse, from the Association of Certified Fraud Examiners, caused over $761 million in total losses, with a disproportionate percentage committed against small businesses--almost half of the frauds in the study took place in businesses with fewer than 100 employees. Not surprisingly such businesses are less likely to be audited or employ antifraud measures than the larger ones.
Several broad conclusions can be drawn from the 2004 report. First, though the losses have been stable over the years, the fact that in one year alone they are approaching $660 billion is cause for concern. Dishonest executives and employees are plying essentially the same schemes with the same results. Second, although large financial statement frauds receive the most attention, they are relatively uncommon compared to asset misappropriations and corruption. Third, small businesses remain the most vulnerable to occupational fraud because of three factors: They are the least likely to have an audit, a hotline or adequate internal controls. Fourth, audits--both internal and external--although excellent prevention devices are not the most effective means of detecting frauds. Fifth, hotlines and other reporting mechanisms are a vital part of any organization's prevention efforts but should go beyond employees to vendors and customers, too. Finally, occupational fraud cannot be eliminated but organizations that use both hotlines and auditors can greatly reduce these costly crimes.
Occupational fraud schemes can be as simple as pilferage of company supplies or as complex as sophisticated financial statement frauds. This article summarizes some of the key findings of certified fraud examiners (CFEs) in cases they investigated. Internal and external auditors and CPAs advising small business clients will learn of the most effective antifraud measures.
MEASURING THE COST OF FRAUD
Determining the true cost of occupational fraud is an impossible task. Because fraud is a crime based on concealment, organizations often do not know when they are being victimized. Many frauds never are detected or are caught only after they have gone on for several years. Many of those are never reported or prosecuted. In fact, there is no agency or organization that is specifically charged with gathering comprehensive fraud-relat.

She often worries about her time running out for establishing a good relationship with a man, getting married, and raising a family. Her friends tell her she gets way too anxious around men, and, in general, she needs to relax a little.
Case Study 3:
Jim was vice-president of the freshmen class at a local college and played on the school’s football team. Later that year, he dropped out of these activities and gradually became more and more withdrawn from friends and family. Neglecting to shave and shower, he began to look dirty and unhealthy. He spent most of his time alone in his room and sometimes complained to his parents that he heard voices in the curtains and in the closet. In his sophomore year, he dropped out of school entirely. With increasing anxiety and agitation, he began to worry that the Nazis were plotting to kill his family and kidnap him.
Case Study 4:
Larry, a 37-year-old gay man, has lived for three years with his partner, whom he met in graduate school. Larry works as a psychologist in a large hospital. Although competent in his work, he often feels strained by the pressures of his demanding position. An added source of tension on the job is his not being out with his co-workers, and, thus, he is not able to confide in anyone or talk about his private life. Most of his leisure activities are with good friends who are also part of the local gay community.
For each case, identify the individual's behaviors that seem to be problematic for the patient.

Introduction to Quality Improvement Essentials
This is a presentation by Dada Robert in a Your Skill Boost masterclass organised by the Excellence Foundation for South Sudan (EFSS) on Saturday, the 25th and Sunday, the 26th of May 2024.
He discussed the concept of quality improvement, emphasizing its applicability to various aspects of life, including personal, project, and program improvements. He defined quality as doing the right thing at the right time in the right way to achieve the best possible results and discussed the concept of the "gap" between what we know and what we do, and how this gap represents the areas we need to improve. He explained the scientific approach to quality improvement, which involves systematic performance analysis, testing and learning, and implementing change ideas. He also highlighted the importance of client focus and a team approach to quality improvement.

This transformation marked the beginning of an era defined by unprecedented territorial expansion, architectural marvels, and profound cultural influence.
The empire's roots lie in the city of Rome, founded, according to legend, by Romulus in 753 BCE. Over centuries, Rome evolved from a small settlement to a formidable republic, characterized by a complex political system with elected officials and checks on power. However, internal strife, class conflicts, and military ambitions paved the way for the end of the Republic. Julius Caesar’s dictatorship and subsequent assassination in 44 BCE created a power vacuum, leading to a civil war. Octavian, later Augustus, emerged victorious, heralding the Roman Empire’s birth.
Under Augustus, the empire experienced the Pax Romana, a 200-year period of relative peace and stability. Augustus reformed the military, established efficient administrative systems, and initiated grand construction projects. The empire's borders expanded, encompassing territories from Britain to Egypt and from Spain to the Euphrates. Roman legions, renowned for their discipline and engineering prowess, secured and maintained these vast territories, building roads, fortifications, and cities that facilitated control and integration.
The Roman Empire’s society was hierarchical, with a rigid class system. At the top were the patricians, wealthy elites who held significant political power. Below them were the plebeians, free citizens with limited political influence, and the vast numbers of slaves who formed the backbone of the economy. The family unit was central, governed by the paterfamilias, the male head who held absolute authority.
Culturally, the Romans were eclectic, absorbing and adapting elements from the civilizations they encountered, particularly the Greeks. Roman art, literature, and philosophy reflected this synthesis, creating a rich cultural tapestry. Latin, the Roman language, became the lingua franca of the Western world, influencing numerous modern languages.
Roman architecture and engineering achievements were monumental. They perfected the arch, vault, and dome, constructing enduring structures like the Colosseum, Pantheon, and aqueducts. These engineering marvels not only showcased Roman ingenuity but also served practical purposes, from public entertainment to water supply.

- 1. The Five stages of Change By Thursday, June 25, 2015, respond to the discussion. Discussion Question Anthony is a 27 year old heterosexual Caucasian male. He was arrested 2 weeks ago for his second DWI and is facing a license suspension. He works as a delivery driver for a local store and after disclosing the arrest to his employer, as well as the consequences including loss of his license, he was terminated. Anthony lives with his girlfriend of 3 years and their 2 year old son. Anthony’s drinking behavior has increased to consumption of a case of beer on Saturday and Sunday evenings each week. He consumes several beers after work during the week “to maintain.” He has also been using methamphetamines, specifically “crystal meth” several times weekly. Anthony’s girlfriend ended their relationship as a result of his increasing substance use and ongoing difficulties. Anthony feels depressed and anxious about his current life situation, especially now that he realizes that he has no job and may be homeless because of his substance use. He is also feeling down about the loss of his relationship. He researched a few outpatient treatment programs to help him stop using both alcohol and methamphetamines, but is ambivalent about entering treatment. Anthony has considered the need to stop using substances to improve his life and relationships with significant others, though fears that he will lose his friends and miss partying with them if he stops. He also fears what life will be like without the comfort of getting high. Consider and discuss the 5 stages of change. Based upon the information provided discuss what stage Anthony is in, and provide a rationale for your decision.
