This document reports on a 29-year-old HIV-positive male patient who presented with fever, cough, and other symptoms and was diagnosed with both COVID-19 infection and pulmonary tuberculosis. Despite multiple antibiotic treatments, the patient continued to have fever. A CT scan showed opacities in the right upper lobe and a cavitation in the right lower lobe. A sputum test was negative for other infections but a gene-Xpert test confirmed TB. The patient was started on a 4-drug anti-TB treatment and his symptoms improved, though he continued on treatment at a community hospital. The case illustrates the need to consider potential triple threats of HIV, TB, and COVID-19 in patients with pulmonary issues.
