This document outlines a mental health reentry improvement initiative with the following objectives: 1. Improve continuity of care planning for mentally ill offenders being paroled or completing their maximum sentences, with or without special probation. 2. Increase paroling rates by improving current practices related to mentally ill offenders. 3. Improve supervision practices for mentally ill offenders on parole. The initiative focuses on Axis 1 mentally ill offenders who are least likely to complete their maximum sentences. It establishes reentry teams and specialists to coordinate care between corrections, parole, and county mental health authorities. It also aims to improve communication, supervision practices, and flexibility in parole decisions for this population.
