As the Safety Director for Millis Equipment, I make safety 
a priority and oversee the following: 
 Each employee goes through a formal orientation including English/Spanish National 
Safety Compliance video with written test before entering job site. 
 Employees are issued appropriate PPE with documented training on usage and care. 
 Each Millis employee is easily identified by: Company issued hard hat with Millis logo, 
matching vests with company logo. 
 Identification stickers with each employees name on hard hat. Also 811 call before you 
dig stickers on hardhats. 
 For safety, each jobsite has a custom made 6’ fire stand with a 20lb fire extinguisher, 
first aid kit, with my contact info if anything needs to be replaced. 
 I conduct routine site inspections to enforce policy, check for hazards. Keep logs with 
pictures of progress. 
 Provide ongoing training with site specific needs including safety videos in 
English/Spanish. Mandatory that every employee is trained on fire extinguisher use, 811 
call before you dig practices, trenching and excavation safety. 
 I make an effort to meet with each jobsite superintendent to go over any concerns, 
hazards specific to that site. 
 Each piece of equipment owned by Millis is identified with a company logo; daily log 
books filled out by operators to maintain OSHA compliance. 
 Each operator is issued an operator card with proper documentation including: date of 
testing, trainer, equipment allowed to operate, expiration date, written skills test. 
 Ensure that each piece of equipment has a fire extinguisher compliant with OSHA 
regulations. 
 I am a certified flagger and make sure each site with road closure has proper barricades; 
employees are trained and have proper stop/slow signs to direct traffic. 
 Occasional site security inspections after hours with documentation and pictures saved. 
 I have worked with Paychex Safety and loss control rep to create a Safety Manual 
tailored to Millis Equipment kept in Main office. 
 Keep in contact with CNA's risk control consultant, George Tellez, CSP,CRIS,RRE, to get 
third party advice. 
 Make myself easily available for any employee to contact me by phone /email.
