CMMB works in Peru to reduce infant mortality through community health programs. Health is a continuous state of physical, mental and social well being and not merely the absence of disease or infirmity, and the person should be able to lead socially and economically productive life (WHO definition). It is very much essential to maintain the health of all the people all the time to keep ourselves healthy, happy and long living. For example you cannot fly peace fully in the flight if someone tells that some people in the same flight is having H1N1 infection. Whether it is communicable disease or non communicable it is necessary to keep all the people healthy. For example a young driver getting painless myocardial infarction can consume the life of all the passengers of his bus or the bus can hit the VIP's car travelling in the same road. So the leaders of the nation / world should not have the attitude that why should I bother if someone is sick somewhere. I feel the leaders of the nation and the world will understand the importance of maintaining the health of all the people with this simple examples. In this chapter some of the facts and figures related to the health is mentioned and it tells that we have not achieved the goals in health, that, what we can achieve for whatever the reasons. The reason may point towards anything like the doctor, minister, staff of the hospital, availability of the facility, roads, infrastructure, transportation facility and so on. Let us try to make all the people healthy, young and energetic.

Further, they will be able to express their difficulties to health workers and family, friends during the adolescent period without any hesitation because these issues are often culturally sensitive.

The 2005 ACC/AHA guidelines for diagnosis and management of chronic heart failure updated recommendations for use of implantable cardioverter-defibrillators (ICDs) and cardiac resynchronization therapy (CRT). The guidelines recommend ICDs for primary prevention of sudden cardiac death in selected heart failure patients based on new evidence. ICDs and CRT are recommended for certain patient groups based on left ventricular ejection fraction, heart failure symptoms, and underlying heart disease or clinical history, as supported by major clinical trials. Ejection fraction measurement is recommended for initial evaluation and monitoring of heart failure patients.
Surgical management of non pediatric ectopia lentis
Ectopia lentis is a partial displacement of the lens caused by the weakness of the zonule. Non traumatic cases can be primary or associated to diseases like Marfan syndrom, homocystinuria and others. The aim of our study is to compare two different surgical techniques for the management of severe cases of ectopia lentis using escleral fixation procedures vs iris-claw lenses.
We performed an interventional consecutive case. The first group included 10 eyes from 5 patients were treated with lens aspiration and Cionni ring scleral fixation and in the bag intraocular IOL or scleral fixated IOL. In the second group we have included 12 eyes from 6 patients treated with pars plana lensectomy, iridectomy and iris-claw anterior IOL. The patients underwent a full ophthalmologic examination including: CVA, keratometry, pachymetry, slit-lamp evaluation, intraocular pressure measurement, posterior segment evaluation, endothelial cell count.
Patients aged 2 to 16 yo. Minimum postoperative follow-up was 2 years in the first group and 6 months in the second group. All patients improved visual acuity by 2 or more Snellen lines after surgery. Complications included scleral-fixated complex subluxation in 2 eyes, IOL dislocation in 1 eye, retinal detachment in 3 eyes and endophthalmitis in 1 eye.
Both techniques are useful for the management of ectopia lentis. Complications in both groups were similar. Scleral-fixated IOL have a higher rate of dislocation and iris-claw IOL require a close follow-up of corneal endothelial cells count. Our actual choice is the second technique because of its simplicity and easier reversibility.

The Europe and Central Asia region has outperformed the rest of the world in the rate of decline in child mortality, with deaths of children under five and infants more than halved since the 1990s. As the situation of the poorest households improves, the difference in the health status of the richest and poorest populations decreases.
However, persistent inequalities reflect a failure to effectively invest in child-centred universal health systems. For example, in South-Eastern Europe, the mortality of Roma children is two to three times higher than the national average.

Ectopia lentis is a partial displacement of the lens caused by the weakness of the zonule. Non traumatic cases can be primary or associated to diseases like Marfan syndrom, homocystinuria and others. The aim of our study is to compare two different surgical techniques for the management of severe cases of ectopia lentis using escleral fixation procedures vs iris-claw lenses.
We performed an interventional consecutive case. The first group included 10 eyes from 5 patients were treated with lens aspiration and Cionni ring scleral fixation and in the bag intraocular IOL or scleral fixated IOL. In the second group we have included 12 eyes from 6 patients treated with pars plana lensectomy, iridectomy and iris-claw anterior IOL. The patients underwent a full ophthalmologic examination including: CVA, keratometry, pachymetry, slit-lamp evaluation, intraocular pressure measurement, posterior segment evaluation, endothelial cell count.
Patients aged 2 to 16 yo. Minimum postoperative follow-up was 2 years in the first group and 6 months in the second group. All patients improved visual acuity by 2 or more Snellen lines after surgery. Complications included scleral-fixated complex subluxation in 2 eyes, IOL dislocation in 1 eye, retinal detachment in 3 eyes and endophthalmitis in 1 eye.
Both techniques are useful for the management of ectopia lentis. Complications in both groups were similar. Scleral-fixated IOL have a higher rate of dislocation and iris-claw IOL require a close follow-up of corneal endothelial cells count. Our actual choice is the second technique because of its simplicity and easier reversibility.
