This study examined the risk of serious infection in patients with rheumatoid arthritis (RA) and interstitial lung disease (ILD). The study identified 181 RA patients with ILD between 1998-2014. It found that these patients faced a high risk of serious infections requiring hospitalization, with an overall rate of 7.4 infections per 100 person-years. The risk was highest for patients with organizing pneumonia ILD (27.1 per 100 person-years) and lower for nonspecific interstitial pneumonia and usual interstitial pneumonia. Use of high-dose prednisone (>10mg per day) was also linked to greater infection risk. Identifying patients at highest risk could help reduce infection-related morbidity.

Early initiation of antiretroviral therapy (ART) within 6 months of HIV infection contributes to the normalization of CD8 T-cell counts. The study found that:
1) CD8 counts decreased significantly from 797 to 588 cells/μL over 24 months in individuals who started ART early, reaching lower levels than those without ART or with delayed ART.
2) CD4 T-cell recovery was more prominent in individuals who started ART early compared to those with delayed ART.
3) ART initiated early in HIV infection is associated with improved resolution of elevated CD8 T-cell counts compared to long-term ART initiated in chronic infection, which may help reduce non-AIDS related events.

Cervical Cancer Disease Burden
Cervical cancer is a major public health problem in India, with over 132,000 new cases and 74,000 deaths annually. India accounts for about 27% of the world's cervical cancer cases and deaths despite having only 17% of the global female population. Human papillomavirus (HPV) infection, which is very common in India, is the primary cause of cervical cancer. HPV types 16 and 18 cause around 75% of cervical cancer cases in India. Without widespread screening or vaccination efforts, cervical cancer will likely remain a leading cause of cancer deaths among Indian women.
Controversies in initial management of
1) The initial management of open fractures, including timing of debridement, choice of antibiotics, and timing of wound coverage, involves several controversies with little consensus in the literature.
2) While early antibiotic administration and debridement are agreed upon, there is no evidence that debridement must occur within 6 hours as was once believed; many surgeons now find urgent rather than emergency debridement acceptable.
3) Timing of wound coverage is also debated, but most evidence suggests covering Type III wounds within 7 days is appropriate once tissues have stabilized and debridement is complete.
Controversies in initial management of

Introductions
Disparities between men and women are omnipresent in many malignancies. In colon cancer, women usually fare slightly better. However, women also often tend to be underrepresented at screening initiatives. We hypothesized that the introduction of nationwide screening for colorectal cancer could have led to shifts in the status quo. We therefore assessed whether differences existed in terms of screening presentation at our center and whether this influenced staging and outcomes in our colon cancer population.
Methods
We included all patients over 50 without a previous history of colorectal cancer that have been treated surgically for colon cancer at our center from 2004 through 2011. Screening events included screening colonoscopies, as well as fecal occult blood tests. Chi-square statistics and relative risk (RR) computations assessed for the significance and magnitude of differences in screening rates between genders; as well as differences in several pathologic characteristics and death rates between women not diagnosed through screening and the remainder of the population.
Results
We included 919 patients, of whom 451 (49.1%) were female. Women were less likely to be diagnosed through screening (26.2% vs. 32.7%; RR: 0.8, 95%CI 0.66-0.98; P=0.037). Compared to the remainder of the population, women not diagnosed through screening were at significantly higher risk (all P<0.001) for having high-grade tumors (RR=1.61), lymph node metastasis (RR=1.37) and distant metastasis (RR= 1.65). This is reflected quite clearly in relative risk of death directly and uniquely attributable to colon cancer (RR: 1.65).
Conclusions
Female patients were less likely to be diagnosed with colon cancer through screening methods and subsequently were at risk for presenting with advanced disease and thus having higher mortality. This demonstrates the beneficial effect of diagnosis through screening, and shows that disparities in screening rates lead to disparities in outcomes.

— Wound infection is the second commonest complication of wound healing. This study was carried out on 250 post-operative cases operated at a district hospital of western Rajasthan, India with the aim the aim to find out prevalence of post-operative surgical site infection and its causing organism. After taking personal information and detailed clinical, operative and post-operative history of these cases, swab from post-operative wound was taken and sent for culture and sensitivity test in Microbiology. Association was inferred with Chi-square test. Post operative surgical site infection rate was found 11.6 % which was found significantly more in intestinal surgeries than the other. Most common causative organism for infection was Staphylococci cases followed by Streptococci, E. Coli and Klebsella. Out of total 11.6% infected cases, majority (8.8%) of patients had infection with more than one organism only 2.8% were having single organism.
