WORK EXPERIENCE
RN Anticoagulation Clinic January 2011-present
Responsibilities:
Start up an anticoagulation clinic in the LaCrosse Skemp clinic to manage
Warfarin/INR’s on all patients requiring anticoagulation. To standardize
the management of patients on warfarin to ensure consistency and safe
practice for all patients across the organization. Required learning proper
sampling technique, use of ISTAT, developing dosing protocols, etc.
Requires evaluation of patient at each visit and anything that relates/could
interact with their warfarin. Requires teaching about risks/reason taking/
adverse affects and how to manage/potential interactions and the meaning of
their INR values. Requires critical thinking, organization, excellent
communication skills, independent working though work with team of other
anticoag nurses/physicians.
RN PATT, Gundersen Lutheran hospital March ‘07 to 12/31/10
Responsibilities:
Assess, plan and intervene in the perioperative setting by phone or in person
patients who are planning to have surgery and complete their pre-surgical
teaching and medication instruction to ensure they are adequately prepared and
safe to proceed with surgery.
RN General Internal Medicine, Gundersen Lutheran clinic Sept ’04 to March ‘07
Responsibilities:
Assess, plan and intervene in clinic setting via phone contacts and assist patients
who present to department as “walkin”. Assist practitioners as needed during
patient appointments (i.e. wound care, straight cath, etc). Provide patient
education on variety of issues including diet, medication, smoking, coumadin.
Identify and assist with care coordinators as needs arise by bringing in homecare
services, social services, and care coordinators. I am worked the internal
medicine dept through the LEAN process to bring three internal medicine
departments together and begin a coumadin clinic.
RN Pediatrics, Gundersen Lutheran hospital January 1997 to Sept ‘04
Responsibilities:
Assess, perform interventions necessary to patient care, following standards of
care and care plans specific to patient and evaluating interventions performed.
Admit patient to unit as assigned.
2. Case manager for Extended Care, La Crosse Visiting Nurses Assoc July ’94 to Jan ‘97
Responsibilities:
Take referrals for Medicare, Medicaid, private insurance and private pay. Admit
clients to LVNA Extended Care (“private duty”—patients who require long
blocks of care by home health aides or nursing staff). Supervise and train staff in
the homes I case managed. Set up nursing care plans and assignments for staff to
follow based on physician orders per state/federal regulations.
Complete POC and prior authorizations for MD to sign. Coordinate care and
activities of my clients between DME, other agencies and therapists. Document
all coordination of activities, conversations, etc. Review all documentation by
staff in my assigned homes. From Jan ’97 to Jan’98, I did not function as case
manager but remained in an on-call status. I functioned as a field nurse in one of
the pediatric patient’s home. In this role, I would provide shift care to patient,
including physical assessment, nursing interventions (trach care, GT care and tube
feedings, administer meds, ROM, suction trach, etc).
This position required excellent organizational and time management skills, as
well as an ability to work under stress and demonstrate good communication
skills with referral sources, care providers involved in the case, staff, parents
(most of our clients were special needs children). Knowledge of Medicaid
regulations and payment system under Medicaid was needed.
