1) This case describes a successful retrograde percutaneous coronary intervention (PCI) for chronic total occlusion of the left circumflex artery (LCX) via an intra-left anterior descending (LAD) collateral channel. 2) An 8 French sheath and guide catheter were used from the femoral artery to facilitate the retrograde approach via the LAD collateral. 3) Dual antegrade and retrograde wire techniques including the knuckle wire technique were used to successfully cross the CTO lesion. Intravascular ultrasound (IVUS) guided the final stent placement.
