It is difficult to fix problems you do not know about.
Often PSOs collaborate. Often a PSO will hire another entity to do the backroom analytics. Providers may want to send reports to more than 1 PSO. Maintaining the privacy and confidentiality of “patient safety work product” (PSWP) under the Patient Safety Act requires careful treatment of data.
26 states have some form of mandatory adverse event reporting. Hospitals are required to report serious injuries involving regulated medical devices to the FDA. AHRQ has also created the Network of Patient Safety Databases to collect non-identifiable AE reports in the Common Formats, as a research database.
