A 76-year-old man was admitted multiple times with confusional episodes. Neurological exams and tests were normal until a blood test found severe hypomagnesaemia. The patient had been taking esomeprazole long-term, which can cause low magnesium levels. Magnesium treatment resolved his symptoms. Long-term proton pump inhibitor use should prompt magnesium testing, as it can have neurological and cardiovascular effects.
