Montgomery Cares: Key Accomplishments System Development and Integration (Montgomery Cares) Developed system from 5 primary care clinic organizations to 12 organizations with 16 fixed locations and 20 +  mobile van sites. (2 FQHC) Increased capacity from 8,000 patients (FY02) to 23,000 uninsured patients (FY09). CHLCare Web-based, Open Source, shared, electronic medical record in 7 clinics+; connect to ED Medicine Access Point of service (32) generic medicine; (30) PAP. $82/person/year. Specialty Care Access Programs Automated referral process, volunteer specialist networks, hub clinics, hospital-based services, and informal relationships. 3,000 referrals in FY 2008 Behavioral Health Care Collaborative care model, providing on-site mental health assessments, medication management, short-term therapy and referrals at three Montgomery Cares clinics. 490 patients in FY 2008 Oral Health Care Linked dental care to primary health care through direct referrals from all Montgomery Cares clinics to dental services at 2 sites. 625 patients in FY 2008 Health Care Services for Homeless Adults Primary care and supports services for homeless adults. 756 patients in FY08. Information & Referral Program I &R program providing assistance to nearly 2,000 callers in FY08. Population health activities IHI Triple Aim; reduce avoidable ED visits at the 5 hospitals; breast cancer screening;  Financial support $11 million annual County; $20 million grants since 2001.
