DESIGNING SURVEY
QUESTIONNAIRES
2023
QUESTIONNAIRE
a series of questions
asked to individuals to
obtain statistical useful
information about a
given topic.
a list of mimeographed
or printed questions that
is completed by or for a
respondent to give his
opinion.
SURVEY
is a general view or description of
someone or something
method of gathering information from a
sample of individuals in order to lean
something about the large population from
which the sample has been drawn.
SURVEY
QUESTIONNAIRE
a set of questions
to a subject who
with his/her
responses will
provide data to a
researcher.
A GOOD
QUESTIONNAIRE
asks what it intend to ask
it gets the same answer if
the same question is
posed repeatedly in a
short time
it is easily understood
Valid
Reliable
Clear
completed by the
repondents and gets
better reponse rate

questionnaire
DESIGN
2023
CLOSED-ENDED
questions provide
options to the
respondents and
require them to
choose one or
more items from
the list. This is used
if the range of
answers are well
known and the
options are limited
OPEN-ENDED
questions allow the
respondents to
express their
opinions freely and
they are not
restricted by the
options.
this is used if the
answer options are
mutiple and
unknown.
CONTINGENCY
QUESTIONS
special case of a
closed-ended
question because
it applies only to a
subgroup of
respondents.
the relevance of
the questions for a
subgroup is
determined by
asking a filter
question.
Use appropriate format
Avoid double-barreled
Arrange the questions logically
Avoid ambiguous questions
Make instructions clear

OBSERVATION
2023
Formulate the survey, keeping in mind
your overall substantive and analytical
needs.-
Define the problem you want to
examine
Identify the population that be
surveyed
Determine the kinds of variables that
you want to measure
What quiestions do you want to have
answered
What's the best way to go about
getting those answers?
1.
Personal approach (Face to
face structured interview;
Telephone survey)
Self-administered approach
(paper and pencil survey;
online survey; mail survey)
2. Determine specifically what
mode of collecting the data will
be used.
by size and type of sample - special
population, groups, geographical area,
etc.
by method of data collection -mail,
telephone interview and in-person
interview. Chart audits - - extracting the
data from sample of medical and other
records- come under the heading of
survey methods
by survey contect- voter preferences,
consumer spending, tnransportation
habits and health issues.
Survey methods can be classified in many
ways:
3.. Determine the number of respondents/
participants of the survey.
4. Develop the questionnaire (the survey
instrument)
5. If you are using telephone or personal
interviews, be sure the interviewers are
carefully trained
6. Conduct an early pretest of the survey,
whenever possible
7. Execute the survey in the field and be
ready to deal with problems
8. Edit and process the data
9. Analyze the data.
