This letter is provided at the written request of Charles Frank who asked me to serve as a reference on his
behalf. It is my understanding that Charles is being considered for a scholarship opportunity. Please be
advised that the information contained in this letter is confidential and should be treated as such. The
information should not be disclosed to anyone in your organization who would not be involved in the
scholarship decision regarding this individual. Additionally, the information should not be disclosed to
anyone outside of your organization without the consent of the student.
I have known Charles for the past ten months as his Clinical I and II instructor. As his instructor, I have
had an opportunity to observe Charles's participation and interaction in class, in lab and in the hospital
setting. His grasp of basic nursing skills and subject matter is excellent.
He has been successful in meeting the course objectives for Clinical I and II as well as his other required
nursing classes. Charles attends all classes and completes the necessary work in a timely manner toward
successful course completion.
Charles is conscientious, caring, and respectful. He is assertive, communicates well and displays a
positive and inquisitive attitude in the classroom as well as the hospital setting. He demonstrates
leadership qualities as evidenced by his role of President in the University of OK student nurse
association as well as his role of leadership in the teaching and coordination of CPR classes offered on
campus. This is a fundraising effort to offset the cost of graduation activities and Charles is a key player.
Based upon the student's academic performance, I believe Charles is very deserving of this scholarship
opportunity.
