This study evaluated agreement between multidisciplinary teams (MDTs) from seven countries on diagnoses of diffuse parenchymal lung disease. 70 patients were evaluated independently by clinicians, radiologists, pathologists and each MDT. Inter-MDT agreement on first choice diagnoses was moderate. Agreement was good for idiopathic pulmonary fibrosis (IPF) and connective tissue disease, moderate for non-specific interstitial pneumonia, and fair for hypersensitivity pneumonitis. MDTs diagnosed IPF with higher confidence and frequency than individual clinicians or radiologists. Inter-MDT agreement for hypersensitivity pneumonitis was low, highlighting the need for standardized diagnostic guidelines.

Abstract Objective: Traditional Chinese medicine (TCM) has been extensively used as one of popular alternative therapies for several cancers. However, it remains unclear whether TCM treatment is associated with longer survival in lung cancer patients. In this study, we explored the effect of long-term TCM treatment on patients with different stages of lung cancer. Methods: All information of lung cancer patients with stage I-III disease from January 2007 to September 2015 was collected for this retrospective cohort study. Those who were treated with TCM after surgery were divided into TCM group and the others were into the non-TCM group (control group). All patients were regularly followed up by clinic appointment or phone, and all survival data were collected from databases after the last follow-up in October 2017. Results: A total of 575 patients were included in this study, with 299 patients in the TCM group and 276 in the control group. For all patients, 5-year disease-free survival (DFS) was 62.2% in TCM group and 42.1% in the control group, and 6-year DFSs were 51.8% and 35.4%, respectively (HR = 0.51, 95% CI: 0.40 to 0.66, log-rank P ≤ 0.001). For patients with stage I, 5-year DFSs were 83.7% (TCM group) and 57.5% (control group) and 6-year DFSs were 73.7% and 51.9%, respectively (HR = 0.30, 95% CI: 0.18 to 0.50, log-rank P ≤ 0.001). For patients with stage II in the TCM group and the control group, 5-year DFSs were 59.4% and 17.6% and 6-year DFSs were 44.7% and 17.6%, respectively (HR = 0.31, 95% CI: 0.19 to 0.52, log-rank P ≤ 0.001), and for patients with stage III, 5-year and 6-year DFSs in the TCM group were 18.7% and 12.5% compared with 28.4% and 20.3% in the control group (HR = 1.06, 95% CI: 0.72 to 1.56, log-rank P = 0.76). Conclusions: This study demonstrated that long-term TCM treatment as an adjuvant therapy is able to improve the DFS of postoperative stage I-III lung cancer patients, especially in patients with stage I and II disease. However, these observational findings need being validated by large sample randomized controlled trials.

Of 285 people who experienced shortness of breath in the past year, only 93 (33%) had ever consulted their GP for this. Neither perception of symptoms nor psychological factors could explain the underpresentation. A random sample of over 1,000 people was screened for respiratory symptoms, and those reporting shortness of breath were studied further. This study examined the attitudes and treatment of 3,415 asthma patients in 11 countries who were prescribed regular maintenance therapy of inhaled corticosteroids. The study found that 74% used short-acting beta-agonists daily and 51% had uncontrolled asthma according to a questionnaire. Even patients with well-controlled asthma reported an average of 6 worsening episodes per year. While patients recognized the early signs of worsening, the most common response was to increase short-acting beta-agonist use rather than inhaled corticosteroids. This represents a missed opportunity to properly manage worsening asthma.

Just as it is complicated to the patients, so is it on service provision, hence mandating for family and friend to be a part of a multidisciplinary approach in relation to provision of care. Hence as a result, the issue on chronically ill patients is not one that can be handled by a single profession but rather one that requires a multidisciplinary approach and a care coordination plan to best deal with the health concerns (Hannigan et. al., 2018).

COPD is a type of obstructive lung disease often characterized by long-term breathing problems and poor airflow resulting in difficulties when breathing. It tends to be characterized by shortness of breath as well as coughs accompanied with sputum production. Therefore, patients with COPD tend to present various complex health issues that demand for immediate pharmacological interventions (Hanania et. al., 2018). Care coordination assists in solving these complexities and in providing alternatives to quality health care. By offering a collaborative approach it is possible to address not only the patient’s needs in relation to the respiratory aspect of the disease but also in addressing the systemic effects and comorbidities associated with the illness (Hanania et. al., 2018). According to the Journal of Chronic Pulmonary Diseases, the most appropriate practices for care coordination for COPD include pulmonary rehabilitation, care integration, a holistic approach and an inclusion of a care transition model. These practices help to meet the complex needs of managing COPD. However, regardless of the clear goals in managing COPD, there still exist certain challenges (Hanania et. al., 2018). One such challenge is dealing with the hopelessness of the condition. Often physicians face uncertainties in diagnosing the disease, identifying its medications and hospitalizations. Even worse, patients in the past have shown non-compliance as a result of the disease’s complexity (Hanania et. al., 2018). All in all, an effective care coordination plan would help to effectively manage the condition of COPD.

EXACERBATION OF COPD _ 11
Acute exacerbations of chronic obstructive pulmonary disease (AECOPD) are associated with high costs, morbidity, and mortality. They result in enormous healthcare expenditures and lost productivity. AECOPD episodes are also linked to accelerated long-term loss of lung function in continuous smokers. While symptoms may recover within a week, full recovery of health-related quality of life can take months. Noninvasive mechanical ventilation should be tried for patients with respiratory acidosis or increased work of breathing from AECOPD as it reduces intubation rates and improves health outcomes. Preventing and properly managing AECOPD is important for reducing its impacts.

As a registered nurse working as a case manager within the home health care setting, I have had the opportunity to provide care to patients diagnosed with various respiratory disorders. A majority of the patients I have worked with were diagnosed with chronic obstructive pulmonary disease (COPD). COPD is defined as a common preventable and treatable disease characterized by persistent airflow limitation that is usually progressive and associated with an enhanced chronic inflammatory response in the airways and the lung to noxious particles or gases (Huether, 2017). Two important facts regarding this respiratory disorder include the following:
COPD is the third leading cause of death in the United States accounting for 138,080 deaths in 2010.
In 2010, the cost of COPD in the United States was estimated to be nearly $50 billion, including nearly $30 billion in direct health care expenditures.
These figures detail the staggering numbers of patients living with COPD and the significant impact on patients, families, communities and the health care system.
During the time that I worked with COPD patients, one of the respiratory disorders of particular interest was emphysema. I wanted to make sure I understood the disease process so I could provide the most appropriate care and teaching to my patients, families and caregivers. Emphysema is abnormal permanent enlargement of gas-exchange airways (acini) accompanied by destruction of alveolar walls without obvious fibrosis (Huether, 2017). Furthermore, the American Lung Association defined emphysema as the gradual damage of lung tissue, specifically thinning and destruction of the alveoli or air sacs (
www.lung.org
). I often used this definition with patients to help them understand how this respiratory disorder effects the body. The pathophysiology of emphysema includes the following:
Air sacs are destroyed in emphysema, making it progressively difficult to breathe.
Emphysema is usually accompanied by chronic bronchitis, with almost-daily or daily cough and phlegm.
Cigarette smoking is the major cause of emphysema.
People with emphysema experience shortness of breath with activities
It is not curable, but there are treatments that can help you manage the disease (www.lung.org).
Medication management of emphysema varies depending upon severity of the disease. Initial drug therapy selection depends on COPD severity, symptoms, and exacerbation risk. In addition, medication therapy may be based upon Global Obstructive Lung Disease (GOLD) guidelines which categorized COPD into four groups (A, B, C, D) ranging from low risk, less symptoms to high risk, high symptoms (Arcangelo, 2017). This study aimed to evaluate the efficacy of oral montelukast in acute asthma exacerbation. It was a randomized, double-blind, placebo-controlled trial conducted in a tertiary care hospital over 2 years. Patients presenting with acute asthma exacerbation were randomized to receive standard therapy with either montelukast or placebo. The primary outcomes of lung function, duration of hospital stay, and secondary outcomes showed no significant differences between the montelukast and placebo groups. The study concluded that montelukast provided no added benefit over standard therapy alone for acute asthma exacerbation. Larger multicenter trials are still needed.
