Summary:
Under the supervision of the Program Manager, the Outreach Case Manager is responsible for:
Essential Duties & Responsibilities:
1. Provide clients crisis intervention and case management services; coordinate and implement mental health
and recovery support services.
2. Ensure that medical and case management services to clients are reflective of agency model, which
emphasizes customer choice, harm reduction, and psycho-social rehabilitation.
3. Assess clients’ legal, financial, social, medical, psychological, and other needs related to their housing
situation.
4. Assess clients’ eligibility for public assistance and provide referrals and advocacy to assist them in accessing
benefits.
5. Assess clients’ ability to increase income through employment services, vocational training, and/or
educational opportunities and assist then in accessing those services as needed.
6. Develop effective, trusting relationships with clients, with a focus on facilitating independence and
maintenance of improved physical and mental health.
7. Collaborate with each client to develop their individual service and housing stability plans; review and
update plans quarterly and upon completion of goals.
8. Maintain up to date, accurate, and complete progress notes as required by the agency and funding sources.
9. Develop and maintain a complete, accurate, and current client file with all required documents; ensure all
paperwork and data collection are complete, timely, accurate, and current in agency records and systems.
10. Provide life-skills training and financial literacy training to enhance housing stability.
11. Coordinate medical, substance abuse, and mental health treatment when appropriate.
12. Mediate conflict between clients and their family and/or landlords to promote housing stability.
13. Advocate on behalf of clients to law enforcement, government agencies, social service agencies, educational
institutions, medical and legal professionals, and others.
14. Conduct site visits to client’s homes to ensure eligibility for Homeless Prevention assistance.
15. Maintain a current, thorough knowledge of community resources and utilize these to provide services to
clients.
Qualifications:
1. High School diploma or GED and two years’ experience working with homeless individuals, preferably those
living with mental illness and or substance addictions, or an equivalent combination of education and
experience.
2. Demonstrated knowledge of case management.
3. Knowledge of or ability to learn local and federal government benefits and entitlements and other social
service providers.
4. Able to work in a high-tolerance harm reduction model with clients who have multiple barriers.
5. Skill in non-violent crisis intervention.
6. Computer literate; able to effectively use computerized database for client file management, with basic
skills in Microsoft Word, Outlook, and Excel programs.
7. Detail oriented with strong time management, organizational, written, verbal, interpersonal, and computer
skills.
8. Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record.
9. Able to transport clients in your vehicle.
10. Able to obtain and maintain CPR/1st
Aid certification.
11. Minimum 30 WPM typing speed preferred.
