6. SSAFA Hallam Care LLP
• In 2009 Hallam Medical joined forces with SSAFA (Soldiers, Sailors, Airmen and Families
Association) to form SSAFA Hallam Care LLP
• The result was a unique and innovative service providing autonomous care at first point of contact
using a Team of Advanced Practitioners.
• Initially an 8-8 service, which extended over the course of the year to become 24/7
• Patients numbers seen by the service averaged approx. 500 per Month
• Referrals were received from Primary Care, Community Services, Ambulance Services, Care Homes
and many others.
Outcomes:
• The service avoided admissions in over 80% of patient contacts
• The service achieved a Financial Saving for the Trust of £1M (after all costs) in the first year of
operation
• High levels of patient satisfaction, with safe and appropriate outcomes and financially viable

7. Nottingham CityCare Partnership
• CityCare is an award-winning Community Health services provider, dedicated to working in
partnership with the NHS to improve long-term patient health and wellbeing
• Delivery teams required to meet the increasing demand upon the following services:
Fully autonomous Advanced Nurse Practitioners to see, treat, diagnose and refer patients with
undifferentiated and undiagnosed conditions (Walk-in Centre & Care Homes)
Patient assessment skills and Community experience (Patient Flow Team)
Rehabilitation and Community experience (Reablement/Intermediate Care Team)
Experience and qualification in Management of Respiratory conditions such as COPD, Asthma, Spirometry
(Respiratory Specialist Nursing Team)
• Our partnership approach with the CityCare Procurement Team allowed the development of a
specific set of recruitment models, designed to mobilise the workforce and deliver co-ordinated
support across all of their Community services
Outcomes:
• Successful delivery of community services in line with CCG and patient demand & enhanced
patient outcomes
• Time & efficiency savings via streamlined process for Management (x4 Days per Month)
• Significant financial savings via standardised Pay and Charge vs Standard Delivery Models
• Compliance and Clinical Governance from an LPP National Framework approved supplier
• Streamlined communication and recruitment process allowing for rapid staff ramp up for new
services
8. Cambridgeshire & Peterborough CCG
• Cambridgeshire & Peterborough CCG planned to deliver a Community based Admission Avoidance
service on an initial 6 month pilot to reduce the increasing number of hospital admissions
• Following initial engagement x2 separate Admission Avoidance services were piloted:
A&E Front Door Discharge Team (FDDT) for Primary Care patients presenting to A&E
Intermediate Care Service; a ward based Step up/Step down service for Community patients
• Success criteria based on:
No. of patients prevented from unnecessarily attending and being admitted to hospital
Financial savings calculated using local hospital tariff charges
Improved patient outcomes
• Acting in an advisory role with the CCG Clinical Lead we fully specified the scope of practice of the
Clinicians and their ability to undertake specific roles within the pilot services leading to an
enhanced service
Outcomes:
• The A&E (FDDT) is now a fully commission service
770 Patients seen in 6 month pilot with 549 (71%) successfully avoided admissions
Estimated cost savings annualised at approx. £1.5M-£2M
• The Intermediate Care Service has extended its pilot, with new referral pathways and extended
patient groups, and doubled in size
• Due to the success of these services the CCG has additionally introduced two more innovative
services:
Care Home Educator
Community Rapid Response Team

• Hallam Medical have been involved in 12+ Primary and Community Care Admission Avoidance
Services and Pilot Schemes nationwide over the past 12 Months
• Over the same period we have also supported hundreds of Primary and Community Care services
within the NHS and on behalf of private healthcare providers
