Consent Form
You are being asked to take part in a research study on the ​exploration into the desires
and improvement of people's livelihoods using the Internet of Things in daily life in the
near future.​ We are asking you to take part because your age fit into our matching
profile of participants for this study. Please read this form carefully and ask any
questions you may have before agreeing to take part in the study.
What the study is about:​ ​The purpose of the study is to ​explore into the desires and
improvement of people's livelihoods using the Internet of Things in daily life in the near
future .​ You must own a smartphone while take part in this study.
What we will ask you to do:​ If you agree to be in this study, we will conduct a group
exercise with you. The group exercise will take about 1 hour to complete and the
location will be held in Emily Carr University of Art and Design on ​1399 Johnston St,
Vancouver, BC​.​ Prior to the group exercise, there will be sensitizing exercise and a
questionnaires that will require you to answer question and take photo with your phone.
The question asked will be about your profession, the hours you spend online, where
you shop, the number of smart object you own, and your surrounding areas. The group
exercise will explore your concerns or problem space or new ideas about the internet of
things. With your permission, we would also like to tape­record the exercise.
Risks and benefits: ​There are no anticipated risks to your participation. When you feel
some discomfort at responding some question, please feel free to ask to skip the
question.
You will not directly benefit from your participation in this research study. The overall
goal is to reveal and ​explore into people’s desires and improvement of people's
livelihoods using the Internet of Things
Compensation:​ You will not receive any payment for your participation in this research
study.

Potential conflicts of interest: ​the investigator of this research do not have any
financial interest in the sponsor or in the product being studied.
Your answers will be confidential.​ The records of this study will be kept private. In
any sort of report we make public we will not include any information that will make it
possible to identify you. Research records will be kept in a locked file; only the
researchers will have access to the records. If we tape­record the interview, we will
destroy the tape after it has been transcribed, which we anticipate will be within two
months of its taping.
Taking part is voluntary:​ Taking part in this study is completely voluntary. You may
skip any questions that you do not want to answer. If you decide not to take part or to
skip some of the questions, it will not affect your current or future relationship with Emily
Carr University. If you decide to take part, you are free to withdraw at any time.
If you have questions:​ The researchers conducting this study are Phil Adams and
Wendy Ye. Please ask any questions you have now. If you have questions later, you

Step 1: Sensitization Material and Consent Form
A week prior to the Co­Creation Exercise the researchers will send out a Consent
Form and a Sensitization Material Package with the goal of informing the participant and
immersing them in their lived experiences.
The participants are sent a homework package of activities to conduct before coming to
the Co­Creation exercise. The first activity of the participants is a sensitization exercise
in which they read and respond to a set of questions put forward in a deck of 4 cards
and complete the brief activity attached to the card. The second step will be an
exercise in collage preparation, the participant will be given a list of criteria to complete
and instructions on how to proceed. Finally, to save time during the Co­Creation session
the questionnaire that will be referenced at Co­Creation stage will be filled out inside the
sensitization material package. Any participant that does not complete their package will
be unable to continue in the research and will be disqualified.
Exercise Steps:

Inside the sensitization package will be the Co­Creation Questionnaire and participants
are to bring it with them on session day.

Step 2: Co­creation and Activity Session
The Activities below will be conducted inside the session of Co­Creation that the
participants have prepared for. The researcher should keep all their training of leading
group activities to bare but remember principally the code of ethics of research consent.
Introduction­ Handout material, discuss ground rules for an open and friendly session,
disclosure of the exercises intentions and the sensitization material, collecting consent
forms, splitting groups in groups of four. Collect the Sensitization Cards back.
Problem Space ­ Expansion Exercise 1 Part A
The participants will explore the problems or issues within their daily life, picks the one
that annoys them the most and then writes it in the smallest circle using the
questionnaire in the Sensitization Material Package.
