Responsibilities

  Do what you can to look after your own
   health – good diet, regular exercise,
   don’t smoke or abuse substances (drugs
   and/or alcohol).
 Care for yourself when appropriate, e.g.
 treat yourself at home for common
 ailments such as coughs, colds and sore
 throats. Get advice from a pharmacist.
 Giveblood if you are able and carry an
 organ donor card or special needs card
 or bracelet.
 Listencarefully to advice on your
 treatment and medication. Tell the
 doctor about any other treatments you
 are taking.
 TreatNHS staff, fellow patients, carers
 and visitors with respect.
 Keep your appointment of let the GP,
 dentist, clinic or hospital know if you
 cannot make it. Book routine
 appointments in advance.
 Returnany equipment that is no longer
 required, e.g. crutches.
 Payprescription charges and any other
 charges promptly when they are due.

  To a service based on clinical need not
   the ability to pay.
  To have information kept confidential.
 Rightto have access to information
 about services, options and treatment
 -including risks associated with
 treatment.
