INFORMED CONSENT FORM REDWOOD FALLS PUBLIC LIBRARY ACTIVITIES I, _____________________________________________________, am registered to participate in the Teen Lock-In on Friday, August 21, 2009, at the Redwood Falls Public Library. I understand that the Lock-In will begin promptly at 8:00 p.m. and will end at 8:00 a.m. on Saturday, August 22nd, and that I am required to remain at the library during this entire time. If I choose to leave the library for any reason and at any time during this event library staff will immediately notify my parents. I also understand that I share responsibility for safety in the program and agree to comply with the instructions and directions of the Redwood Falls Public Library staff members and other adults during the program in which I am enrolled. If I choose not to comply, library staff will notify my parents. Parents will then be expected to come and pick me up within a reasonable time to take me home. I hereby release the Redwood Falls Public Library from any and all liabilities to me with respect to injury, sickness, disease, loss or damage. I also consent to receive medical treatment, which may be deemed advisable in the event of an injury, accident or illness during this program. Additionally, I hereby authorize and give my full consent to the Redwood Falls Public Library, to copyright or publish all photographs in which, I, the undersigned, appear while enrolled in this program.
