Januvia Tablets (Sitagliptin Phosphate Monohydrate Tablets)
Januvia Tablets (Sitagliptin Phosphate Monohydrate Tablets) are
indicated as an adjunct to diet and exercise to improve glycaemic
control in adults with type 2 diabetes mellitus as monotherapy
when metformin is considered inappropriate due to intolerance; or
in combination with other anti-hyperglycaemic agents, including
insulin.
Controlling high blood sugar helps prevent kidney damage,
blindness, nerve problems, loss of limbs, and sexual function
problems. Proper control of diabetes may also lessen your risk of a
heart attack or stroke. Sitagliptin is a diabetes drug that works by
increasing levels of natural substances called incretins. Incretins
help to control blood sugar by increasing insulin release, especially
after a meal. They also decrease the amount of sugar your liver
makes.

Januvia Tablets (Sitagliptin Phosphate Monohydrate Tablets)
Uses of Januvia Tablets
Januvia (Sitagliptin Phosphate Monohydrate Tablets) is used alone
or together with other medicines (eg, Insulin, Glimepiride,
Metformin, or Pioglitazone) and with proper diet and exercise to
treat high blood sugar levels caused by type 2 diabetes.
Sitagliptin helps to control blood sugar levels by increasing
substances in the body that make the pancreas release more insulin.
It also signals the liver to stop producing sugar (glucose) when there
is too much sugar in the blood.
This medicine does not help patients who have insulin-dependent or
type 1 diabetes. Januvia Tablets have not been studied in patients
with a history of pancreatitis.

Januvia Tablets Dosage
The dosage of Januvia tablets (Sitagliptin Phosphate Monohydrate
Tablets) is given below:
The recommended dose of Januvia tablets (Sitagliptin Phosphate
Monohydrate tablets) is 100 mg once daily. Januvia can be taken
with or without food.
Dosage adjustment is recommended for patients with moderate or
severe renal insufficiency or end-stage renal disease.

Contraindications
Januvia is contraindicated in patients with a hypersensitivity to
Sitagliptin Phosphate or any other component of this
medication.

Storage Instructions
Januvia (Sitagliptin Phosphate Monohydrate
Tablets) have to be stored at controlled room
temperature i.e. from 20°C to 25°C (68°F to
77°F) with excursions permitted between 15°
to 30°C (59° to 86°F).
Keep this as well as all other medicines away
from children and pets.

Warnings
Pancreatitis: There have been postmarketing reports of acute pancreatitis,
including fatal and non-fatal hemorrhagic or necrotizing pancreatitis. If
pancreatitis is suspected, promptly discontinue Januvia (Sitagliptin
Phosphate Monohydrate Tablets).
Heart failure: Heart failure has been observed with two other members of
the DPP -4 inhibitor class. Consider risks and benefits of Januvia in
patients who have known risk factors for heart failure. Monitor patients
for signs and symptoms.
Acute Renal Failure: Has been reported postmarketing, sometimes
requiring dialysis. Assessment of renal function is recommended prior to
initiating Januvia and periodically thereafter.

Warnings
Hypoglycemia with Concomitant Use with Insulin or Insulin
Secretagogues: Increased risk of hypoglycemia when used in combination
with insulin and/or an insulin secretagogue. Lower dose
of insulin or insulin secretagogue may be required .
Hypersensitivity Reactions: There have been postmarketing reports of
serious allergic and hypersensitivity reactions in patients treated with
Januvia such as anaphylaxis, angioedema, and exfoliative skin conditions
including Stevens-Johnson syndrome. Promptly stop Januvia, assess for
other potential causes, institute appropriate monitoring and treatment.

Warnings
Severe and Disabling Arthralgia: Has been rep orted in patients taking
DPP-4 inhibitors. Consider as a possible cause for severe joint pain and
discontinue drug if appropriate.
Bullous Pemphigoid: There have been postmarketing reports requiring
hospitalization in patients taking DPP-4 inhibitors. Tell patients to report
development of blisters or erosions. If bullous pemphigoid is suspected,
discontinue Januvia.
Januvia has no or negligible influence on the ability to drive and use
machines. However, when driving or using machines, it should be taken
into account that dizziness and somnolence have been reported.
In addition, patients should be alerted to the risk of hypoglycaemia when
Januvia is used in combination with a sulphonylurea or with insulin.

Januvia Tablets Side Effects
Side effects reported in 5% or more of patients treated
with Sitagliptin Phosphate Monohydrate (Januvia tablets)
and more commonly than in patients treated with placebo
are: upper respiratory tract infection, nasopharyngitis and
headache. In the add -on to sulfonylurea and add-on to
insulin studies, hypoglycemia was also more commonly
reported in patients treated with Januvia compared to
placebo.

Januvia Tablets (Sitagliptin Phosphate Monohydrate Tablets)
Overdose
In the event of an overdose with Sitagliptin Phosphate Monohydrate
(Januvia tablets), contact the Poison Control Center. In the event of an
overdose, it is reasonable to employ supportive measures, e.g., remove
unabsorbed material from the gastrointestinal tract, employ clinical
monitoring (including obtaining an electrocardiogram), and institute
supportive therapy as dictated by the patient's clinical status.
Sitagliptin is modestly dialyzable. In clinical studies , approximately
13.5% of the dose was removed over a 3- to 4-hour hemodialysis session.
Prolonged hemodialysis may be considered if clinically appropriate. It is
no t known if sitagliptin is dialyzable by peritoneal dialysis.

Januvia Tablets During Pregnancy
Sitagliptin Phosphate Monohydrate (Januvia tablets) has been
classified by the US FDA as Pregnancy Category B.
Reproduction studies have been performed in rats and rabbits.
Doses of Sitagliptin Phosphate Monohydrate (Januvia tablets) up
to 125 mg/kg (approximately 12 times the human exposure at the
maximum recommended human dose) did not impair fertility or
harm the fetus.
There are, however, no adequate and well-controlled studies in
pregnant women. Because animal reproduction studies are not
always predictive of human response, this drug should be used
during pregnancy only if clearly needed.

Sitagliptin helps to control blood sugar levels by increasing substances in the body that make the pancreas release more insulin. Consider as a possible cause for severe joint pain and discontinue drug if appropriate. Bullous Pemphigoid: There have been postmarketing reports requiring hospitalization in patients taking DPP-4 inhibitors. Tell patients to report development of blisters or erosions. If bullous pemphigoid is suspected, discontinue Januvia. Januvia has no or negligible influence on the ability to drive and use machines. However, when driving or using machines, it should be taken into account that dizziness and somnolence have been reported. In addition, patients should be alerted to the risk of hypoglycaemia when Januvia is used in combination with a sulphonylurea or with insulin. In the event of an overdose, it is reasonable to employ supportive measures, e.g., remove unabsorbed material from the gastrointestinal tract, employ clinical monitoring (including obtaining an electrocardiogram), and institute supportive therapy as dictated by the patient's clinical status. Sitagliptin is modestly dialyzable. In clinical studies , approximately 13.5% of the dose was removed over a 3- to 4-hour hemodialysis session.
