2. What is an Intern?
A person who has successfully
completed his or her second year in,
and is enrolled at,
an accredited school of pharmacy and
whose practice of pharmacy is limited to
performing duties under the
direct supervision of a person
licensed as a pharmacist by the board.

4. Student Non-Academic Internships

“The supervising pharmacist shall keep a written record of
the hours and location worked by an intern …
signed by the intern and the supervising pharmacist.”
In Wisconsin, you can practice pharmacy as an intern
for no internship credit.
5. Documentation
• Supervising pharmacist keeps record of hours and
location worked
• You and your supervising pharmacist must sign the written record
• No limit on the number of hours you can earn
• Can complete at multiple locations
• Certification form submitted at the time of original
licensure
• Form #2535 from WI DSPS for internships in WI
• Form #2537 from WI DSPS for internships outside WI

6. Things to Remember this Summer…
• Check other state’s rules/regulations
• Search for their Board of Pharmacy or Pharmacy Intern websites
• Work with supervising pharmacist on documentation
• Hours / location
• Need pharmacist and intern signatures
• Complete Certification Form at the end of the summer
• YOU are responsible for keeping these forms until you apply for a
license
• DO NOT send to Pharmacy Examining Board until you apply for a
pharmacist license
Best to do this at the time instead of
waiting until graduation!
