A Japanese clinical psychologist stayed in the Kesennuma City for nearly two years after the earthquakes on 11 March 2014. He worked with students, their parents, and teachers in schools.

and is “one of the fundamental rights of every human being,
without distinction of race, religion, political belief, economic or
social condition.”
「健康とは、完全な身体的、精神的および社会的に完全によい
状態であって、単に病気あるいは欠陥がないことだけではない。
そして、それは人種、宗教、政治的信念、経済的あるいは社会
的条件の差別なく、各人の基本的権利の一つである。」
14. WHO緩和ケアの定義(2002)
Palliative care is an approach that improves the quality of life of patients and their
families facing the problem associated with life-threatening illness, through the
prevention and relief of suffering by means of early identification and impeccable
assessment and treatment of pain and other problems, physical, psychosocial and
spiritual.
Palliative care
・ provides relief from pain and other distressing symptoms;
・ affirms life and regards dying as a normal process;
・ intends neither to hasten or postpone death;
・ integrates the psychological and spiritual aspects of patient care;
・ offers a support system to help patients live as actively as possible until death;
・ offers a support system to help the family cope during the patients illness and in their own
bereavement;
・ uses a team approach to address the needs of patients and their families, including
bereavement counselling, if indicated;
・ will enhance quality of life, and may also positively influence the course of illness;
・ is applicable early in the course of illness, in conjunction with other therapies that are
intended to prolong life, such as chemotherapy or radiation therapy, and includes those
investigations needed to better understand and manage distressing clinical complications.
24. 「早期から」の緩和ケアのエビデンス
Early Palliative Care for Patients with Metastatic Non-Small-Cell Lung Cancer

【目的】
本研究では転移を伴う非小細胞性肺がんの患者の診断時からの早期緩和ケアの導入が患者のQOLおよび終末期のケアを改善する
かについて無作為化臨床試験において検証することを目的とした。
【方法】
転移を伴う非小細胞性肺がんと診断された患者を「標準的ケア+緩和ケア」と「標準的ケアのみ」にランダム割り付けした。評価指標は
QOLと精神症状であり、ベースラインと12週後にQOL尺度であるFACT-Lと抑うつ、不安の測定尺度HADSを測定した。主要評価項目は12
週後のQOLの変化とした。終末期のケアに関しては病院の電子記録からデータを抽出した。
【結果】
151人が無作為割り付けの対象となった。27人が12週までに死亡したが、残りのうち86%である107人のQOLを12週後に評価することが可能であっ
た。早期から緩和ケアを受けた群の患者のほうが有意にQOLが良好だった(FACT-Lで「標準的ケア+緩和ケア」群98.0、「標準的ケア群」91.5、
P=0.03:以下の記述も同順)。早期から緩和ケアを受けた群の患者は抑うつ症状の割合が少なかった(16%、38%、P=0.01)。早期から緩和ケアを受
けた群の患者は終末期に積極的治療を受けている割合が少なかったにも関わらず(33%、54%)、生存期間の中央値は有意に長かった(11.6か月、
8.9か月、P=0.02)
【結論】
転移を有する非小細胞性肺がんの患者は早期から緩和ケアを導入することによりQOLと精神症状を改善し、終末期の積極的治療を抑
制したにも関わらず、生存期間を延長した。
