This document contains a survey for students from the Intercambio Motherwell – Niño Jesús del Remedio 2012 exchange program to rate and comment on various activities during their visit. The survey includes ratings for 20 different activities using smiling/frowning face icons to indicate if an activity was very bad, normal, or very good, along with a comments section for each one. The activities span visits to educational sites, parks, workshops, museums, beaches, and hotels during the students' time in Madrid and other cities in Spain.
