3. Đại cương
• Khớp cổ tay là khớp có cấu trúc và chức năng phức tạp.
• MRI cho hình ảnh cấu trúc vùng cổ bàn tay chi tiết, tương phản cao.
Nhiều quá trình bệnh lý đánh giá tốt trên MRI như: bệnh lý dây chằng,
chấn thương, họai tử vô mạch, hội chứng ống cổ tay.
• Kỹ thuật Thường sử dụng các mặt cắt axial, coronal, Sagittal. Các
chuỗi xung cơ bản là SE T1W, T2W, STIR, FSE T2W xóa mỡ. Khảo
sát có Gd đánh giá quá trình viêm, u…
4. Chỉ định chụp MRI ở khớp cổ tay
MRI ở khớp cổ tay được chỉ định trong các trường hợp:
1. Bất thường của sự phát triển của các yếu tố khớp
2. Chấn thương cổ tay
3. Viêm khớp
4. Viêm xương khớp
5. Hoại tử xương thuyền, xương nguyệt
6. Bệnh mô mềm của khớp cổ tay
7. Viêm túi khớp
8.Viêm bao gân và viêm gân
9. Hội chứng ống cổ tay
5. Chống chỉ định chụp MRI khớp cổ tay
Chống chỉ định tuyệt đối chụp MRI ở khớp cổ tay trong trường hợp:
1. Có đang đặt máy tạo nhịp tim, các phương tiện cấy ghép trong cơ thể.
2. Mảnh kim loại trong cơ thể người (mảnh đạn, mảnh bom...)
3. Bệnh nhân suy thận giai đoạn cuối hoặc quá mẫn cảm với các thành phần tác
nhân tương phản
Chống chỉ định tương đối chụp MRI ở khớp cổ tay:
1. Phụ nữ có thai 3 tháng đầu
2. Bị rối loạn tâm thần, sợ không gian kín
3. Trạng thái mất nước nặng
4. Có hình xăm với thuốc nhuộm chứa kim loại
5. Say rượu, ma túy
6. Người béo phì (>120kg)
6. PROTOCOL WRIST
•Coronal: PD, PD FS ( Hoặc T1W, STIR)
•Axial: PD, PDFS ( Hoặc T1W, STIR)
•Sagital: PD, PD FS ( Hoặc T1W, STIR).
• T1 FS có tiêm Tiêm thuốc áp dụng:

- Gân cơ gấp chung nông các ngón tay.
- Dây thần kinh giữa.
- Dây chằng ngang cổ tay.

23. Ghi nhớ
• Tại mặt gan cổ tay có 2 hội chứng quan trọng cần nhớ:
+ Hội chứng ống cổ tay ( Carpal Tunnel Syndrome): chèn ép dây thần kinh
giữa gây tê bì giảm vận động 3 ngón đầu ( ngón 1,2,3) mặt gan tay bàn tay.
+ Hội chứng kênh Guyon ( Guyon’s Canal Syndrome): chèn ép dây thần kinh
trụ gây tê bì giảm vận động ngón 5 và ½ ngón 4.
• Tại mặt mu cổ tay cần ghi nhớ 2 bệnh lý:
+ Viêm bao gân duỗi ngắn và dạng dài ngón cái ( De Quervain’s
Tenosynovitis)
+ Intersection Syndrome: do sự va chạm giữa các bao cơ vùng cẳng tay ( mặt
mu tay) gần cổ tay ( Cơ dạng ngón cái dài và cơ duỗi ngón cái ngắn- va chạm
với gân cơ duỗi cổ tay quay dài và gân cơ duỗi cổ tay quay ngắn.
24. Các tổn thương xương cổ tay
• Các tổn thương như họai tử vô khuẩn, gãy xương vùng cổ bàn tay dễ
phát hiện trên MRI.
• Các xương hay bị tổn thương trong họai tử vô khuẩn là xương thuyền
(sau gãy), xương nguyệt (Kienboch), xương cả và xương móc.
• Các xương thường gãy trong chấn thương gồm: xương thuyền, đầu
dưới xương quay, mỏm trâm trụ, nền xương bàn 1, gãy giật mặt gan và
mu tay các xương cổ tay, móc xương móc.

30. Phức hợp sụn sợi tam giác
(Triangular Fibrocatilage Complex)
• Sụn sợi tam giác là cấu trúc
giải phẫu có hình tam giác:
xòe rộng ra ở cạnh dưới của
khớp quay trụ dưới. Nó là một
phần của hệ thống sợi rộng
bám từ viền cổ tay của khuyết
trụ đầu dưới xương quay và
đến hòa lẫn với các dây chằng
quay trụ trước và sau, che phủ
xương nguyệt, tháp. Sụn sợi
tam giác có nền dầy từ 1-2
mm, đỉnh dầy 5-9 mm.

(Triangular Fibrocatilage Complex)
• Phức hợp sụn sợi tam giác
(Triangular Fibrocatilage
Complex): được cấu tạo bởi
nhiều cấu trúc trong khoang
cổ tay trụ. Chức năng tạo sự
ổn định bên trụ cổ tay và
khớp quay – trụ dưới, hấp thu
lực cơ học theo trục bên trụ
cổ tay. Sụn có hình tam giác,
giống như sụn chêm ở khớp
gối.

33. Chấn thương phức hợp dây chằng sụn sợi tam giác
cổ tay ( Rách TFCC)
• Những động tác vặn và xoay bàn tay, động tác nghiêng trụ và chịu lực như chơi
nhảy với xà, chơi bóng chuyền, té chống tay, khoan... có thể dẫn đến chấn thương
phức hợp dây chằng sụn tam giác vùng cổ tay.
• Tổn thương phức hợp dây chằng sụn tam giác thường gây đau vùng cổ tay bên trụ,
đau lan theo xương trụ, có thể nghe tiếng “click” vùng cổ tay khi vận độnggây
khó khăn khi cầm vật nặng.
• Khám: vận động cổ tay nghiêng về phía trụ và xoay cổ tay đau
Làm một số test: test ép TFCC, Piano Key test, press test, Grind test...
• Xquang và MRI:
- Xquang: Cho một cái nhìn tổng quát về hình dạng xương, và các tổn thương.
- MRI: Giúp chẩn đoán tổn thương sụn khớp, và phức hợp dây chằng sụn sợi tam
giác, sạn khớp,... Tuy nhiên khi có nhiều trường hợp khó, có thể tiêm cản quang
vào khớp để chẩn đoán chính xác hơn

• Chấn thương dây chằng sụn sợi
tam giác cổ tay (TFCC) khi các
dây chằng kết nối giữa xương trụ
và các xương khác trong cổ tay
bị rách do chấn thương hoặc cọ
mòn theo thời gian (hình dưới).

42. Phân loại rách chấn thương TFCC: 4 ĐỘ
• Độ 1A: Rách hoặc thủng phần đĩa
trung tâm
• Độ 1B: Bong nơi bám bên trụ, có
hay không gãy đầu xa xương trụ.
• Độ 1C: Bong nơi bám phức hợp
sụn sợi tam giác(PHSSTG) ở
xương nguyệt hoặc xương tháp.
• Độ 1D: Bong nơi bám PHSSTG ở
khuyết trụ đầu dưới xương quay.
• Điều trị rách TFCC:
Điều trị bảo tồn: (theo Dan C. Byck, MD)
- Độ 1A, 1B, 1C: mang nẹp 4-6 tuần, kháng
viêm, giảm đau, sinh tố, vật lý trị liệu.
- Một số trường hợp có thể tiêm Steroid.
Điều trị phẫu thuật:
- 1B, và khi điều trị bảo tồn thất bại:
Phẫu thuật:
- Cắt lọc, nếu rách vùng ít máu nuôi.
- Khâu: vùng có nhiều máu nuôi.

48. Types of TFCC tears

Type 1A is central TFC perforation. 1B, peripheral ulnar
side TFCC tear (± ulna styloid fracture). 1C, distal TFCC

1A tear is at the central or paracentral region of the
triangular fibrocartilage. It is usually situated at the
paracentral region, ~2–4 mm from the radial attachment;
1B is avulsion tear at the ulnar attachment with or without
an ulnar styloid fracture. The tear is either at the ulnar
styloid process attachment or the foveal attachment;
1C is avulsion tear at the distal attachment at the lunate or
triquetrum i.e., the ulnolunate and ulnotriquetral ligament
with or without avulsion fracture;
1D is avulsion tear at the radial attachment with or without
distal radial fracture.

During arthroscopy, a hook sign and trampoline effect are suggestive of unstable tear of the TFCC. Trampoline sign is positive when TFCC is
soft and lax while hook test is positive when the TFCC can be pulled upward and radially towards the center during the arthroscopy using the
probe. They are indicative of peripheral tear and foveal tear respectively; (D) cystic changes within the proximal and distal laminae of the TFCC
attachments (long solid arrows) due to intrasubstance partial tear; (E) oedematous and thickened fibres of the proximal and distal lamina (long
block arrows) due to high grade partial tear. Overall lamina continuity seems to be maintained. TFCC, triangular fibrocartilage complex.

arrow) to the foveal attachment (solid arrow). This is due to a combination of Palmar classification type 1A and type 1B tears; (B) coronal proton-density fat suppressed MR
image shows chronic avulsion fracture at the base of distal ulnar styloid process (solid long arrow). Associated with abnormal high signal in the attachment of TFCC (solid
arrowhead). Features are compatible with Type 1B tear. There is also a small linear tear at the paracentral portion of the TFC (block long arrow) compatible with type 1A tear;

82. Hội chứng rách dây chằng thuyền- nguyệt
(Scapholunate Ligament Tear Syndrome)
• Hội chứng rách dây chằng thuyền- nguyệt (Scapholunate Ligament Tear
Syndrome) có nguyên nhân do chấn thương, hoặc hiếm hơn là do các biến đổi
thoái hóa ở cổ tay.
• Dây chằng thuyền – nguyệt có tác dụng như một bộ ổn định các lực xoay mặt
gan tay xương thuyền chống lại lực xoay phía mu của xương nguyệt. Dây chằng
còn có tác dụng duy trì khoảng cách giữa xương thuyền và xương nguyệt. Giữ
cho cực dưới của xương thuyền ở vị trí tương ứng thích hợp so với xương
nguyệt.
• Quá trình thoái hóa phức hợp dây chằng thuyền – nguyệt xảy ra như một phần
của quá trình tự nhiên theo tuổi, bắt đầu vào thập niên 30 của đời người. Qúa
trình thoái hóa này dẫn đến phức hợp dây chằng thuyền – nguyệt có thể tổn
thương do chấn thương. Các chấn thương thường gặp gây rách dây chằng
thuyền – nguyệt bao gồm các chấn thương do ngã khi cổ tay bị duỗi quá mức.
nếu chỉ gây rách một phần, bệnh nhân có biểu hiện đau phía quay – mu của cổ
tay. Nếu như rách hoàn toàn, đau thường đi kèm với mất vững cổ tay. Một vài
bệnh nhân nói có tiếng lách cách khi lệch cổ tay từ phía trụ sang phía quay.
83. Chẩn đoán Hội chứng rách dây chằng thuyền-
nguyệt (Scapholunate Ligament Tear Syndrome):
• Thăm khám lâm sàng ở bệnh nhân bị rách dây chằng thuyền-nguyệt sẽ xuất hiện đau khi cổ tay
lệch trụ, triệu chứng đau tăng lên khi bệnh nhân nắm chặt bàn tay, gây ra lực căng lên các xương
cổ tay. Đau xuất hiện khi sờ nắn vùng hố lào giải phẫu, và nhận thấy có sự rộng khoang giữa
xương thuyền và xương nguyệt.
• Người khám có thể có cảm giác lách cách khi thực hiện một số cử động. Test Watson dương tính
khi cổ tay chuyển động từ tư thế bên trụ sang bên quay khi bệnh nhân nắm chặt bàn tay. Nếu không
được điều trị, quá trình thoái hóa các khớp quay- thuyền, các khớp ở cổ tay, khớp quay-nguyệt do
hậu quả của sự biến dạng gây xẹp thuyền-nguyệt sớm, còn gọi là cổ tay SLAC (scapholunate
advanced collapse).
• Chụp X quang thường quy được chỉ định cho tất cả các bệnh nhân bị hội chứng rách dây chằng
thuyền-nguyệt để loại trừ các quá trình bệnh lí xương tiềm ẩn, để xác định các đặc điểm như sự
rộng ra của khoảng thuyền-nguyệt (còn được gọi là dấu hiệu Terry Thomas dương tính hay David
Letterman dương tính, theo đặc điểm rộng khoảng cách giữa hai xương này), đặc điểm xương
thuyền gấp về phía gan tay, xương nguyệt gấp về phía mu, hay còn được gọi là sự phân li thuyền-
nguyệt với sự mất vững xen kẽ nhau phía mu cổ tay.
• Chụp cộng hưởng từ cổ tay được chỉ định cho tất cả các bệnh nhân nghĩ đến rách dây chằng
thuyền-nguyệt hoặc nếu nghi ngờ các nguyên nhân khác gây khớp mất vững, nhiễm khuẩn hoặc
khối u.
84. Chẩn đoán phân biệt
• Các bệnh lí đi kèm như viêm khớp và gout ở các khớp quay-trụ,
các khớp cổ tay, bàn tay và các khớp liên đốt, bạch mu cổ tay,
viêm bao gân hoạt dịch De Quervain và viêm gân có thể đi kèm
với hội chứng rách dây chằng thuyền-nguyệt và làm trầm trọng
thêm triệu chứng đau và khuyết tật ở bệnh nhân. Bệnh
Kienblock, hoại tử vô mạch xương thuyền, gẫy xương thuyền
cũng có thể giả triệu chứng đau giống trong rách dây chằng
thuyền-nguyệt.

• Bệnh lý Kienbock là tình trạng hoại tử xương nguyệt (1 xương nhỏ ở cổ tay) do tình trạng cấp máu
bị ảnh hưởng. Sự cấp máu xương bị gián đoạn làm cho xương rơi vào tình trạng thiếu máu và hoại
tử nên còn gọi là hoại tử vô mạch của xương.
• Tổn thương xương nguyệt dẫn đến tình trạng đau cổ tay, hạn chế vận động cổ tay lâu dài dẫn đến
tình trạng thoái hoá khớp.
• Nguyên nhân của Kienbock chưa được hiểu rõ. Một số tác giả cho rằng tình trạng chấn thương cổ
tay là 1 yếu tố thuận lợi. Một số yếu tố nguy cơ khác như cấu trúc giải phẫu mạch máu, sự bất cân
đối giữa chiều dài 2 xương cẳng tay,...
• Một số triệu chứng của bệnh Kienbock như:
- Đau và có thể sưng nề cổ tay
- Hạn chế vận động cổ tay
- Giảm sức cầm nắm của bàn tay
• Ở những giai đoạn sớm của bệnh, khá khó khăn cho chẩn đoán bệnh Kienbock vì các triệu chứng
tương đối giống với 1 tình trạng chấn thương cổ tay hoặc viêm màng hoạt dịch bao gân. Các
phương tiện chẩn đoán thông thường như x quang, siêu âm thường không phát hiện được tổn
thương, muốn phát hiện được chỉ có dựa vào phim chụp cộng hưởng từ.

94. Tiến triển của bệnh kienbock diễn ra qua 4
giai đoạn: AVN Lunate Stage
- Ở giai đoạn đầu, các triệu chứng lâm sàng mơ hồ, hình ảnh x quang chưa
phát hiện gì bất thường, siêu âm có thể bình thường hoặc cũng có thể thấy
1 số hình ảnh gián tiếp như phù nề hoặc ít dịch khớp hoặc viêm bao hoạt
dịch gân duỗi. Phim cộng hưởng từ có thể gợi ý tổn thương với biểu hiện
thiếu máu của xương nguyệt.
- Giai đoạn 2: Biểu hiện thiếu máu xương nguyệt rõ hơn. Trên x quang
xương nguyệt có thể biểu hiện tăng cản quang với hình ảnh trắng hơn và
sáng hơn trên phim. Việc đánh giá bằng phim cộng hưởng từ ở giai đoạn
này là thực sự cần thiết. Về triệu chứng lâm sàng, các biểu hiện đau, sưng
nề và hạn chế vận động có biểu hiện thường xuyên hơn.
- Giai đoạn 3: do thiếu máu nuôi, cấu trúc giải phẫu của xương nguyệt bị
phá vỡ, có thể quan sát tương đối dễ dàng trên phim x quang.
- Giai đoạn 4: biểu hiện thoái hoá khớp cổ tay.

97. Điều trị hoại tử vô mạch xương nguyệt cổ tay
(AVN Lunate)
Điều trị: chưa có biện pháp nào điều trị hoàn hảo cho tình trạng hoại tử xương nguyệt, tuy nhiên có 1 vài lựa chọn điều trị
cho bệnh nhân nhằm mục đích giảm áp lực lên xương nguyệt và tăng tưới máu nuôi xương
- Điều trị không phẫu thuật: ở giai đoạn sớm có thể dùng thuốc giảm viêm giảm nề kèm theo các biện pháp bất động cổ
tay. Nếu các triệu chứng không cải thiện, có thể sẽ cân nhắc phương án phẫu thuật.
- Điều trị phẫu thuật: có 1 vài kỹ thuật có thể được thực hiện tuỳ theo mức độ tổn thương, nhu cầu vận động, mong muốn
của bệnh nhân:
+ Tái cấp máu cho xương (revascularisation) bằng các phẫu thuật chuyển vạt xương có cuống mạch.
+ Nâng diện khớp (joint leveling): khi 2 xương cẳng tay có bất thường về chiều dài và do đó làm tăng áp lực lên xương
nguyệt. Việc nâng diện khớp có thể được thực hiện bằng cách làm dài xương với ghép xương hoặc làm ngắn xương.
+ Lấy bỏ hàng tụ cốt thứ nhất cổ tay (proximal row carpectomy): nếu tổn thương xương nguyệt nặng, xương nguyệt bị vỡ,
lúc này phẫu thuật lấy bỏ mảnh vỡ là cần thiết, cùng với đó là lấy bỏ ít nhất 2 xương lân cận, thủ thuật này gọi là Lấy bỏ
hàng tụ cốt thứ nhất. Phẫu thuật này giúp giảm đau nhưng vẫn duy trì được biên độ vận động của cổ tay.
+ Hàn khớp cổ tay: giúp giảm đau tuy nhiên làm giảm biên độ vận động của khớp cổ tay. Hàn khớp có thể 1 phần hoặc
hoàn toàn nếu tình trạng thoái hoá khớp cổ tay nặng.
+ Thay thế xương nguyệt: là 1 hướng điều trị tuy nhiên khá khó khăn.
98. Kienböck's disease is characterized by avascular necrosis of the lunate bone (Figure 3). It is an insidious condition that affects the dominant wrist
of young adults and is related to repetitive microtrauma(5). The most common symptoms are pain in the dorsal surface of the wrist, mild edema,
stiffness, and clicking(5). Approximately 75% of cases have negative ulnar variance, which is defined as an ulna that is abnormally shorter than the
radius(5). Conservative treatment is highly effective in mild cases. As the disease progresses, there is sclerosis and fragmentation of the lunate.
The most common surgical procedure used for the correction of negative ulnar variance is radial shortening. Proximal row carpectomy is a
salvage procedure for refractory cases

• Thực tế làm việc tôi nhận thấy rằng khi gặp 1 chấn thương vùng cổ bàn tay,
việc tìm được 1 tài liệu viết về MRI đầy đủ, chi tiết và dễ hiểu nhất thật sự
khó khăn.
• Các bản dịch thường ngắt quãng, không liền mạch và chưa có mối tương
quan chặt chẽ giữa lâm sàng và hình ảnh.
• Bài viết còn nhiều vấn đề tôi chưa hài lòng do hiểu biết hiện tại của tôi về
vấn đề này không nhiều và tôi chưa gặp được 1 bs lâm sàng chấn thương cơ
