2. In connection with above reference, please be informed that the undersigned could not attend
the said inspection as scheduled because her flight/trip going to ____________ was likewise scheduled on
the same day (December 5-8, 2012) as the schedule conduct of AGI-ORSITE at PNP, HS.
3. Moreover, please be informed that undersigned already sought permission from her
immediate superior and to allot her the said dates as her official day-off for reason as mentioned above.
Hence, the undersigned has really no intention of not attending the said inspection and therefore seek
before your good Office for consideration hereof.
4. Request acknowledge receipt.
