This document outlines a project to recruit 5 Georgia congregations to join the 100 Congregations for Million Hearts initiative by December 2015. The initiative aims to prevent 1 million heart attacks and strokes by 2017 through raising awareness of heart disease risks like hypertension. The project goals are to designate advocates in congregations to provide heart health resources, establish community partnerships, disseminate prevention messages, and track blood pressure. The methodology involves providing technical assistance to congregations. Effectiveness will be evaluated by measuring awareness improvements and potential decreases in heart events. The project is federally funded and supported.
