This study identified a novel heterozygous mutation, c.525G>C (Q175H), in the VSX1 gene in one individual with keratoconus out of 66 patients studied. Bioinformatics tools predicted this mutation would be potentially damaging. This adds to evidence that VSX1 plays a role in keratoconus, though its exact pathogenic mechanism requires further investigation given the low frequency of this mutation. This is the first report of a VSX1 mutation identified in keratoconus patients from India.

Objective: To evaluate the clinical and demographic characteristics of patients with idiopathic and non-idiopathic
vocal cord paralysis (VCP).
Study Design: Descriptive cross sectional study.
Place and Duration of Study: Department of ENT Combined Military Hospital Quetta and Rawalpindi, from 10
Dec 2012 to 31 Dec 2015.
Material and Methods: The study was a descriptive cross sectional study. The study was conducted after
approval by the ethical committee. Patients with fixed vocal cords due to some growth of glottic region were
enrolled. All the patients presenting with hoarseness of voice in ENT outpatient department CMH Quetta and
Rawalpindi undergoing indirect laryngoscopy and the patients with vocal cord paralysis were selected. Informed
written consent was taken and gender, age, name, hospital record number, address and phone number of each
individual was noted. Every patient was evaluated by detailed history and thorough clinical examination.
Patients were not investigated further if cause were revealed after some investigation. Follow-up of patients was
done regularly in ENT OPD. CT scans/US neck was done by radiologist and FNAC/biopsy was reported by
histopathologist. Data collected were recorded on proforma.
Results: In our study, out of 245 cases, 47.76% (n=117) were 16-40 years old and 52.24% (n=128) were 41-80 years,
mean ± SD was calculated as 41.23 ± 11.25 years, 45.71% (n=112) male and 54.29% (n=133) were females.
Frequency of causes of vocal cord paralysis was recorded as 15.92% (n=39) for idiopathic, 46.53% (n=114) had
iatrogenic, 33.06% (n=81) had malignant neoplasm while 4.49% (n=11) had radiation.
Conclusion: Vocal cord paralysis is a common clinical condition with substantial morbidity. Awareness on the
clinical characteristics and identification of the underlying etiology are keystones for foreseeing complications
and determining the required therapeutic modality.

ABSTRACT- Nocardial brain abscesses are a rare central nervous system infection with high morbidity and mortality.
Infection is acquired through inhalation or direct inoculation which then spreads hematogenously to other organs. They
are usually associated with immunocompromised patients but may appear in otherwise healthy individuals. They should
be considered in the differential diagnosis of brain abscesses. Serological tests are not useful. Culture reports are
important in establishing a diagnosis. Early diagnosis, evacuation and long term treatment might help in reducing
significant morbidity and mortality. Here we present a case of nocardial cerebellar abscess in an immunocompetent male.

Isolated central nervous system tuberculosis is uncommon in immunocompetent patients. It resembles a pyogenic brain abscess
clinically and radiologically and poses a problem in diagnosis and treatment. Here we described a case of recurrent frontal lobe
abscess, which was diagnosed as a tubercular abscess. There was no clinical or radiological evidence of active tuberculosis
elsewhere in the body. The diagnosis of tubercular abscess was confirmed by Mycobacterium tuberculosis by Polymerase Chain
Reaction (TB-PCR) in the abscess material aspirated via a burr hole.

Vascular endothelial growth factor-A (VEGF-A) has been observed as the predominant angiogenic factor in colorectal cancer (CRC) and the assessment of microvessel density (MVD) has been used to quantify tumor neoangiogenesis. This study aimed to determine clinicopathological and prognostic significance of both angiogenic markers in the local CRC patients. We analyzed tissue samples obtained from 81 cases with CRC. VEGF-A expression and MVD counts were immunohistochemically detected using anti VEGF-A and CD31. The assessments of both markers were classified as low and high. Correlation between VEGF-A expression and MVD value and clinicopathological characteristics were examined using Chi-square test. The overall survival (OS) was plotted using the Kaplan-Meier method. The results indicated a high VEGF-A expression was found more frequently in the rectal location (P=0.042) and T4 tumors (P=0.041) compared to their counterparts. Older patients tended to show a higher MVD value compared to younger cases (P=0.062). In addition, survival analysis showed that males had a worse OS compared to females (P=0.029), and VEGF-A expression and MVD count did not correlate with patients’ survival. In conclusion, there were significant differences of VEGF-A expression according to tumor location and T invasion. Sex, but not angiogenic markers, had an influence on the survival of CRC patients.

A female patient aged 6 years, a suspected case of sickle cell trait (SCT) having symptoms of Rheumatoid arthritis (RA),
while evaluating joint complaints in adult sickle cell disease (SCD) patients, a number of sickle cell-based entities come
to mind such as avascular necrosis, osteomyelitis, bone infarcts, and septic arthritis. RA is a chronic systemic
inflammatory disease, many reports highlighted the occurrence of RA in SCD presenting as diagnostic challenges for
cases with chronic inflammatory arthritis, SCT also have appeared to persist in some populations at a perplexingly high
rate given the degree of early mortality of homozygosity of SCD, our case report showed that not only SCD but if a patient
has SCT they can develop RA as complication. Our case report concludes that during the evaluation of a SCT patient who
presents with chronic synovitis, one should strongly consider the possibility of coexistence of RA and SCT.
