- A 40-year-old female presented with a gradually enlarging midline neck swelling over the past year without symptoms of thyroid dysfunction.
- On examination, a 3cm x 4cm solitary, smooth, mobile swelling was found in the lower midline neck extending to the right sternocleidomastoid.
- Differential diagnoses included benign thyroid nodule, colloid goiter, follicular adenoma, or cyst. Tests planned included bloodwork, thyroid function tests, ultrasound of the neck, and FNAC of the neck swelling.

MRI showed disc bulges in her spine and she was found to have acute transverse myelitis. She developed recurrent urinary tract infections with Klebsiella that were difficult to treat. An abdominal mass was discovered on CT and biopsy was inconclusive. She required long-term antibiotic treatment and management of her neurogenic bladder and lupus.

CLINICAL CASE PRESENTATION OF STRESS FRACTURE OF FEMUR NECK.
This document presents a case study of a 46-year-old female patient who presented with pain in both knee joints and inguinal regions for the past 8.5 and 5 months respectively. A thorough history, examination, and investigation results are provided. The patient was diagnosed with osteoarthritis of both knees based on x-ray findings and stress fractures of the femoral neck bilaterally based on MRI pelvis. She was also found to have vitamin D deficiency. The case was analyzed based on Ayurvedic principles and diagnosed as a case of vata-kapha disorder manifesting as knee pain and stiffness.

- A young lady presented with headache, fever and gradual loss of vision with other neurological symptoms and was diagnosed with tuberculous meningitis based on investigations. She developed drug-induced liver injury during treatment and her anti-tubercular treatment was modified. She also developed seizures and hydrocephalus requiring VP shunt placement. Her condition is being closely monitored during anti-tubercular treatment and management of complications.

Puan Rahayu, a 32-year-old housewife at 34 weeks and 5 days of gestation, was admitted to the hospital due to giddiness, headache, and bilateral leg swelling for 1 day. She has a history of gestational hypertension and was diagnosed with pregnancy induced hypertension. On examination, her blood pressure was normal and she had mild pitting edema. Laboratory tests showed mild anemia. She was admitted for blood pressure monitoring and fetal heart monitoring to determine an appropriate management plan.

PGx Analysis in VarSeq: A User’s Perspective
Since our release of the PGx capabilities in VarSeq, we’ve had a few months to gather some insights from various use cases. Some users approach PGx workflows by means of array genotyping or what seems to be a growing trend of adding the star allele calling to the existing NGS pipeline for whole genome data. Luckily, both approaches are supported with the VarSeq software platform. The genotyping method being used will also dictate what the scope of the tertiary analysis will be. For example, are your PGx reports a standalone pipeline or would your lab’s goal be to handle a dual-purpose workflow and report on PGx + Diagnostic findings.

Discuss and demonstrate the approaches with array and NGS genotyping methods for star allele calling to prep for downstream analysis.
Following genotyping, explore alternative tertiary workflow concepts in VarSeq to handle PGx reporting.
Moreover, we will include insights users will need to consider when validating their PGx workflow for all possible star alleles and options you have for automating your PGx analysis for large number of samples. 5-hydroxytryptamine or 5-HT or Serotonin is a neurotransmitter that serves a range of roles in the human body. It is sometimes referred to as the happy chemical since it promotes overall well-being and happiness.
It is mostly found in the brain, intestines, and blood platelets.
5-HT is utilised to transport messages between nerve cells, is known to be involved in smooth muscle contraction, and adds to overall well-being and pleasure, among other benefits. 5-HT regulates the body's sleep-wake cycles and internal clock by acting as a precursor to melatonin.
It is hypothesised to regulate hunger, emotions, motor, cognitive, and autonomic processes.

- 2. • 24 Y Female, Lalita Raut, a Resident of CHHINDWARA, MP came to Lata Mangeshkar Hospital, Digdoh Hills, Nagpur with • C/o Chest pain since 2 months. • C/o Palpitations since 2 months.
- 3. HISTORY OF PRESENTING ILLNESS • Pt was apparently all right 2 months ago when she developed Chest pain that was insidious in onset, throbbing in character, did not get alleviated by taking rest, did not get aggrevated by doing physical work, did not radiate to other sites in the body, had no exacerbating factors and was mild in intensity. (Pain did not hamper day to day activities of patient). • Patient also c/o Palpitations that started 2 months ago, were sudden in onset, had no aggravating or relieving factors and was mild in intensity.
- 4. • Pt is a k/c/o HTN since 2 months on regular medication. (TAB METXL 50 MG PO OD) • Pt gives No H/o Trauma to Chest. • No H/o Fever. • No H/o Radiation of pain to Left Arm. • No H/o Pedal edema. • No H/o Giddiness. • No H/o Pain in legs on Exercise. • No H/o Pain in Abdomen. • No H/o similar complaints in the past.
- 5. • No H/o previous Major Surgery. • No H/o TB/BA/DM
- 6. GYNAECOLOGICAL HISTORY • Pt is unmarried, Nulligravida, LMP: 6 months ago. • No H/o white discharge pv • No H/o sudden breakthrough bleeding pv. • No H/o itching pv.
- 7. PERSONAL HISTORY • Pt takes Mixed diet. • Pt has regular bowel/ bladder movement. • Pt has regular sleep pattern. Pt is not awaken in sleep by palpitations. • Pt claims to have no addictions.
- 8. GENERAL EXAMINATION • P: 100/min, Regular, bounding pulse taken in Right Radial artery in Supine position. • BP: 150/90 mm Hg, taken in Right Forearm, Supine position by a Mercury Sphygmomanometer. • Pallor: present. Icterus: Absent. • No Cervical lymphadenopathy/ Supraclavicular lymphadenopathy.
- 9. SYSTEMIC EXAM • CVS: S1, S2 Normal. • CNS: Conscious, Oriented • RS: AEBE
- 10. PER ABDOMEN EXAM • Inspection: Abdomen is soft, Non tender, No e/o Visible lump or swelling • Palpation: done in well lit room in the presence of a female attendant. • No e/o Palpation lump, dilated veins.
