This document outlines a case study for a Lean Six Sigma Green Belt certification course, focused on the define phase of the DMAIC process to address billing inaccuracies in a forklift procurement system. The project involves collecting customer feedback to identify issues such as double billing, incorrect charges, and discrepancies in inventory records. The outcomes of the define phase include establishing a team charter, documenting the problem, and creating necessary tools like SIPOC and CTQ trees to facilitate improvement.
